About this opportunity
The National Institutes of Health is conducting market research to identify contractors capable of performing Occupational Medical Services, with a sources-sought response deadline of September 22, 2026. The Government will use responses to determine acquisition strategy and set-aside eligibility for Service-Disabled Veteran-Owned Small Businesses, HUBZone Small Businesses, Women-Owned Small Businesses, Economically Disadvantaged WOSB, and other small business concerns. No award will be made from this notice.
This is a follow-on contract to an existing requirement. The Government anticipates awarding a single-award, cost-reimbursement (cost-plus-fixed-fee) contract with a base period and option periods, subject to Service Contract Labor Standards.
Performance spans five locations: Bethesda, Maryland (main clinic); Baltimore, Maryland; Frederick, Maryland; Hamilton, Montana; and Research Triangle Park, North Carolina. The contractor must provide approximately 18,400 annual outpatient medical visits serving over 18,000 NIH employees. Services include emergency and urgent care for occupational injuries and illnesses, pre-placement and periodic medical evaluations (approximately 940 initial evaluations plus 500 summer hires annually), work-related immunizations and travel medicine, physical therapy, acupuncture, laboratory support, specialized surveillance programs (Biological Surety, Tuberculosis, Animal Exposure, Hearing Conservation, Asbestos, Hazardous Drug), and an Employee Assistance Program with approximately 5,000 annual client contacts. After-hours on-call medical support is required seven days per week for work-related emergencies involving biological hazards and serious injuries.
Staffing at the main clinic includes 1.5 Nurse Practitioners, 7 registered nurses, 1 licensed practical nurse, 0.6 physical therapist, 2 physicians (on as-needed basis), 1 psychologist, 0.4 acupuncturist, and 8.5 medical records specialists. Key personnel—Contract Program Manager, Contract Deputy Program Manager, physicians, psychologist, and EAP Lead—require government approval. The contractor must maintain a 108-procedure Occupational Medical Service Procedure Manual and achieve performance metrics including maximum ten-minute median wait times for routine appointments and three business day appointment availability for non-urgent requests.
Notice text
1. Purpose
The NIH, on behalf of the ORS Division of Occupational Health and Safety, is conducting market research to identify potential sources capable of performing Occupational Medical Services and to determine the appropriate acquisition strategy, including whether the requirement can be set aside for small business concerns under a socioeconomic program. This notice specifically seeks to determine the availability and capability of:
- Service-Disabled Veteran-Owned Small Businesses (SDVOSB)
- HUBZone Small Businesses
- Women-Owned Small Businesses (WOSB) / Economically Disadvantaged WOSB (EDWOSB)
- Other small business concerns
The Government will use responses to this notice to make decisions regarding set-aside determinations consistent with applicable regulations and Department policy. No award will be made from this notice.
2. Background
NIH DOHS operates an Occupational Medical Service that provides work-related medical care and counseling to the NIH workforce, including emergency medical care, pre-placement medical evaluations, medical care for occupational injuries and illnesses, work-related immunizations and health surveillance programs, and employee assistance/counseling services. Performance supports a biomedical research community with potential chemical, biological, animal, physical, and radiological workplace hazards in addition to typical occupational hazards.
This is a follow-on to an existing requirement. The Government is considering a single-award, cost-reimbursement (cost-plus-fixed-fee) contract with a base period and option periods, subject to the Service Contract Labor Standards (SCLS).
3. Anticipated Scope of Work (Summary)

- Emergency and urgent medical care for work-related conditions
- Pre-placement and periodic medical evaluations
- Work-related immunizations and travel medicine
- Medical surveillance programs (e.g., animal exposure, respiratory/asbestos, TB, hearing conservation, and related biomedical-hazard programs)
- Employee Assistance Program (EAP) counseling services
- Clinical operations across multiple fixed sites, with a limited mobile/traveling component (e.g., seasonal flu clinics)
- Associated quality control/quality improvement, reporting, records management, and information-security/privacy compliance (moderate-impact PII/IT security)
Anticipated performance locations include: Bethesda, MD; Frederick, MD; Baltimore, MD; Research Triangle Park, NC; and Hamilton, MT. (Respondents should indicate ability to staff all locations.)
