About this opportunity
The Rockland County Department of Mental Health is issuing Request for Proposal RFP-RC-2026-010 to procure Transition Management (TM) and Re-entry (RE) services for individuals with serious mental illness (SMI) and/or substance use disorders (SUD) being released from Rockland County Jail. The selected non-profit agency will employ one full-time Re-entry/Transition Coordinator positioned at the jail 3-4 times weekly to provide voluntary, person-centered services beginning during incarceration and continuing post-release, including connection to behavioral health treatment, public benefits assistance, housing referrals, employment services, and life skills programming. Key deliverables and objectives include establishing bi-weekly case conferences with jail and Department of Mental Health staff, conducting quarterly Transition Management/Re-entry Task Force meetings with community stakeholders, initiating contact with referred inmates within two business days, and providing post-release follow-up within five business days of release, with the ultimate goal of improving behavioral health outcomes, supporting successful community reintegration, and reducing recidivism. A virtual pre-proposal meeting was conducted on March 25, 2026 at 11:00 AM via Microsoft Teams, with a written question deadline of March 23, 2026 at 3:00 PM and a proposal submission deadline of April 2, 2026 at 3:00 PM via the Bonfire Portal. Proposals will be evaluated across five weighted criteria: demonstrated re-entry service experience (200 points), willingness and ability to meet RFP requirements (150 points), knowledge of Rockland County's behavioral health and criminal justice systems (150 points), ability to expeditiously implement services (200 points), and cost proposal and value-added services (300 points). Required qualifications include submission of an executive summary, detailed narrative addressing organizational overview, experience and capacity, program approach, service delivery plan, and data collection methodology, plus a detailed three-year budget with narrative justification and minimum references of three current projects and two past projects.
This procurement is open to qualified non-profit agencies and specifically encourages submissions from Minority- and Women-Owned Business Enterprises (MWBE) and Disadvantaged Business Enterprises (DBE). There are no identified incumbent vendors or current contractors referenced in the solicitation materials. The contract is contingent upon funding approval from the New York State Office of Addiction Services and Supports (OASAS) and the Office of Mental Health (OMH), with an initial term commencing upon funding approval and extending through December 31, 2028, followed by two optional one-year renewal periods exercisable at the County's sole discretion. The total award value is $86,250 for the period through December 31, 2026 (contingent upon start date), $118,450 for 2027, $122,004 for 2028, $125,664 for 2029 (first renewal option), and $129,434 for 2030 (second renewal option). Successful applicants must provide evidence of New York State Workers' Compensation insurance certification, a Certificate of Liability Insurance naming Rockland County as an additional insured with minimum coverage of $1,000,000 general liability, and must comply with County of Rockland Standard Terms and Conditions for Service Contracts and all applicable federal contract clauses including equal opportunity, anti-kickback, debarment and suspension, and lobbying disclosure provisions.
