INSTRUCTIONS FOR COMPLETION OF
PAST PERFORMANCE QUESTIONNAIRE
1. Blocks 1 through 10 will be completed by the SSEB reviewer.
2. The remainder of the form shall be completed by the agency having a past performance history with the contractor specified in Block 1.
3. Block 11 requests an assessment of the complexity of the work required by the subject Contractor under the referenced contract.
4. Block 12 identifies the specific area of past performance to be evaluated, with space available in Block 13 for agency comments.
| a. Block “a” requests an evaluation of the overall quality of the Contractor’s performance, including submission of well written, organized, and professional reports and deliverables. |
| b. Block “b” requests information regarding the Contractor’s personnel, i.e., the skills and expertise of the personnel performing and whether they were appropriate for the specific contract requirements, the quality of the Contractor’s personnel team, etc. |
| c. Block “c” requests information about the Contractor’s ability to manage subcontracts, if applicable, i.e., how they managed performance problems with subcontractors. |
| d. Block “d” requests information on the business relationship maintained with the Contractor, i.e., were they cooperative in negotiations and in resolving issues or requests. |
| e. Block “e” requests information regarding whether services, including submission of reports and deliverables, were generally performed in a timely manner or not. |
| f. Block “f” requests information regarding your satisfaction or dissatisfaction with the Contractor, including any weak points or what you liked least about them. Please justify your adjectival rating in the comments block. |
| g. Block “g” requests information regarding the Contractor’s ability to control costs and stay within the allowable budget provided for in the contract, i.e., significant cost overruns, etc. |
5. Block 14 shall be completed with one of the following adjectival ratings, based on your assessment of the Contractor:

PAST PERFORMANCE RATING GUIDELINES
Summarize Contractor Performance in each of the rating areas. Assign each area a rating of Unsatisfactory, Poor, Fair, Good, Excellent. Use the following instructions as guidance in making these evaluations. Note: There is no corresponding guidance for “Customer Satisfaction”. Please use the comments area on the preceding form to justify the rating given “Customer Satisfaction.”
