AMENDMENT 0001
Solicitation W912P418B0022 Amendment 0001
This Amendment 0001 addresses questions/comments that were received concerning the above
Solicitation.
Comment: Page 280 of the .pdf list 13 attachments that can be found in the “attachments” panel of the file. These attachments are not available.
Response: This amendment provides the attachments.
Comment/question: Sheet G-002, Note 2 indicates there is an attachment to the specifications further detailing the disposal site. I am unable to locate any additional information on the disposal site.
Response: Drawing RE-102 is provided in this Amendment.
Questions: Who is responsible for maintaining the disposal site? Who is responsible for E&S measures at the disposal site? In our previous work in Lake County, access to the disposal site was restricted during wet weather and by time of day which would greatly impact the cost of this project. Are these restrictions accounted for in the attachment indicated above?
Response: The disposal site is property of Lake County. Section 01 35 13 Paragraph 1.3.1 Lake
County Points of Contact is provided for further questions concerning this property. During the execution of the contract, a USACE Contracting Officer Representative (COR) point of contact will be available.

Identify the hazards, what are the man, machine, and environmental hazard factors of an operation. Note: include hazards that could potentially change the risk, such as weather, changing site conditions, abnormal operating procedures, etc. Do a change analysis when site conditions change.
ACTIONS TO ELIMINATE OR MINIMIZE HAZARDS
List the controls needed to eliminate or reduce the risks. Controls can be in the form of educational controls (training), physical controls (barriers, guards, signs, etc.), Administrative Controls, Personal Protective Equipment, or Engineering Controls. Risk avoidance is another alternative, are there ways to do the job differently so that risks aren't encountered.

Occurs frequently. Several times in the life of an item with a probability of occurrence less than 10-1 but greater than 10-2 in that life.:
Occurs several times. Likely to occur some time in the life of an item with a probability of occurrence less than 10-2 but greater than 10-3 in that life.:
Unlikely but can reasonably be expected to occur. Unlikely but possible to occur in the life of an item with a probability of occurrence less than 10-3 but greater than 10-6 in that life.:

g. TYPE AND SOURCE OF INJURY/ILLNESS (CAUSE) - Type and Source Codes are used to describe what caused the incident. The Type Code stands for an ACTION and the Source Code for an OBJECT or SUBSTANCE. Together, they form a brief description of how the incident occurred. Where there are two different sources, code the initiating source of the incident (see example 1, below). Examples:
(1) An employee tripped on carpet and struck his head on a desk. TYPE: 210 (fell on same level) SOURCE: 0110 (walking/working surface).
NOTE: This example would NOT be coded 120 (struck against) and 0140 (furniture).
(2) A Park Ranger contracted dermatitis from contact with poison ivy/oak.
TYPE: 510 (contact) SOURCE: 0920 (plant)
(3) A lock and dam mechanic punctured his finger with a metal sliver while grinding a turbine blade.
TYPE: 410 (punctured by) SOURCE: 0830 (metal)
(4) An employee was driving a government vehicle when it was struck by another vehicle.
TYPE: 800 (traveling in) SOURCE: 0421 (government-owned vehicle, as driver)
NOTE: The Type Code 800, "Traveling In" is different from the other type codes in that its function is not to identify factors contributing to the injury or fatality, but rather to collect data on the type of vehicle the employee was operating or traveling in at the time of the incident.
Select the most appropriate TYPE and SOURCE identifier from the list below and enter the name on the line and the corresponding code in the appropriate box.
