About this file

This is a Bidder Response Form for the WMR001 solicitation issued by the Commonwealth of Massachusetts, Operational Services Division (OSD), for hazardous waste, universal waste, medical waste, electronic waste disposal, and emergency response services. The procurement encompasses nine service categories: hazardous waste collection and disposal, household hazardous waste collection events, mercury-containing products and batteries collection and disposal, sharps collection and disposal, medical/infectious waste collection and disposal, pharmaceutical waste collection and disposal, electronic waste collection and recycling, tank collection and disposal, and emergency response services with immediate response action. Bidders may respond to one or multiple categories and must demonstrate statewide coverage (or justify limitations), with preference given to vendors offering service throughout Massachusetts, including all counties from Berkshire to Worcester. The form requires bidders to complete all applicable category questionnaires, delete tabs for categories not being bid, and submit documentation by November 5, 2025, with an anticipated bid posting date of July 16, 2025. A mandatory two-hour maximum emergency response time applies to all islands and mainland locations for Category 9 services.

Bidders must submit comprehensive documentation including a completed Bidder Response Form (Excel format), Bidder Price File, SDP Form, PPD Form, Certificate of Good Standing from the Department of Revenue, Certificate of Insurance meeting specified thresholds with the Commonwealth identified as Certificate Holder, CEP Form, and any additional supporting documents. All files must follow specified naming conventions, remain unpassword-protected, and include searchable PDFs. Required qualifications include minimum three years in business, gross annual revenue documentation for fiscal years 2022-2024, a written Continuity of Operations Plan, disclosure of any bankruptcy, litigation, contract defaults, and environmental violations within the previous five years, and three business references per category bid (with at least one Commonwealth of Massachusetts Agency or Municipality reference recommended and one high-volume order reference). Bidders must list all receiving and destination facilities (RCRA and non-RCRA separately) with detailed facility summaries including permits, licenses, environmental enforcement history, Superfund liability information, and insurance coverage. The form requires bidders to disclose third-party certifications, licenses, affiliations, and memberships, and includes specific end-market information requirements for Categories 3 and 7 regarding material recycling destinations and tonnage accepted in calendar year 2024.
