About this opportunity

U.S. Customs and Border Protection is conducting a sole-source extension of medical services at the southwest border, with a response deadline of June 19, 2026. CBP intends to award task order Option Periods 10 and 11 to the incumbent contractor Loyal Source Government Services, LLC under the Veterans Affairs Federal Supply Schedule 621-I for a combined value of $37,684,162.62 across two three-month periods.

Medical personnel under this extension deliver 24/7/365 screening, health intake interviews, infectious disease support, supervisory physician oversight, and electronic medical record documentation at Medical Priority Facilities, Office of Field Operations Ports of Entry, and forward-deployed locations along the southwest border to address medical and public health concerns for individuals in CBP custody. CBP's justification for sole-source award cites the incumbent's specialized expertise in CBP medical protocols and directives; market research conducted in December 2025 concluded no other contractor could mobilize required services within the necessary timeframe, as a new contractor would require a minimum of 120 days for background investigations plus additional training time. The incumbent has held the contract since September 28, 2022. The option periods are not guaranteed and will be exercised at the government's discretion.

Notice text

CBP requires healthcare practitioners to work out of Medical Priority Facilities (MPFs), Office of Field Operations (OFO) Ports of Entry (POEs), and potentially other forward deployed locations within the United States, to address medical and public health concerns for individuals in CBP custody.

To discharge agency responsibilities along the southwest border, DHS/CBP has established enhanced medical provisions for individuals in custody at the southwest border crossings and OFO POEs.
Contracted medical personnel deliver medical support at various locations.
