A usage report is required to be submitted to the COR and KO within 72 hours after the last day of performance identified on individual orders.

5.1.2 More than four untimely deliveries or pick-ups on any one or more orders placed in a 12-month period will result in an order reduction, equal to the daily rental price times the number of units delivered or picked up untimely and the number of days late.

A late delivery or pick-up of multiple items on the same day is considered one untimely delivery or pick-up. For example, if an order for six units results in one or more units being delivered one day late, the daily unit rate shall be subtracted from the contractor’s invoice. If the same six units are delivered three days after the date specified on an order, the daily unit rate will be multiplied by three and subtracted from the contractor’s invoice. If six units on one order are delivered a day late and picked up three days late, the daily unit rate will be multiplied by four and subtracted from the contractor’s invoice.

More than four orders with untimely delivery or pick-up in a 12-month period will result in a less than satisfactory rating on the contractor’s annual review and may result in the agreement being cancelled.

5.1.3 The Contracting Officer’s Representative (COR) specified in an individual order may reduce quantities with at least 48 hours’ notice prior to the first delivery date at no cost to the Government unless otherwise specified in an individual order. The COR specified for an individual order may also change the delivery and/or pick-up dates by a maximum of one week with at least two weeks’ notice prior to the first delivery date at no cost to the Government unless otherwise specified on an individual order. 5.2.1.2 Cleaning of portable hand-washing stations includes water re-fill and restocking of supplies to maintain initial stocking levels.

5.2.1.3 Shower trailer rental shall include maintenance repair and deficient equipment replacement services. Upon notification by the COR of a need for repair or equipment replacement, the contractor shall be on-site within an 8-hour response time with repair or replacement within 24 hours.

5.2.2 More than four untimely or non-conforming services on any one or more orders placed in a 12-month period will result in a reduction, equal to the additional cleaning price times the number of units not serviced timely, or not in compliance with the PWS, and the number of days late. A late service of multiple units on the same day is considered a multiple occurrence. For example, an order which includes weekly rental of six portable latrines is not serviced for three days, the rate for an additional cleaning on the Blanket Purchase Agreement (BPA) will be multiplied by six (six units x one day late) and subtracted from the contractor’s invoice. More than four orders with untimely or non-conforming services within a 12-month period will result in a less than satisfactory rating on the contractor’s annual review and may result in the agreement being cancelled.

5.2.3 An individual order issued against this BPA may include service more frequently than every two days. If more frequent services are required, the additional service quantity shall not include services already included every two days. For example, if a unit requests one week rental (Friday to Friday) of a portable latrine with daily service, the quantity for the additional cleaning line-item will be three (Saturday, Monday, Wednesday).
