About this program
The Department of Health and Human Services Centers for Disease Control and Prevention funds community-based HIV prevention programming through CFDA 93.939, with the most recent award disbursed July 1, 2025. This Assistance Listing supports nonprofit organizations—local, regional, and national—to develop and implement community-based HIV prevention programs aligned with national goals, coordinate prevention efforts across multiple agencies, and evaluate supported activities. Project periods run one to five years, with continuation available non-competitively based on satisfactory progress and federal interest.
The program has sustained activity over two decades, with 1,200 total awards since 2001 and recent activity showing uneven annual cadence: 55 awards in 2021, 18 in 2022, 1 in 2023, 48 in 2024, and 1 in 2025. Combined federal obligation stands at $2.8 billion. Funding is concentrated within CDC: CDC placed 942 awards worth $931.8 million and a separate CDC unit placed 234 awards worth $1.74 billion, together representing approximately 100 percent of awards. Indian Health Service, HRSA, and a small HRSA adjustment round out the funder base.
The assistance-type mix is predominantly Cooperative Agreements (61 percent), with Project Grants comprising 38 percent and Block Grants the remaining 2 percent. Funded activities include community-based HIV testing and prevention services, care coordination and linkage to treatment, syndemic approaches addressing HIV alongside hepatitis C and syphilis, peer navigation and health education, mobile health outreach, prevention programs targeting American Indian and Alaska Native populations, and capacity building for health care providers. Representative recent awards span syndemic prevention in tribal health systems (ranging from $145 thousand to $23.3 million), mailed HIV self-test distribution to 875,000–1 million kits nationally ($100.3 million), and targeted testing and prevention for young men of color who have sex with men and transgender youth ($2.4–4.9 million per award).
No current grant opportunities are open under this Assistance Listing. The most recent solicitation, CDC-RFA-PS-24-0020 (Capacity Building Assistance for HIV Prevention Programs), closed April 30, 2024, and addressed workforce development rather than direct prevention services. Prior open opportunities include comprehensive high-impact prevention for young populations of color (CDC-RFA-PS22-2203, due November 2021), community-based organization programs (CDC-RFA-PS21-2102, due November 2020), and mass mailing of HIV self-tests (CDC-RFA-PS22-2210, due July 2022). Applicants should monitor Grants.gov for future NOFOs, which typically appear annually or biennially targeting CBOs, health departments, and service providers.
