About this file
This is a Worker Protection and Investment Certification Form required by the Commonwealth of Pennsylvania for contractors and grantees working on state and local contracts, including the Pennsylvania Game Commission's Southeast Region demolition and debris removal project. The form mandates that all contractors certify compliance with Pennsylvania's Unemployment Compensation Law, Workers' Compensation Law, and applicable state labor and workforce safety laws, encompassing thirteen specific statutory requirements including the Construction Workplace Misclassification Act, Employment of Minors Child Labor Act, Minimum Wage Act, Prevailing Wage Act, Equal Pay Law, and other employment-related statutes. The certification must be executed by an official duly authorized to bind the contractor or grantee and submitted as part of the contract award process for the demolition project, which involves removal of abandoned structures, vehicles, and debris from multiple Game Commission properties.
The certification is subject to Pennsylvania penalties for false statements, including contract termination and three-year debarment from bidding on state contracts under 62 Pa.C.S. § 531. Certifying officials must understand that the form constitutes an unsworn falsification to authorities under 18 Pa.C.S. § 4904 and must report any changes in compliance status to the purchasing agency immediately. The form establishes a binding commitment that the contractor will maintain compliance with all applicable Pennsylvania labor and workplace safety laws throughout contract performance, with no exceptions or waivers permitted under Executive Order 2021-06.
