APPENDIX C-1
Quality Assurance Surveillance Plan (QASP)
A quality assurance surveillance plan (QASP) describes how Lake Shelbyville Project Office will survey, observe, test, sample, evaluate and document the contractor’s performance in meeting the critical performance standards identified in the contract. The QASP and the performance requirements are developed concurrently because of their influence on one another. Types of monitoring include: random sampling, periodic inspection, and customer feedback (see FAR 46.401).
1. TASK ORDER TITLE: Mowing Contract
2. WORK REQUIREMENTS:
Work Documentation/Service Log Section
Equipment Monitoring Section
3. PRIMARY METHOD OF SURVEILLANCE: The Government reserves the right to alter or change the type of inspection plan at its discretion at any time, and to make deductions accordingly. The Lake Shelbyville Project Office will inspect in an interval convenient to the government using the following surveillance methods:
• Random sampling
• Periodic inspection - a pre-determined plan based on analyses of Lake Shelbyville Project Office resources and requirements.
• Customer input
• Contractor self-reporting – This will be developed and approved by The COR, where the contractor can provide documentation of performance, i.e. daily reports that detail problems encountered.
4. SCOPE OF PERFORMANCE: The contractor shall provide remedial maintenance service on-site with problem resolution completed within 6 hours from the notice. Remedial maintenance is defined as efforts necessary to comply with the contract requirements at no cost to the government.
5. ACCEPTABLE QUALITY LEVEL (AQL): The AQL must be 98% compliant with the contract specifications within 24 hours of completion of mowing an area.
6. EVALUATION METHOD: The Quality Assurance Representative (QAR) will document the time of verbal notification to the contractor and if necessary the QAR will document the official time and date of the written notification. The QAR will review actual maintenance performance and enter in the record and if needed confirm the date and time of problem resolution.
7. REPERFORMANCE AND REINSPECTION: Should it become necessary for the Government's Inspector to perform re-inspections of defective work that was required to be redone (due to failure of Contractor Quality Control System to locate and cure these deficiencies prior to inspections), there will be a re-inspection/administration charge equal to the Actual Government Cost (AGC) at the Effective Hourly Rate (EHR) applied to re-inspect recreation area per inspection, per park. The AGC re-inspection time will start the minute the inspector is called and/or stops his other duties to perform the re-inspection and will end after the inspector returns to his duty site after the inspection. These charges will be deducted from the Contractor's monthly invoice. Re-inspection charges will only be charged when Contractor has been given the opportunity to do over again work that was not originally performed correctly. Deductions for re-inspection will be taken based on the following example:
Re-inspection of grounds maintenance at Coon Creek and Bo Wood. The Government's Inspector was at the office at the time he was called to re-inspect. His actual time to inspect the two parks and return to his duties at the office was 105 minutes. The Effective Hourly Rate (EHR) is $11.30 per hour.
The re-inspection deduction will be made as follows:
EHR/60 minutes x Inspection Minutes = Deduction $11.30/60 x 105 minutes = $19.78
