About this opportunity
The State of Illinois, through the Illinois Veterans Home (IVHQ), is seeking vendors to provide various liquid food supplement products for the period of July 1, 2023 to June 30, 2024. The Request for Quote (RFQ) specifies the required quantities and brand names of the food supplements, including Two Cal HN, Glucerna, and Ensure products from Abbott Nutrition. The total quantity ranges from 2,400 to 13,200 units per item, to be ordered on an as-needed basis throughout the fiscal year. Quotes must be submitted electronically either via email or the BidBuy system by the due date of April 11, 2023. The RFQ outlines the pricing and other requirements for the vendors, including that all prices must include shipping and handling, and no price increases will be accepted. The selected vendor must be able to provide all the required supplies and offer the lowest price. The agency reserves the right to reject any offers and cancel the procurement up to 72 hours in advance. The RFQ also includes COVID-19 protections, requiring the selected vendor to adhere to the Governor's executive orders regarding face coverings, vaccinations, and testing.
The Standard Illinois Certifications document outlines the 33 certifications that vendors must make to be eligible for state and local government contracts in Illinois, covering various legal, ethical, and compliance requirements. Vendors must comply with these certification requirements for the duration of any awarded contract and are subject to penalties for false statements or noncompliance. The Standard Terms and Conditions document further establishes the contractual terms and conditions that will apply to the selected vendor, including payment terms, assignment, audits, confidentiality, and termination provisions. The contract is subject to the laws of the State of Illinois and may be terminated for cause or convenience, contingent upon the availability of appropriated funds.
