About this program
The Department of Health and Human Services Health Resources and Services Administration (HRSA) operates Grants to Increase Organ Donation (CFDA 93.134), which funds living organ donation reimbursement mechanisms, public education outreach, and social and behavioral interventions to increase organ donation, with the most recent grant awards made September 1, 2025.
The program comprises two primary funding lines: the Living Organ Donation Reimbursement Program, which reimburses qualified expenses for living organ donors including lost wages, dependent care, travel, and meals for medical evaluation and follow-up, and the Public Education for Living Organ Donation Reimbursement Program, which conducts national outreach to increase awareness and access to reimbursement resources, particularly in medically underserved communities. Historical awards also supported social and behavioral intervention research to increase deceased and living organ donation registration, family discussions about donation, and parental consent for pediatric organ donation.
Recent grant activity shows 2 awards in 2025 out of 290 total awards dating to 2000, reflecting variable annual funding (7 awards in 2017–2018, declining to 1 in 2020). The portfolio is predominantly Project Grants (67 percent, 193 awards) with smaller shares of Cooperative Agreements (20 percent, 58 awards) and Block Grants (13 percent, 39 awards). HRSA is the sole funder, obligating $237 million in combined federal dollars across the program's history.
Recent awards exemplify the scope: a Public Education cooperative agreement for $2,998,402 (September 1, 2025–August 31, 2028) and the Living Organ Donation Reimbursement Program cooperative agreement for $27,694,717 (same period). Earlier awards supported demonstration projects on lost wages reimbursement ($1,992,684) and social and behavioral interventions to increase organ donation (awards ranging from approximately $146 thousand to $1 million).
Two competitive opportunities are currently open with April 18, 2025 due dates: HRSA-25-082 (Living Organ Donation Reimbursement Program) and HRSA-25-083 (Public Education for Living Organ Donation Reimbursement Program), both soliciting Cooperative Agreements from eligible public entities, nonprofits, institutions of higher education, tribes, small businesses, and for-profit organizations. The program's three-year cooperative agreement cycle began September 1, 2025 and will run through August 31, 2028.
