About this opportunity
The Georgia Department of Public Health is requesting quotes for incentive cards (MasterCard/Visa) under eRFQ DPH0000227, with quotes due September 8, 2023. The contract is structured as an open agency contract with no guaranteed minimum purchase volumes.
The scope is procurement of approximately 4,000 physical (non-personalized) incentive cards loaded with $25.00 each and distributed to program participants. Cards must display only the payment partner's generic branding with the MasterCard logo, carry a minimum three-year expiration date from activation, prohibit ATM withdrawal or cash access, and include no reloadable functionality. Suppliers must load funds onto cards within 24 hours of payment receipt. Virtual card options are not acceptable. The initial contract term is one calendar year from execution, with anticipated renewals for up to four additional years. Launch is expected immediately upon award.
Issuance fees must be all-inclusive of associated and supplemental costs; shipping costs are listed separately. The State Entity's payment methods are limited to check or ACH transfer referencing an invoice. Suppliers must acknowledge receipt of Addendum 1, which extended the closing date from August 31 to September 8, 2023, and revised the mandatory requirements worksheet and cost worksheet (removing customization fees for logo and artwork).
Rellevate Inc was selected as the successful offeror in the Notice of Award posted November 16, 2023. Olive Branches Property Management was deemed unsuccessful and non-responsive. Protests must be submitted electronically to doas.protests@doas.ga.gov in accordance with the Georgia Procurement Manual. The issuing officer is Arnaz Fanning (arnaz.fanning@dph.ga.gov).
