About this file
This is a Conflict of Interest Questionnaire form provided by the Texas Ethics Commission for vendors doing business with local governmental entities in Texas, specifically applicable to Collin County's construction project for sprinkler head replacement at the Collin County Jail. The form requires vendors to disclose potential conflicts of interest pursuant to Chapter 176 of the Texas Local Government Code. Vendors must file the completed questionnaire with the records administrator of the local governmental entity no later than the seventh business day after the vendor becomes aware of facts requiring disclosure or after initiating discussions regarding a potential contract. The questionnaire requires disclosure of any employment or business relationships between the vendor and local government officers or their family members that result in taxable income exceeding $2,500 during the preceding 12-month period, gifts with aggregate value exceeding $100 provided to officers or their families within the preceding 12-month period, or any family relationships with local government officers.
The form contains sections requiring vendors to identify themselves, name the specific local government officer involved, describe employment or business relationships, and provide information regarding any connections with corporations or entities in which the officer holds officer, director, or one percent or more ownership positions. Vendors must certify their responses by signature and date. The questionnaire may be filed as an update to a previously submitted form if the original submission was incomplete or inaccurate. Failure to comply with the disclosure requirements constitutes a misdemeanor offense under Texas law. The form applies to the Collin County construction contract valued at $150,000 for replacing institutional sprinkler heads in the county jail facility.
