SUMMARY OF EXPERIENCE FORM
Competency Area: Logistics Support Competency Instructions: Offerors shall demonstrate a minimum of three years of experience providing contract support in five of the eight areas of Logistics Support competency listed in Section 4.1 of the Source Selection Plan (Volume 4-Factor 4). The minimum of three years of experience shall have been performed within the past 9 years. As part of the submission, the offeror shall include all applicable contract numbers, contract titles, identify the prime or major subcontractor that performed the effort, PWS references, and contract period of performance dates that will allow the Government to successfully evaluate the offeror’s experience volume of the proposal. In order for a contract effort to be considered, the discussion below must confirm that the prime or subcontractor that has the experience identified will support the same competency areas if the offeror receives the resultant award.
