About this opportunity

The State of Delaware Department of Health and Social Services, Division of Public Health, is conducting a Request for Proposals (RFP) to procure professional services supporting the Rural Health Transformation Program (RHTP) in rural Sussex and Kent Counties. The procurement seeks qualified vendors to provide either grant program oversight services, monitoring and evaluation services, or both components to support the coordination, monitoring, and execution of 15 integrated initiatives designed to expand healthcare access, strengthen workforce capacity, modernize technology, and improve population health outcomes for approximately 400,000 rural residents representing 38-40% of Delaware's population. Program Coordination services encompass subrecipient management, budget oversight, compliance monitoring, and consolidated reporting, while Monitoring & Evaluation services include framework development, performance metrics tracking, data analysis, and support for the AchieveIt platform. Vendors must submit proposals by June 22, 2026, at 1:00 PM EDT through the Euna Procurement portal, with written questions due by May 18, 2026, at 3:30 PM EST. The estimated contract award date is July 13, 2026, with a project start date of September 15, 2026. The contract term is one year with four optional one-year extensions for a potential five-year engagement. Required proposal submissions include a transmittal letter, executive summary, technical proposal (maximum 10 pages), budget and narrative (maximum 8 pages), organizational chart, key personnel resumes, sample reporting templates, and completion of standardized vendor forms. Vendors must maintain comprehensive insurance including Workers' Compensation, Commercial General Liability ($1,000,000 per occurrence/$3,000,000 aggregate), and Medical/Professional Liability, Errors and Omissions, or Product Liability ($1,000,000 per occurrence/$3,000,000 aggregate), register with the System for Award Management (SAM) with a Unique Entity Identifier, obtain Delaware business licensure, comply with HIPAA through a Business Associate Agreement, and adhere to Department of Technology and Information Standards and Policies.

The procurement does not identify set-aside designations or restrictions limiting participation to specific organization types; however, the vendor forms document requires vendors to provide state and federal certification status and diversity vendor certifications. No incumbent contractors or existing vendors are identified in the solicitation materials. The State secured approximately $157 million in federal funding from the Centers for Medicare and Medicaid Services (CMS) for the first year of RHTP programming, with potential to receive up to $1 billion over five years, indicating substantial funding availability. Pricing must be submitted using the Project Cost Template (Appendix F) with separate budgets for each component if applicable, including line-item budgets and detailed narratives establishing a performance-based compensation model where payment is contingent upon successful completion and approval of specified deliverables. The State reserves the right to award to multiple vendors, reject any or all proposals, and negotiate with multiple vendors simultaneously. Monthly usage reports and quarterly subcontracting diversity reports are mandatory for awarded vendors. Detailed pricing information specific to award values or budget ranges is not disclosed in the publicly available solicitation materials.
