SECTION SF 30 - BLOCK 14 CONTINUATION PAGE
The following have been added by full text:
SOLICITATION AMENDMENT 0003
CHANGES TO THE PWS
Replace paragraph #4, Section L – General, with the following:
4. The proposal shall be submitted in one volume, containing Past Performance, Technical Evaluation Documentation, and Price. The binder shall be divided by indexed tabs, with Price tabbed last. The proposal is limited to forty (40) 8.5” x 11” pages, not including other performance evaluation forms, schedules, tabs, Table of Contents, and title sheets. The Accident Prevention Plan and Safe Practices Manual (as discussed in Section C, paragraph #13) do not count towards the page limit. The Personnel Qualifications and Experience (diver resumes and certifications, e.g.) do not count towards the page limit, however a summary is required which does count towards the page limit. Typing is permitted on both sides of the sheet, head to head with the spacing being 1.5 or double spaced utilizing no less than a size 12 font. A page printed on both sides will be counted as two (2) pages, and pages shall be consecutively numbered.
Drawings or other graphics shall be reduced only to the extent legibility is not lost. This requirement also applies to all proposed subcontractors/team members. One original and three copies of the proposal are required. In addition, an electronic copy is required, utilizing .PDF format and delivered on one disc. The computer files must be searchable and allow for compilation of separate files into a larger file as needed.
QUESTIONS & ANSWERS
Q1. Will the Comprehensive Accident Prevention Plan and the Safe Practices Manual be counted towards the 40 page limit?
See change to PWS as noted above.
Q2. Page (21) #4. The last sentence states: The computer files must be EDITABLE to allow for compilation of separate files into a larger file as needed. Please verify if this is the same as
SEARCHABLE.
See change to PWS as noted above.
Q3. Page (24) #1. The second to last sentence states: Dive team members shall be DAN certified. In reference to the EM 385-1-1, [30 NOV 14], and on page 7 and 9 of the solicitation, and on page 24 of Section 1, can you please clarify and verify that the HANDS-ON training of administering emergency Oxygen through the American Red Cross, is acceptable, as is acceptable on the current USACE contract.
The mandatory requirements of EM 385-1-1 for Emergency Oxygen Training (hands-on) is as follows:
30.A.08 - (a) Each dive team member SHALL have a current certification in the use of emergency oxygen systems, and, if provided on the dive site, the use of Automated External Defibrillators (AEDs).
(b) All classes SHALL contain a hands-on component and cannot be taken online. Evidence of this will be a photocopy of the certificates. The certificate(s) shall state the date of issue and length of validity.
NOTE: Training in the use of emergency oxygen systems SHALL be specific to underwater diving and SHALL meet the requirements of a nationally recognized training organization: Such as Divers Alert Network (DAN), Professional Association of Diving Instructors (PADI), National Association of Underwater Instructors (NAUI) the YMCA, or OTHER RECOGNIZED
SOURCES.
Q4. I have a question that is relevant to this package; In addition to the Commercial medical requirements a D.A.N. Cert is required. D.A.N. is relative to SCUBA - Recreational Diving. We dive US Navy Manual Rev. 6 requirements & Tables. My team members are Hat Divers - Veterans never in my 30+ years has SCUBA been accepted or tolerated as a method of work. It is forbidden in this industry - especially as it relates to all USACE work. Why is a SCUBA Certification required for a commercial diving project?
See answer to Question #3. DAN Certification only refers to Emergency Oxygen training, and is one of the recognized sources acceptable for this training. It does not refer to any requirements for commercial diving.
Q5. Will past post dive surveys (ref Encl 2) be acceptable past performance?
Post dive surveys do not replace the Past Performance Questionnaires or surveys, but can be included as part of the proposal and included in the 40-page limit.
Q6. In review of the solicitation, I noticed that there is a mileage component. The mileage component of the solicitation is unfair to local contractors. Our location is less than ten miles from many of the locations subject to diving. A contractor from Texas or Florida would be subject to receive significant mileage from this project, whereas we would be eligible for far less mileage. Vendors further from the local area would be subject to utilize more of the miles in the pay scale than we would. Therefore, it is an unfair solicitation. Perhaps the bidding schedule could be modified to include number of trips to each site per year. That would even out the solicitation so each bidder is bidding the job based on their location in lieu of mileage that would not be utilized by local contractors.
The mileage component line item compensates a Contractor fairly for mobilization and demobilization as needed. Each offeror is proposing a rate for the same number of miles, regardless of whether the mileage is used by the Contractor. This allows for all proposals to be based on the same quantities, and therefore can be considered fair and reasonable.
