About this file
This document is a Health-Related Social Need (HRSN) Service Provider Application for the WholeYouNYC Social Care Network, involving Public Health Solutions in New York City. The application is seeking Community-Based Organizations (CBOs) and Healthcare Providers to join a Social Care Network (SCN) that will provide health-related social services across Brooklyn, Manhattan, and Queens. Applicants can choose to participate as either a CBO or Healthcare Provider, with services ranging from health screening and care management to housing support, nutrition counseling, and transportation. The application requires organizations to specify the specific HRSN services they will provide, such as home modifications, meal services, utility assistance, and support for various vulnerable populations including those with chronic conditions, justice-involved individuals, and specific age groups.
The application requires comprehensive documentation for submission, including a New York State Certificate of Incorporation, Internal Revenue Service 501(c)3 or 501(c)4 Determination Letter, IRS form 990, and completed attachments detailing services and language capabilities. Organizations must confirm their submission through the Bonfire procurement platform and verify their ability to serve at least one zip code in the specified boroughs. The application is specifically seeking new applicants, with previous applicants from prior WholeYouNYC Social Care Network solicitations being instructed not to reapply. Eligible organizations must be non-profit entities working at the local community level and capable of providing direct health-related social services to Medicaid members and other targeted populations.
