About this file
This is a Proposal Cover Sheet for a Mini-RFP issued by the Oregon Department of Transportation for the Cape Arago Highway Corridor Plan project, which spans from Coos Bay to Charleston. The document serves as the formal submission vehicle for interested firms to propose services related to this corridor planning initiative. While the cover sheet itself does not specify detailed project scope, deliverables, quantities, or specific dates, it establishes the Mini-RFP number as 73000-00012193 and requires proposers to identify their firm name, primary contact information, and demonstrate how they meet any stated minimum requirements outlined in the base Mini-RFP document.
The cover sheet contains mandatory certifications that all proposers must complete and have authorized representatives execute, including attestations regarding receipt of all addenda, truthfulness of proposal contents, non-discrimination in subcontracting practices, compliance with sexual harassment and discrimination prevention policies for contracts exceeding $150,000, absence of delinquent state debt, acceptance of all terms and conditions, and conflict of interest disclosures. Proposers must certify either that no conflicts of interest exist or provide detailed disclosures as required by ODOT's Conflict of Interest Guidelines. The authorized representative's signature and approval of the cover sheet binds the proposing firm to all representations, attestations, and certifications contained within the proposal, establishing legal accountability for the submission.
