About this program

The Department of Health and Human Services Centers for Disease Control and Prevention funds state and local health department HIV prevention and surveillance programs under Assistance Listing 93.940, with the most recent award made on September 30, 2025. The program supports state, local, and territorial health departments, including American Indian and Alaska Native tribal governments, in establishing and maintaining comprehensive HIV prevention and surveillance systems that monitor prevalence and incidence, target prevention activities, and measure program effectiveness across the United States and its territories.

Grant awards under this program have remained active and steady. The portfolio spans 936 total awards from 2001 to September 2025, with annual award volume averaging around 54 awards in recent years—82 in 2021, declining to 34 in 2025. Combined federal dollars obligated total $17.21 billion across the portfolio lifetime. Recent awards include cooperative agreements funding the Illinois Department of Public Health Medical Monitoring Project ($3.7 million), Texas Department of State Health Services TB elimination activities ($27.9 million), San Francisco Department of Public Health Medical Monitoring Project ($643,882), and New York City TB program activities ($20 million). The program also funds smaller awards in the $500,000 to $2 million range across states such as Hawaii, Kentucky, and Louisiana for TB control and HIV prevention.

Funding is concentrated almost entirely within CDC; approximately 928 of 936 awards (99 percent) flow through the CDC National Center for HIV, Viral Hepatitis, STD, and TB Prevention. The assistance-type distribution is predominantly Cooperative Agreements (87 percent, 811 awards) with smaller shares of Project Grants (13 percent, 119 awards) and Block Grants (1 percent, 6 awards). Awards typically range from a 12-month budget period with performance periods up to five years, subject to availability of funds and satisfactory progress. Non-federal cost-share contributions total $310 million against $17.21 billion in federal obligated dollars.
