About this opportunity

Public Health Solutions (PHS) has been selected by the New York State Department of Health's Office of Health Insurance Programs as the Social Care Network (SCN) lead entity for Manhattan, Queens, and Brooklyn, issuing a solicitation for Health-Related Social Need (HRSN) Service Providers. The procurement seeks Community-Based Organizations (CBOs) and Healthcare Providers to participate in the WholeYouNYC Social Care Network, with the primary objective of screening, navigating, and delivering social care services to eligible Medicaid members. Providers must offer services across multiple domains including screening, care management, housing support, nutrition assistance, and transportation, with specific requirements to deliver services in languages such as Spanish, Chinese, Mandarin, Russian, and others. Providers must use the Unite Us data and IT platform for referrals, screenings, and service documentation, and commit to flexible service availability including standard business hours, after-hours, and weekend coverage. The application process is a rolling submission through the PHS Procurement Portal, with the initial posting date of January 21, 2025, and an extended due date of February 28, 2027.

The solicitation is restricted to specific organizational types, specifically requiring applicants to be either registered 501(c)(3) or 501(c)(4) organizations for CBOs, or physical and/or behavioral healthcare providers. Disadvantaged or local community-based organizations are encouraged to apply, with a focus on entities capable of serving at least one zip code within the targeted boroughs. The fee schedule, effective from October 1, 2025, through March 2027, provides region-specific funding allocations, with examples like Manhattan receiving $1,864,583 for transportation services and $1,055,260 for cooking supplies. Reimbursement is contingent upon meeting specific requirements, including using approved screening tools and adhering to NYS certification standards. Providers will be evaluated based on their service capacity, language capabilities, operational readiness, and ability to integrate with the existing Social Care Network infrastructure, with potential service contracts spanning 6-12 months and opportunities for re-authorization based on ongoing member needs.
