About this opportunity
The Commonwealth of Kentucky Department of Agriculture, Office of the Kentucky State Veterinarian is issuing a Request for Quotation to procure 50,000 Equine Certificates of Veterinary Inspection (CVI) books, with each book containing 25 consecutively numbered certificate sets in four-part No Carbon Required (NCR) format, totaling 1,250,000 individual form sets. The specifications require books in 8.5 by 11-inch horizontal format with one-half inch left staple stub, printed in black with seven-digit crash numbering, vertical perforation edge-to-edge, and bound with ivory wraparound covers and tag backing, stitched twice through the stub. Delivery must be included in the quoted price and shipped to the Office of the Kentucky State Veterinarian at 109 Corporate Drive, Frankfort, Kentucky 40601. Award will be determined using a best value ranking approach allocating 90 points to price and 10 points to delivery performance, with the lowest bidder receiving maximum price points and other bids scored proportionally. Vendors must offer delivery within the required 30-day timeframe after receipt of order, with faster delivery eligible for bonus points. The solicitation requires vendors to be registered in the Kentucky eProcurement system and Vendor Self Service (VSS) system prior to submission. Written vendor questions are due by March 21, 2025 at 10:00 a.m. Eastern Time, and quotation closing date is March 28, 2025 at 1:30 p.m. Eastern Time.
Kentucky reciprocal preference applies to resident bidders as defined by KRS 45A.490-494, and qualified Kentucky Correctional Industries and nonprofits serving individuals with disabilities receive preference equal to 15-20 percent of maximum points. All quotations must be submitted electronically through the Kentucky eProcurement system on an F.O.B. Destination Freight Prepaid and Allowed basis, with vendors responsible for all shipping costs and excluding federal excise tax and Kentucky sales or use tax from quoted prices. Prior to award, successful bidders must submit proof of Commercial General Liability Insurance with minimum limits of $1,000,000 per occurrence and $2,000,000 aggregate, along with Kentucky Tax Registration documentation, required affidavits, and a Certificate of Authority if a foreign entity. Proposed pricing must remain valid for a minimum of 60 calendar days after the bid due date. The solicitation does not specify a unit price or total contract amount, requiring vendors to provide their best price and best delivery terms in their responses. The resulting contract will be awarded as a group award to a single vendor who must bid all line items, with the Commonwealth of Kentucky retaining the right to deem non-compliant vendors ineligible and to cancel the contract with 30 days' written notice.
