About this opportunity
San Francisco Department of Public Health is soliciting proposals for community-based doula and midwifery prenatal care models for pregnant people in San Francisco under RFP SFGOV-0000012083, with a pre-proposal conference on November 4, 2026 at 9:00 a.m. PT and proposals due December 7, 2026 at 2:00 p.m. PT. No set-aside restricts eligibility. Certified Small or Micro-LBE proposers receive rating bonuses of up to 10%. The RFP summary states it establishes no LBE subcontracting requirement, while the contract template (Attachment M) references LBE subcontracting participation requirements for a specified percentage of services.
Proposers may submit one or more care models, including community-based doula models, non-clinical midwifery support models, group prenatal care models, or innovative models. DPH will fund only one selected model per awarded contractor. Integrated models must coordinate relationship-based doula or midwifery support, care navigation, and social services. PEI funds may not pay for licensed clinical services, which must be billed separately to Medi-Cal or other third-party payers. Work is performed in San Francisco.
Compensation is cost-reimbursable for actual, allowable, and reasonable costs, up to a not-to-exceed amount of approximately $175,000 for year one. The initial term is one year, with nine one-year options at the City's sole discretion, and the City may modify the term. All funding derives from the California Department of Public Health's Perinatal Equity Initiative, and continued funding beyond the initial term is not guaranteed.
Proposals are evaluated on pass/fail minimum qualifications, firm qualifications (25 points), staff qualifications (20 points), scope of work understanding (30 points), and cost proposal (20 points). Interviews may add 25 points.
Required forms include three professional references, a non-collusion affidavit, the MCO and HCAO declarations, the First Source Workforce Projection Form, the CMD LBE Form 2A, and a cover sheet certifying California good standing. Proposals are submitted through the San Francisco City Partner portal with the forms in Electronic File #2. Questions are due November 12, 2026 at 5:00 p.m. PT, with a Q&A addendum estimated for November 20. Shortlisting, interviews, and award announcement are estimated for January 2027, a Health Commission presentation for February 2027, and contract start for July 1, 2027.
