About this file

This is a Wage Certificate document for a state contract opportunity issued by Hawaii State Public Library System (HSPLS) for RFQ No. HSPLS 25-05, specifically related to the Wahiawa Library Temporary Location Repair project. The document appears to be a mandatory certification form for contractors bidding on a service contract valued over $25,000, with requirements for wage and labor compliance.

The wage certificate mandates that successful bidders must comply with federal and state laws regarding workers' compensation, unemployment compensation, wage payments, and safety standards. Contractors are required to pay employees wages comparable to public sector employees for similar work, with exceptions for professional, managerial, supervisory, and clerical personnel. Failure to comply with these conditions could result in contract cancellation, and final payment or bond release is contingent upon the procurement officer's determination of compliance. The document is to be submitted as an attachment on the HIePRO procurement platform, with the offeror's signature, name, title, and contact information.
