About this file
This Supplier Q and A Responses document addresses supplier inquiries regarding an electronic Request for Quotes issued by the Georgia Department of Public Health for the provision of Incentive Cards (MasterCard/Visa) under RFX Number DPH0000227. The RFX was posted on August 23, 2023, with Arnaz Fanning serving as the Issuing Officer. The State Entity intends to procure approximately 4,000 incentive cards to be loaded with $25.00 each and distributed to program participants by Department staff. The contract has an initial term of one calendar year from execution, with anticipated renewals for up to four additional years. Cards must not be personalized with participant names and shall display only the payment partner's generic branding with the MasterCard logo. Launch is expected immediately upon successful contract award.
The cards are subject to specific technical requirements, including a minimum three-year expiration date from activation, no ATM withdrawal or cash access capabilities, and no reloadable functionality. The State Entity will not accept virtual card options and requires physical card delivery. Suppliers must load funds onto cards within 24 hours of payment receipt by the Department. The State Entity will not guarantee minimum purchase volumes, with actual card quantities and annual disbursement amounts determined by entity needs. The Department's forms of payment are limited to check or ACH transfer referencing an invoice. Issuance fees must be inclusive of all associated and supplemental costs, with shipping costs to be listed separately in vendor proposals.
