W9133L-13-R-0015 Exhibit J 7: Past Performance Questionnaire

SOLICITATION W9133L-13-R-0015

OFFEROR’S LETTERHEAD (Date)

The National Guard Bureau (NGB) is in the process of selecting a contractor for a contract in support of the ANG Mass Notification System (MNS) Net-Centric Alerting System (NCAS). This contract will include multiple support locations in which the ANG must be capable of rapidly and reliably informing appropriate personnel about anti-terrorism/force protection conditions (FPCON, including chemical, biological, radiological, and nuclear threats), hazardous weather conditions, and many other critical events through the use of NCAS.

One of the considerations in proposal evaluation is the verification of an offeror’s past and present performance on contracts reflecting on the offeror's ability to perform the proposed effort. NGB depends on information received from agencies or firms such as yours with first-hand experience with an offeror in order to evaluate the offeror's performance on those contracts.

NGB’s specific areas of interest are summarized in the attached Past Performance Questionnaire. Because evaluation timeline is constrained your written response is requested as soon as possible but no later than March 12, 2013.

To assist you in preparing your response and expediting your reply, the attached Past Performance Questionnaire may either be filled out by hand or in Microsoft Word and then either faxed to the address shown at the end of the questionnaire or e-mailed (in Word or Adobe PDF format only) to willie.l.holmes2.ctr@mail.mil . As stated on the questionnaire, please send the form directly to this NGB evaluator and not through the firm being evaluated. Your completed questionnaire will become a part of the official source selection records.

Your help is greatly appreciated and your prompt response will be one of the keys to the successful and timely completion of this Government contractor source selection.
