MATOC TASK ORDER SCOPE OF WORK

Mississippi River Levee Maintenance, Ramsey Creek Scour Repair

PROJECT LOCATION

Scott County, Missouri.

SCOPE OF WORK

The work consists of providing all plant, labor, and equipment necessary to repair existing bank scour according to the plans and specifications along Ramsey Creek near Scott City, MO. The work for this project will be the first Task Order for the MATOC.

1.1 LINE ITEMS

This Task Order will be awarded as one lump sum with unit prices required for specifically selected work. 1.2 NOTES

NOTE 1: Bidders shall furnish unit prices for all items listed on the schedule of bid items, which require unit prices. If the bidder fails to insert a unit price in the appropriate blank for required items, but does furnish an extended total or an estimated amount for such items, the Government will deem his unit price to be the quotient obtained by dividing

BIDDER OMITS BOTH THE UNIT PRICE AND THE EXTENDED ESTIMATED AMOUNT FOR ANY

ITEM, HIS BID WILL BE DECLARED NONRESPONSIVE.

In the event there is a difference between a unit price and the extended total, the unit price will be held to be the intended bid. If the bidder shows only the total price but fails to enter a unit price, the total divided by the estimated quantity will be held to be the intended unit price.

Any bid price for items indicated above which are unbalanced as to price may be rejected as non-responsive. An unbalanced bid is one which is based on price significantly less than cost for some work and price which is significantly overstated for other work.

Award will be made as a whole to one bidder.

MEASUREMENT AND PAYMENT

PART 1 GENERAL

1.1 TASK ORDER PAYMENT ITEMS

Payment items for the work of this contract for which contract job payments will be made are listed in the BIDDING SCHEDULE and described below. All costs for items of work, which are not specifically mentioned to be included in a particular job or unit price payment item, shall be included in the listed job item most closely associated with the work involved. The job price and payment made for each item listed shall constitute full compensation for furnishing all plant, labor, materials, and equipment, and performing any associated Contractor quality control, meeting safety requirements, tests and reports, and for performing all work required for which separate payment is not otherwise provided.

1.1.1 Mobilization and Demobilization

a. Measurement

No measurement will be made for Mobilization and Demobilization.

b. Payment

Payment will be made for costs associated with mobilization and demobilization, as defined in Contract Clauses PAYMENT FOR MOBILIZATION

AND DEMOBILIZATION.

c. Unit of measure, Job: JA

1.1.2 Environmental Protection

a. Measurement

No measurement will be made for Environmental Protection.

b. Payment

Payment will be made for costs associated with operations necessary for environmental protection as specified in Section 01 57 20.00 11 ENVIRONMENTAL PROTECTION. This price and payment shall be considered full compensation for furnishing all plant, labor, materials, and equipment and for performing all operations necessary for "Environmental Protection." This price and payment does not include those items covered under "Storm Water Pollution Prevention;" however, it does include items incidental thereto. The Contractor shall be responsible for payment of fees associated with environmental permits, application, and/or notices obtained by the Contractor. The Contractor shall be responsible for payment of all fines/fees arriving from violation or non-compliance with Federal, State, Regional, and local

SECTION 01 22 00.00 11 Page 2 laws or regulations. The Government will not provide any compensation for payments of fines/fees, in the event that fines/fees are incurred by the Contractor.

c. Unit of measure, Job: JA

1.2 Stormwater Pollution Prevention Plan

a. Measurement

No measurement will be made for Stormwater Pollution Prevention Plan.

b. Payment

Payment for Stormwater Pollution Prevention Plan (SWPPP) will be made at the job price for "Stormwater Pollution Prevention Plan", as broken down by the percentages listed under Stormwater Pollution Prevention below. Price and payment shall constitute full compensation for initial Contractor creation and submittal of a SWPPP and application for temporary construction stormwater permit to the appropriate state agency, obtaining SWPPP approval and permit from said agency, continuous updating of the SWPPP and all other operations incidental thereto for the duration of the construction activity, and filing the notice of termination upon reaching final stabilization of the site.

c. Unit of measure, Job: JA

1.3 Channel Shaping

a. Measurement

No measurement will be made for Channel Shaping.

b. Payment

Payment for Channel Shaping will be made at the job price for "Channel Shaping". Price and payment shall constitute full compensation for furnishing all material and equipment and performing all labor for shaping the channel bottom and placing the material as shown in the contract drawings. Price and payment shall include drifting of loose channel material above the design grade to low flow channels below the design grade, the placement of excess material not used in achieving the design grade in the scour hole downstream of STA. 3+90, processing or removal of material greater than 30", and removal of deliterious material from the channel bottom all in accordance with SECTION 31 23 00.00 11 CHANNEL SHAPING. Compaction of channel bottom material and removal of sound bedrock will not be required under this contract.

1.4 UNIT PRICE PAYMENT ITEMS

Payment items for the work of this contract on which the contract progress payments will be based are listed in Section 00 41 00.00 11 - BIDDING SCHEDULE and are described below. The unit price and payment made for each item listed shall constitute full compensation for furnishing all plant, labor, materials, and equipment, and performing any associated Contractor quality control, meeting safety requirements, tests, and reports, and for performing all work required for each of the unit price items.

1.4.1 Storm Water Pollution Prevention

No separate measurement or payment will be made for the work "Storm Water Pollution Prevention" required by Section 01 57 25.00 11 STORM WATER POLLUTION PREVENTION PLAN (SWPPP) except as described in the paragraphs SILT FENCE, EROSION CONTROL BERM and CONSTRUCTION EXIT below. Any additional costs associated with Storm Water Pollution Prevention, other than these listed, shall be included in the contract price under Environmental Protection. The Government will pay all costs for the "Storm Water Pollution Prevention" at the contract unit price for each sub-item listed below as follows:

(1) Sixty percent of the unit price upon completion of the installation.

(2) The remaining 40 percent throughout the contract life, with a maximum of 20 percent being retained until final removal and cleanup of the premises.

The Contracting Officer will require the Contractor to furnish cost data to calculate the exact percentage to be paid upon installation.

1.4.1.1 Silt Fence

a. Measurement

Measurement for Silt Fence satisfactorily placed will be made by the linear foot.

b. Payment

Payment for Silt Fence as specified in Section 01 57 25.00 11 STORM WATER POLLUTION PREVENTION PLAN will be made at the contract unit price per linear foot for "Silt Fence" as broken down by the percentages above. Price and payment shall constitute full compensation for furnishing all plant, labor, materials and equipment, including geotextile fabric, and performing all operations necessary for the placement and maintenance of Silt Fence throughout the contract period, including final dressing and cleanup.

c. Unit of measure, Linear Foot: LF

1.4.1.2 Construction Exit

a. Measurement

Measurement for Construction Exit satisfactorily designed, constructed and maintained by the Contractor will be per each placed.

b. Payment

Payment for the temporary Construction including its maintenance and removal, will be made at the contract unit price for "Construction Exit " as broken down by the percentages above. Price and payment shall constitute full compensation for furnishing the design and all plant, labor, equipment, mechanical street sweeper and material to complete the work as specified herein and as shown on drawings including final dressing and cleanup.

c. Unit of measure, Each: EA

1.4.2 Clearing

a. Measurement

Measurement for Clearing shall be made by the acre, measured along the areas of work for embankment, excavation, and/or any other areas requiring clearing as indicated on the plans. For measurement purposes, the acreage will be computed to the nearest 1/10 of an acre for the satisfactorily completed and accepted work.

b. Payment

Payment for Clearing as specified in this section will be made at the contract unit price per acre for "Clearing," which price and payment shall constitute full compensation for furnishing all plant, labor, material, equipment, and performing all operations necessary for "Clearing" as specified in SECTION 31 11 00.00 11 - CLEARING, GRUBBING, AND STRIPPING. If regrowth of vegetation or trees occurs after Clearing and before placement of riprap, the Contractor is required to clear again prior to riprap construction, no payment will be made for this additional clearing.

c. Unit of measure, Acre: AC.

1.4.3 Riprap

a. Measurement

Measurement for Riprap shall be by the ton (2,000 pounds), measured by weighing each truckload to the nearest 1/10 ton, and the final quantity of the whole sum for Riprap rounded to the nearest whole ton. When Riprap is delivered by truck, the material shall be measured for payment, in the presence of a Government Inspector, unless the Contracting Officer waives such right, by weighing on approved, accurately calibrated scales furnished by and at the expense of the Contractor. Scales shall be of sufficient length to permit simultaneous weighing of all axle loads and shall be inspected, tested and sealed as directed to assure accuracy within 0.5 percent throughout the range of the scales. Scales shall be checked and certified before any material hauling; also, rechecked and re-certified whenever a variance is suspected. Individual weight tickets for each truckload of Riprap shall be furnished to the Government Inspector at the time of delivery. At a minimum, all weight tickets shall have the contract number or project name, the gross weight, the vehicle identity and empty weight, and the net load weight. Scales that produce this information printed on the weight tickets are preferred. Weight certificates furnished by a Public Weighmaster, where available, will be acceptable, when authorized by the Contracting Officer. The Contractor has the option to use certified, and otherwise acceptable, scales that are Contractor furnished, quarry/supplier furnished, or commercial/public facilities.

b. Payment

Payment for Riprap will be made at the contract unit price per ton for "Riprap, R-400," which price and payment shall include all costs of

SECTION 01 22 00.00 11 Page 5 furnishing, hauling, handling, placing and maintaining the Riprap, as specified in SECTION 35 31 19.00 11 STONE PROTECTION. Excavation for the placement of riprap shall be considered incidental to this item of work.

c. Unit of measure, Ton: TN

1.4.4 Seeding & Mulching

a. Measurement

Measurement for Seeding & Mulching shall be by the acre, measured along the slopes of the areas seeded within the right-of-way limits and computed to the nearest 1/10 of an acre for the satisfactorily completed and accepted work. Measurement for Seeding & Mulching shall be limited to those areas specified in the drawings or indicated in the field by the Contracting Officer.

b. Payment

Payment for Seeding & Mulching will be made at the contract unit price per acre for "Seeding & Mulching," which price and payment shall constitute full compensation for furnishing all plant, labor, equipment, materials, and performing all operations necessary for Seeding & Mulching as specified in SECTION 32 92 35.00 13 SEEDING & MULCHING - SOUTHEAST MO (LRDD). No payment will be made until acceptance by the Contracting Officer or his/her representative.

The work covered by this section consists of furnishing all plant, labor, equipment, and materials, and performing all operations necessary for excavation to the lines and grades indicated on the drawings, and/or as specified herein.

1.2 DEFINITIONS

Channel Shaping - Shaping shall consist of the grading of existing channel bottom inside the banks of the channel to the lines and grades shown on the drawings.

1.4 QUALITY CONTROL

The Contractor shall establish and maintain quality control for all operations to assure compliance with contract requirements, and shall maintain records of his quality control for all construction operations, including but not limited to the following:

a. Excavation

Layout, grades, widths, slopes, alignment, transitions, , bottom grades and widths, side slopes, berm widths, lateral ditches, V-type roadside ditches, inlet drains, riprap paved inlet drains, excavation for placement of stone protection, transitions.

b. Disposal of Excavated Material

Layout, limits, maximum elevation, restricted areas, drainage, slopes, minimum elevations.

c. Existing Channel

Temporary traps to catch drift, location, limits, right-of-way agreements between the Contractor and landowners where work is outside the right-of-way shown on the drawings.

A copy of the inspection and tests, as well as the records of corrective action taken, shall be furnished the Government.

PART 2 PRODUCTS - NOT USED

PART 3 EXECUTION

3.1 CHANNEL SHAPING

The limits of work shall be from Centerline Station 3+00-7+60 along Ramsey Creek. Prior to commencing work placing riprap, grade the existing channel bottom to the lines, grades, and elevations shown. Final grade shall be within 3 inches, plus or minus, of the grades and elevations indicated on the drawings except in the vicinity of the sound bedrock extending above the final grade. All sound bedrock shall remain. No rock ripping or blasting will be allowed. In the event that exposed bedrock is higher than the design finish grade, grade the channel bottom in order to smoothly transition from design grade to that existing bedrock. Acceptable excess material resulting from the shaping operations shall be placed in the area

The rock or rubble cleared during the channel shaping operation that has any dimension greater than 30" should either be removed from the site or processed into pieces less than 30". Debris shall be disposed as prescribed in the "DISPOSAL OF DEBRIS MATERIAL" paragraph below.

3.2 DISPOSAL OF DEBRIS MATERIAL

Debris material encounted during channel shaping or excavation shall be removed and disposed off-site. Any deliterious or debris material such as tires, wood, organics, plastic, steel, or rubber that is encountered during clearing, excavating, or shaping operations shall be removed from the channel and disposed off-site. The Contractor shall perform all hauling of material as may be necessary to dispose of deliterious material and no separate payment will be made for hauling operations.

3.3 EXISTING CHANNEL

The Contractor shall construct temporary traps to catch drift and/or conduct his operations in such manner that the improved channel within the limits of work, and the existing channel below the limits of the work will not be deteriorated. Any drift racks, temporary crossings or other obstructions within such channel caused through the fault or negligence of the Contractor shall be removed as directed by the Contracting Officer at the expense of the Contractor. In the performance of this work, the necessary right of way for ingress, egress and operation of equipment and area for disposal of excavated material and debris resulting from such obstruction removal outside the right of way limits indicated on the drawings shall be obtained by the Contractor from the landowners at the expense of the Contractor. Three copies of the agreement between the Contractor and each landowner granting the Contractor permission for the use of lands for such purpose shall be furnished the Contracting Officer before the Contractor's entry thereon.

3.4 CULVERTS AND DRAINAGE DITCHES

All culverts that are encountered shall not be damaged, stopped up, or have drainage impeded. Any damage shall be corrected as directed by the Contracting Officer at the Contractor's expense. Drainage ditches without culverts are to be left open and if clogged up shall be opened by the Contractor.

PART 2 PRODUCTS

2.1 MATERIALS

2.1.1 Seed

The Contractor shall furnish seed of the latest season's crop labeled in conformance with AMS Seed Act and applicable State seed laws. Seed shall be furnished in original sealed packages, bearing producer's guaranteed analysis for percentages of mixtures, purity, germination, weedseed content, and inert material, unless written exception is granted. Field mixes will be acceptable when field mix is performed on site in the presence of the Contracting Officer. Seed that is wet or moldy or that has been otherwise damaged in transit or storage will be rejected. Between 1 December and 30 May, inclusive, seed mixtures shall conform to one or more of the following "Sample Custom Mixtures" in Tables 1 and 2 provided below, unless otherwise approved by the Contracting Officer. Between 1 September and 30 November, inclusive, seed mixtures shall conform to the mixtures provided in Table 3 provided below, unless otherwise approved by the Contracting Officer. Between 1 June and 31 August, inclusive, temporary seed mixtures shall conform to the mixtures provided in Table 4 provided below, unless otherwise approved by the Contracting Officer.

2.1.2 Mulch

The Contractor shall use mulch material to protect the seeded areas as well as reduce erosion. The material used for mulching shall be threshed straw from wheat, rye, barley, or rice, applied at a rate of 2 tons per acre (4,000 lbs/AC). Furnish mulch in air-dry condition and of proper consistency for placing with commercial mulch blowing equipment. Straw shall contain no fertile seed. Mulch shall be free from noxious weeds, mold, and other deleterious materials. Use of hay for mulch will not be permitted. The Contractor shall be prepared to submit, on request, Mulch Supplier Certification.

2.1.3 Soil for Repairs

For fill of areas to be repaired, soil shall be of a quality at least equal to that which exists in areas adjacent to the area to be repaired. Soil used shall be free from roots, stones, and other materials that hinder grading, planting, and maintenance operations and shall be free from objectionable weed seeds and toxic substances.

2.2 SAMPLING, TESTING, AND CERTIFICATES

Sampling and testing shall be the responsibility of the Contractor and shall be performed at no additional cost to the Government. Sampling and testing shall be performed by a recognized testing agency.

2.2.1 Seed

The Contracting Officer shall be furnished duplicate signed copies of statements certifying that each container of seed delivered is labeled in accordance with the AMS Seed Act and is at least equal to the requirements specified in the paragraph entitled "Seed" above. This AMS Seed Act Certification shall be obtained from the supplier and shall be furnished on or with all copies of Seed Invoices.

PART 3 EXECUTION

3.1 AREAS TO RECEIVE SEEDING AND MULCHING

Seeding and mulch shall be applied at any areas within the right-of-way limits, outside of top bank, denuded of grass by construction and/or clearing operations.

3.2 COMMENCEMENT, PROSECUTION, AND COMPLETION

Seeding and mulch shall be placed no more than 14 days after construction activity temporarily or permanently ceases on any portion of the site.

Prior to prosecuting the seeding operation, the Contractor shall repair rainwash, if any, dress, and prepare the areas for seeding and mulching.

3.3 PREPARATION OF GROUND SURFACE

After areas have been brought to the indicated finish grade, remove debris and stones larger than 3/4 inch remaining on the surface. Correct irregularities in finish surfaces to eliminate depressions. Provide equipment, in good condition, for the proper preparation of the ground and for handling and placing all materials.

3.3.1 Clearing

Prior to grading and finish dressing, vegetation that may interfere with seeding and mulching operations shall be removed and shall be disposed of by removal from site and/or burning. The surface shall be cleared of roots, cable, wire, and other materials that might hinder the work or subsequent maintenance.

3.3.2 Dressing

Surfaces to receive seeding and mulching, shall be dressed to the extent necessary to provide drainage and the specified slopes, and as necessary to remove high points and fill depressions sufficiently to provide reasonably smooth surfaces. Surfaces shall be prepared by dressing so as to produce smooth profiles, crown widths, and end slopes.

3.4 SEEDING

3.4.1 General

Seed sown during the season between 1 December and 31 May, inclusive, shall consist of one or more of the mixtures indicated in Tables 1 or 2 depending on the equipment used for seeding. Seed sown during the season between, 1 September and 30 November, inclusive, shall consist of the mixtures provided in Table 3. Use temporary seeding mixtures from Table 4 during the season between 1 June and 31 August, inclusive. Reseed temporarily seeded areas with seed mixtures from Tables 1 or 2 once within the appropriate seasonal time frame. No additional ground preparation or mulching will be required for temporary areas requiring reseeding. The contractor may submit an alternate seeding mixture, for Contracting Officer Representative approval, consisting of the seed listed in Tables 1 and 2 but at different rates based on availability. A satisfactory method of sowing shall be employed, using approved Native Grass Drills, broadcast-seeders, or other approved methods. When conditions are such by reason of drought, high winds, excessive moisture, or other factors that satisfactory results are not likely to be obtained, seeding shall be halted as directed and resumed only when conditions are favorable or when Contracting Officer Respresentateive approved alternative or corrective measures and procedures have been implemented. If inspection either during seeding operations or after there is a show of green indicates that areas have been left unplanted, additional seed shall be sown if so directed.

3.4.2 Methods for Seeding

Conduct thorough tillage as required so that all vegetation is controlled.

Provied a smooth surface for ample fine soil particles to insure good seed coverage and soil contact. Roll or culti-pack seedbed before seeding so that ground surface is firm enough for a person on foot to sink less than 1/2". Use a "Brillion-type" seeder or a drill with seed box and metering equipment designed for the seed being sown. Place the seed at 1/8" to 1/4" and use press wheels to insure good seed-soil contact. Seed shall be drilled or broadcasted with approved sowing equipment and distributed uniformly over the areas. Seed shall not be broadcast during winds in excess of 10 miles per hour.

3.4.3 Compacting

Immediately upon completion of seeding operations, surfaces shall be compacted by one pass of a cultipacker, corrugated roller, or other

SECTION 32 92 35.00 13 Page 7 approved equipment weighing 100 to 160 pounds per linear foot of roller.

Drilled seed does not require follow-up compaction.

3.4.4 Damage to Seeding

The Contractor shall be fully responsible for any damage to the seeded areas caused by his operations. Areas that become damaged shall be repaired without additional cost to the Government. Additionally, areas that become damaged as a result of poor workmanship or failure to meet the requirements of the specifications may be ordered to be repaired and reseeded to specification requirements, without additional cost to the Government

3.5 APPLICATION OF MULCH

Mulch shall be spread uniformly in a continuous blanket, using 2 tons per acre of straw mulch. Straw mulch shall be spread either by hand or by a manure spreader or by a modified grain combine with straw-spreader attachment or by a blower-type mulch spreader. Mulching shall be started at the windward side of relatively flat areas, or at the upper part of a steep slope, and continued uniformly until the area is covered. The mulch shall not be bunched. Immediately after spreading, straw mulch shall be anchored to the soil by a V-type-wheel land packer, a scalloped-disk, tackifier, punch roller, or other suitable equipment operated parallel to the embankment centerline. Straw mulch with tackifier shall not be applied during or immediately before rainfall. The Contracting Officer will determine the number of passes needed for anchoring, not to exceed three.

3.6 INSPECTION AND ACCEPTANCE

Acceptance of the seeded and mulched areas will be determined by visual inspection. Existence of rainwash damage will not be acceptable. 60% of the unit price payment for seeding and mulching will be made upon application of seeding and mulching and the remaining 40% of the unit price payment will be made upon final acceptance by the Contracting Officer.

Acceptance of the turfing areas may be obtained in 2,000 intervals along the channel.
