About this file

This document is a Bid Cover Sheet for Invitation to Bid (ITB) #S-73000-00013125 issued by the Oregon Department of Transportation (ODOT). The cover sheet serves as a comprehensive bidder registration and certification document for potential contractors, requiring detailed company information, qualifications, experience, and legally mandated certifications. While specific project details are not provided in this document, the form is designed to collect critical vendor information, including business structure, contact details, employee count, years in business, and references for comparable projects.

The cover sheet requires bidders to affirm multiple legal and ethical compliance standards, including non-discrimination practices, tax law adherence, conflict of interest disclosures, and verification of not being debarred or suspended from government contracting. Notable certification requirements include having a written policy preventing sexual harassment and discrimination, being an independent contractor, and confirming no violations of specified tax laws. Bidders must provide three references for comparable projects, detailing firm names, contact information, project descriptions, and dates, which will presumably be used to evaluate the vendor's qualifications and past performance.
