STABILITY OF THE TOWER.

SECOND: "AVOID OFF AIR / DOWN TIME". THESE ARE SYSTEM INTEGRITY ITEMS, WHICH CAN AFFECT

THE SIGNAL OR ABILITY TO REMAIN ON-AIR.

THIRD: "AVOID LIABILITY RISK". THESE ARE LIABILITY RISK ITEMS SUCH AS HAZARDS TO CLIMBERS OR

PERSONS AT OR AROUND THE STRUCTURE, INCLUDING "ATTRACTIVE NUISANCE" POTENTIALS.

FOURTH: "KEEP THE COST OF OWNERSHIP AS LOW AS PRACTICAL". THROUGH APPROPRIATE

MAINTENANCE AND PREVENTATIVE MAINTENANCE, THE UTILITY VALUE OF THE STRUCTURE CAN BE

Recommendations Concluded We also recommend our annual maintenance agreement to ensure the tower is maintained on a yearly basis. As ample maintenance agreement is enclosed. This report and comments reflect the general condition of the tower on the date of inspection.

Recommendations listed on the preceding pages may be incorporated into a 3-5 year program. If so, the price does not include the additional repair of latent defects if found during this work. If latent defects are found, Allstate Tower INC. will submit a lump-sum bid to the owner to perform the necessary additional repairs. Under no circumstances will the additional repair work be done without owner's written authorization in the form of a separate repair contract agreed to by both parties. Owner agrees to undertake all measures necessary to expedite decisions regarding repairs and avoid delaying the work outlined under the terms of the contract. We recommend $5,000 be set aside for latent defects.

Handling, removing, and/or disposal of hazardous or contaminated material (asbestos, lead, or any substance requiring special handling) are not include in any price submitted or work Herein.

Section 7

- Bolts on the Safety climb extension at the top of the tower are too short and do not have locking pal nuts.

Explanation of Bolt Torque Check Structural bolt connections are "sampled" at the rate of at least 10 bolts per each 20 vertical feet of tower height. The tower is considered to be in acceptable condition if the number of under-torqued bolts is found to be no more than 5% of the total sampled.

The tower is said to have a "maintenance deficiency" if the number of under-torqued bolts is 6% to 30% of the total sampled. If this is the case, the tower should be re-checked, at least annually. Since each under-torqued bolt found during the random sampling process is re-tightened to the proper level, the number of under-torqued bolts will eventually drop back below the structurally acceptable condition and all bolts should be checked and re-tightened to correct levels.
