Class A—Fatality, permanent total disability, $2.5M or more in property damage.
Class B—Permanent partial disability, three or more admitted to the hospital, $600K or more, but less than $2.5M in property damage.
Class C— An occupational injury or illness with one or more days away from work, $60K or more but less than $600K in property damage.
Class D—An occupational injury or illness that results in restricted duty, restricted duty or job transfer, medical treatment beyond first aid, $25K or more but less than $60K in property damage.
Class E—$5K or more but less than $25K in property damage.
Class F—Aviation only.
Class G—Any injury or illness not listed in A through E, and property damage less than $5K.
