About this program

The Department of Health and Human Services Office of the National Coordinator for Health Information Technology (ONC) funds sole-source cooperative agreements to enhance and maintain the Logical Observation Identifiers Names and Codes (LOINC) standard, CFDA 93.346, with the most recent award disbursed on August 7, 2023.

This program funds the development and maintenance of LOINC, a universal coding system for laboratory and clinical observations, measurements, and health documents. Funded activities include upgrading technical infrastructure for LOINC code development and dissemination, accelerating terminology development to support evolving health information exchange needs and the U.S. Core Data for Interoperability (USCDI), bridging gaps between LOINC and HL7 FHIR standards, deploying FHIR Terminology Services, and updating public-facing tools such as SearchLOINC and RELMA used by implementers. All tools and content developed must remain publicly accessible. The program has funded public health needs during emergent situations, including COVID-19 pandemic response through development of SARS-CoV-2 testing terminology.

All awards are Cooperative Agreements. Three awards have been disbursed since 2018, with one award every two to three years. Combined federal dollars obligated total $6.1 million. The Office of the National Coordinator for Health Information Technology is the sole funding source.

Representative recent awards include Cooperative Agreement 90AX0038 for $2.5 million (awarded August 2023, completion September 2026) to enhance LOINC infrastructure and content, and Cooperative Agreement 90AX0021 for $3.578 million (awarded September 2018, completion September 2023) to upgrade technical infrastructure and enhance LOINC capabilities. Cooperative Agreement 90C30006 (awarded September 2020) focused on accelerating LOINC development for public health needs under the CARES Act, particularly COVID-19 terminology.

This program operates as a non-competitive sole-source arrangement with Regenstrief Institute, the exclusive standards development organization with intellectual property rights to LOINC. No open grant opportunities are currently posted. The most recent solicitation (NAP-AX-23-001) closed July 5, 2023.

Objective and description

Objective

This single source cooperative agreement to Regenstrief, is being made in order to expedite improvements and bridge gaps between Health Level 7 International's (HL7®) Fast Healthcare Interoperability Resources (FHIR®). Additionally, the cooperative a

Description

This is a single source award for Regenstrief Institute. Regenstrief Institute (Regenstrief) is a non-profit medical research organization, associated with Indiana University, and is the exclusive standards development organization (SDO) with the ability, authority, and intellectual property rights to develop and maintain the Logical Observation Identifiers Names and Codes (LOINC®), an open source common terminology for laboratory and clinical observations. As the field of medicine continues to evolve, there is an ongoing need for Regenstrief to provide new codes in a timely manner. As such, it is important that the LOINC terminology standard remains up-to-date and able to support new data exchange standards, interoperability design patterns, and data policy, including the U.S. Core Data for Interoperability (USCDI).
