STATEMENT OF WORK

REPALCE BLDG 48 EPDM ROOFS

KANSAS AIR NATIONAL GUARD, McCONNELL AFB

PRQE172478

5 January 2018

SW-1 LOCATION: McCONNELL Air Force Base, Wichita, KS. Arrangements for entrance onto the Base must be made by contacting Base Engineering at (316) 759-7492 between the hours of 7:00 a.m. and 4:00 p.m., Monday through Friday or refer to SW-10 below for long term access.

SW-2 SCOPE OF WORK: The work covered in this contract consists of furnishing all labor, expertise, equipment, materials, facilities, transportation, and appliances necessary to perform operations in connection with the work described in paragraph SW-3. It is the intent of the government for the contractor to provide a complete and useable system. Any relevant details required for a complete system that is omitted in the contract plans and specifications will be provided by the Contractor at no cost to the government.

SW-3 SUMMARY OF WORK: Remove and replace approximately 2200 SF of EPDM roofing at building 48 with 60-Mil (TPO) Thermoplastic olefin roofing systems. Remove and replace existing roof board and insulation as required.

See attached Drawings for details.

SW-4 CODES & STANDARDS: All work performed under this contract shall be in accordance with federal, state and local codes & standards, as well as widely accepted industry standards, including but not limited to: American Institute of Electrical Engineers, American Society of Heating, Refrigerating and Air-Conditioning Engineers, American Society of Mechanical Engineers, American Society of Testing Materials, National Bureau of Standards, National Electric Code, IEEE, National Fire Protection Association, Occupational Safety & Health Administration, and Underwriters Laboratories.

Additionally, all requirements of the Kansas Department of Health must be satisfied in their entirety for any work performed or materials bought under this contract. Failure to comply with any of the aforementioned codes and standards shall be considered as a failure to comply with contract specifications.

SW-5 UTILITIES: Contractors shall coordinate with all local utilities having jurisdiction: Kansas Gas and Electric, Wichita Water Dept., Southwestern Bell, AT&T, Wichita Sewer Dept. and the Base Engineers Office.

SW-6 CALL BEFORE YOU DIG: Contractors shall apply for a digging permit to the Base Engineers Office for any excavations or penetrations into the ground, greater than 4” deep. The contractor should expect a minimum of 7 days from the date of the request to the issuance of a permit. The Contractor is encouraged to attend the Digging Permit Meeting.

These meetings are scheduled for every Tuesday at 10:00 A.M.

SW-7 CONFINED SPACE: NA to this Project

SW-8 MATERIALS: The materials shall conform to specifications, applicable industry codes and standards and the manufacturer's recommendations, whichever is more stringent. Certificates of compliance and Technical Data Sheets are required for all materials supplied.

SW-9 SITE INSPECTION: The Contractor is required to acknowledge, at time of Bid Opening that said contractor has inspected, and is fully cognizant of conditions at the site. No allowance or consideration will be given after award for any condition(s) which was (were) existing at the time of the site visit.

SW-10 BASE ACCESS: Contractors will be provided a Base Access Form that must be filled out for any individual requiring access to the Base for this project. The completed forms must be e-mailed to the POC who will forward them to Security to be processed. The security check process usually takes a minimum of five days. Individuals must provide picture identification and if driving they will need a valid driver’s license, auto registration and proof of insurance at the time they pick up their Base Passes or Badges at the Visitor Center located at the East Gate off Rock Road. After Contractors have their Pass or Badge they should enter the Base through the Inspection Area at the West Gate. Access is limited to travel from the Gate to the job site. All other areas are off limits. Vehicle Passes are not transferable to other vehicles. The Government reserves the right to deny access to any person, with no consequence to the contract.

Access and General Protection/Security Policy and Procedures. This standard language is for contractor employees with an area of performance within Government controlled installation, facility, or area. The contractor shall comply with all applicable installation/facility access and local security policies and procedures, which may be obtained from the Contracting Officer’s Representative (COR). The contractor shall also provide all information required for background checks to meet installation access requirements to be accomplished by the local installation’s Security Forces, Director of Emergency Services or local Security Office. The contractor shall ensure compliance with all personal identity verification requirements as directed by DOD, HAF and/or local policy. Should the Force Protection Condition (FPCON) change, the Government may require changes in contractor security matters or processes.

For Contractors that do not require CAC, but require access to a DoD Facility and/or Installation. Contractor and all associated sub-contractors employees shall comply with adjudication standards and procedures using the National Crime Information Center Interstate Identification Index (NCIC-III) and Terrorist Screening Database (TSDB) (AFI 31-101 and AFI 10-245), applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative), or, at OCONUS locations, in accordance with status of forces agreements and other theater regulations.

SW-11 SAFETY: Compliance with OSHA and other applicable laws and regulations for the protection of employees is exclusively the obligation of the commercial contractor. Government employees must comply with AFOSH.

SW-12 CLEAN-UP: Clean-up and remove debris generated by this contract to an approved off-base location. Two (2) copies of the receiving (tipping or dumping) tickets, with a cover letter from the Prime Contractor stating the full name and address of the location to which debris was taken.

SW-13 GOVERNMENT RESOURCES: No Government resources shall be made available to the contractor, unless otherwise stated in this Statement of Work. The contractor shall provide for all necessary resources to provide a complete and usable facility or systems as described in the contract documents. The contractor shall not obligate Government resources in any manner. The Government shall seek remedy for any obligation or encumbrance imposed by the Contractors actions. Government resources include, but are not limited to, government owned materials, equipment, tools, vehicles, personnel and facilities.
