Performance Work Statement (PWS)
Landfills Fencing Installation
Mountain Home Air Force Base, Idaho
Part 1
General Information
1. GENERAL: This Performance Work Statement (PWS) is a non-personal services contract to install fencing at the landfills located at the Mountain Home Air Force Base, ID. The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government.
1.1. Description of Projects/Introduction: The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, testing, and other items necessary to service an estimated of 9,600 linear feet of 6 feet high galvanized chain link fence and 13,000 liner feet of 4 feet high cable barrier at the lagoon landfills located on Mountain Home Air Force Base (MHAFB), Idaho.
1.2 Background: Security fencing was required surrounding the landfills.
1.3 Objectives: Install fencing and cable barrier around the limited use controls on these closed landfills which the state, federal and base have agreed upon.
1.4 Scope: Provide all labor and materials necessary to execute this PWS. The contractor shall work as directed in the PWS and which is administered by the
Government’s Contracting Officer (KO).
1.5. Period of Performance: Period of performance shall be 365 days from date of
Notice to Proceed (NTP).
1.6 General Information
1.6.1 Quality Control and Work Plan (QC/WP). Develop and maintain an effective (QC/WP) to ensure services are performed in accordance with this PWS. Develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The Contractor’s QC/WP is the means by which any work performed is verified to comply with the requirements of the PWS. The QC/WP must be provided within five calendar days from the NTP date and resubmitted within five calendar days from any changes that occur. Field work shall not start until the QC/WP has been approved by the Government. The Government reserves the right to require the Contractor to make changes in his CQC Plan and operations including removal of personnel, as necessary, to obtain the quality specified.
1.6.2 Quality Assurance: The Government shall evaluate the Contractor’s performance under this contract in accordance with the Quality Assurance Surveillance Plan (QASP).
This plan is primarily focused on what the Government must do to ensure that the contractor has performed in accordance with the performance standards. It defines how the performance standards will be applied, the frequency of surveillance, and the minimum acceptable defect rate(s).
1.6.3 Recognized Holidays: The contractor is not required and typically does not perform work on the installation during the following Federal holidays.

1.6.4 Hours of Operation: The Contractor’s working hours shall be from 0730 to 1630
Monday thru Friday except Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. Note: When possible, the Government will allow the
Contractor to continue to perform duties in the event of an administrative closing.
The Contractor must at all times maintain an adequate workforce for the uninterrupted performance of all tasks defined within this PWS when the Government facility is not closed for the above reasons. When hiring personnel, the Contractor shall keep in mind that the stability and continuity of the workforce are essential.
1.6.5 Place of Performance:
The work to be performed under this contract will be at Mountain Home Air Force Base, Idaho. The Contractor will be required to provide all necessary documentation for both personnel and vehicles to gain base entrance.
1.6.6 Work Coordination
1.6.6.1 Interfacing With Others
Plan work activities in such a manner that there will be minimal interference and inconvenience to ongoing traffic and daily operations in landfills and local areas.
Schedule all work and coordinate with the Government Project Management Team or their designee at all times.
Do not block traffic, roads, access routes, or exits without prior coordination and authorization from the Contracting Officer’s Representative (COR) or other authorized
Government representative.
1.6.6.2 Pre-Construction Conference
Attend a Pre-Construction Conference presided over by the COR prior to commencing construction activities. Contact the COR within five business days of issuance of the
Notice to Proceed (NTP) to establish a date and time for the Pre-Construction

Conference. The Conference will include base orientation and will cover all emergency, safety, environmental, and other procedures for working on Mountain Home AFB.
Submit a complete work schedule within seven days after the Pre-Construction
Conference and update the schedule weekly throughout the performance period. The schedule shall be in sufficient detail to identify all aspects of the work including project start, finish, and all tasks and duration times.

1.6.6.4 Pre-Construction Contractor Quality Control (CQC) Coordination Meeting
1.6.7 Type of Contract: IFB/Firm Fixed Price.
1.6.9 Post Award Conference/Periodic Progress Meetings:
The Contractor shall attend in person a post award conference convened by the contracting activity or contract administration office. The Contracting Officer’s
Representative, and other Government personnel, as appropriate, may meet periodically with the Contractor to review the Contractor's performance. At these meetings the KO will apprise the Contractor of how the Government views the Contractor's performance and the Contractor will apprise the Government of problems, if any, being experienced.
Appropriate action shall be taken to resolve outstanding issues. These meetings will be at no additional cost to the Government. These meetings may be held at Mountain Home
Air Force Base. At the discretion of the KO, these meetings may be held via conference calls.
1.6.9.1 Compliance with Rules, Regulations, and Statutes
All Contractor employees shall observe and comply with all applicable local, State, and
Federal rules, regulations, and statutes including those concerning fire, safety, sanitation, security, vehicle safety, and hazardous material handling while performing this work. All work shall meet or exceed all applicable industry standards.
1.6.9.2 Firearms
Contractor personnel shall carry no weapons or firearms while performing work under this Contract. Contractor personnel shall comply with all related restrictions set by the installation.
1.6.10. Key Government Personnel: The follow personnel are considered key personnel by the government:

1.6.11. Key Contractor Personnel and Sub-Contractor Qualifications:
Provide a Contract Manager who shall be responsible for the performance of the work.
The name of this person and an alternate who shall act for the Contractor when the manager is absent shall be designated in writing to the KO. The Contract Manager or alternate shall have full authority to act for the Contractor on all contract matters relating to daily operation of this contract. The Contract Manager or alternate shall be available between 0730 and 1630, Monday thru Friday except Federal holidays or when the facility is closed for administrative reasons. Qualifications for all key personnel are listed below:
1.6.11.1. The Contracting Officer (KO) or the Contracting Officer’s Representative will approve the Contractor’s team members in writing. Provide résumés to the USACE
Contracting Officer’s Representative within 5 days of NTP.
1.6.11.2. Contractor’s Project Manager. The Project Manager must have a minimum of three years experience working within similar technical fields as described in Section
Five below. Additionally, the Project Manager must have a minimum of three years experience leading and managing multiple teams of three or more employees.
1.6.11.3. Contractor’s Alternate Project Manager: The Alternate Project Manager must have a minimum of one year working within similar technical fields.. As a minimum, the
Contractor’s alternate Project Manager must have a minimum of one year experience leading and managing multiple teams of three or more employees.
1.6.11.4. Site Safety and Health Officer
Provide a Site Safety and Health Officer (SSHO) as required in EM 385-1-1 Government
Safety Requirements. The SSHO may be assigned other duties in addition to the duties required as SSHO.
1.6.11.5. Construction Quality Control System Manager
Provide a Construction Quality Control (CQC) System Manager. The CQC System
Manager may be assigned other duties in addition to the duties required as CQC System
Manager. The CQC System Manager shall have 5 years of experience in that capacity on similar projects.
1.6.11.6. Superintendent
The Superintendent shall have Site Safety and Health Officer approved training.

1.6.11.7. Testing
Fence’s gates will be tested and verified to operate as intended. Provide Contractor
Verification Test (CVT), followed by Performance Verification Test (PVT).
1.6.15. Payment: The Contractor shall invoice the Government on a monthly basis.
The invoice, including supporting documentation for work performed, a monthly activity report describing actions taken during the previous 30 days, planned action for the next
45 days, issues, updated project schedule, and a summary table of expenditures broken into contract items, shall be submitted not later than 30 days following the end of each calendar month.
1.6.15.1 QUALITY CONTROL SYSTEM (QCS).
Download and upload the schedule data into the Resident Management System (RMS) prior to RMS databases being transferred to the Government and is considered to be additional supporting data in a form and detail required by the Contracting Officer. The receipt of a proper payment request is contingent upon the Government receiving both acceptable and approvable hard copies and electronic export from QCS of the application for progress payment.

1.6.17. Organizational Conflict of Interest: Contractor and subcontractor personnel performing work under this contract may receive, have access to, or participate in the development of proprietary or source selection information (e.g., cost or pricing information, budget information or analyses, specifications or work statements, etc.) or perform evaluation services which may create a current or subsequent Organizational
Conflict of Interests (OCI) as defined in FAR Part 9 Contractor Qualifications, Subpart
9.5 Organizational and Consultant Conflicts of Interest. The Contractor shall notify the
Contracting Officer immediately whenever it becomes aware that such access or
PWS – Landfill Fencing Installation Mt Home AFB participation may result in any actual or potential OCI and shall promptly submit a plan to the Contracting Officer to avoid or mitigate any such OCI. The Contractor’s mitigation plan will be determined to be acceptable solely at the discretion of the
Contracting Officer and in the event the Contracting Officer unilaterally determines that any such OCI cannot be satisfactorily avoided or mitigated, the Contracting Officer may implement other remedies as he or she deems necessary, including prohibiting the
Contractor from participation in subsequent contracted requirements which may be effected by the OCI.
PART 2
DEFINITIONS & ACRONYMS
2. DEFINITIONS AND ACRONYMS:
2.1. DEFINITIONS:
2.1.1. CONTRACTOR. A supplier or vendor awarded a contract to provide specific supplies or service to the government. The term used in this contract refers to the prime.
2.1.2. CONTRACTING OFFICER (KO). A person with authority to enter into, administer, and or terminate contracts, and make related determinations and findings on behalf of the government. Note: The only individual who can legally bind the government.
2.1.3. CONTRACTING OFFICER’S REPRESENTATIVE (COR). An employee of the
U.S. Government appointed by the contracting officer to administer the contract. Such appointment shall be in writing and shall state the scope of authority and limitations.
This individual has authority to provide technical direction to the Contractor as long as that direction is within the scope of the contract, does not constitute a change, and has no funding implications. This individual does NOT have authority to change the terms and conditions of the contract.
2.1.4. DEFECTIVE SERVICE. A service output that does not meet the standard of performance associated with the Performance Work Statement.
2.1.5. DELIVERABLE. Anything that can be physically delivered, but may include non-manufactured things such as meeting minutes or reports.
2.1.6. KEY PERSONNEL. Contractor personnel that are evaluated in a source selection process and that may be required to be used in the performance of a contract by the Key
Personnel listed in the PWS. When key personnel are used as an evaluation factor in best value procurement, an offer can be rejected if it does not have a firm commitment from the persons that are listed in the proposal.
2.1.7. PHYSICAL SECURITY. Actions that prevent the loss or damage of Government property.
2.1.8. QUALITY ASSURANCE. The government procedures to verify that services being performed by the Contractor are performed according to acceptable standards.
2.1.9. QUALITY ASSURANCE Surveillance Plan (QASP). An organized written document specifying the surveillance methodology to be used for surveillance of
Contractor performance.

2.1.10. QUALITY CONTROL. All necessary measures taken by the Contractor to assure that the quality of an end product or service shall meet contract requirements.
2.1.11. SUBCONTRACTOR. One that enters into a contract with a prime Contractor.
The Government does not have contract with the subcontractor.
2.1.12. WORK DAY. The number of hours per day the Contractor provides services in accordance with the contract.
2.1.12. WORK WEEK. Monday through Friday, unless specified otherwise.

GOVERNMENT FURNISHED PROPERTY, EQUIPMENT, AND SERVICES
3. GOVERNMENT FURNISHED ITEMS AND SERVICES:
3.1. Services: None with the exception of shop personnel and other knowledgeable
Government employees providing historical information on the background on each facility, operational concerns, known issues, and other related information that could assist the Contractor in accomplishing their tasks.
3.2 Facilities: Upon request, the Government can provide the Contractor with limited work space during the time work is being conducted in support of this PWS.
3.3 Utilities: General use of utilities (i.e., electric, gas, water) Refer to 10.6 for detailed instructions.
3.4 Equipment: None.
3.5 Materials: The Government will provide copies of various facility and infrastructure as-built drawings and other administrative items which are necessary to help facilitate the requirements of the PWS.

PART 4
CONTRACTOR FURNISHED ITEMS AND SERVICES
4. CONTRACTOR FURNISHED ITEMS AND RESPONSIBILITIES:
4.1 General: Furnish all supplies, equipment, facilities and services required to perform work under this contract with the exception of Government Furnished Items listed in
Sections 3 or Section 5 of this PWS.
4.2 Secret Facility Clearance: None required.
4.3. Materials: General administrative items and tools in support of this PWS with the exception of items listed or identified as Government Furnished Equipment in Sections 3 or Section 5 of this PWS.
4.4. Equipment: General administrative items and tools in support of this PWS with the exception of items listed or identified as Government Furnished Equipment in Sections 3 or Section 5 of this PWS.
4.5 As-Built Information: Provide drawings and other As-Built data at the completion of the project. These documents, in marked up version, shall be available for periodic check throughout the contract period. Final payment shall be withheld if complete As-Built information is not provided.

PART 5
SPECIFIC TASKS
5. Specific Tasks:
5.1. Basic Services. The Contractor shall provide services for the following tasks and in accordance with the timelines established in Section 1.5, Period of Performance:
a. Furnish and install the 6 feet high galvanized chain link fence with standard width galvanized chain link manual man gate entrance and warning signs at every 200 feet at lagoon landfill 002 (LF001) located on MHAFB.
b. Furnish and install and test the 6 feet high galvanized chain link fence with two
(2) 20 feet 6 feet high galvanized manual vehicle swing gate entrances and warning signs at every 200 feet at lagoon landfill 002 (LF002) located on
MHAFB.
c. Furnish and install 4 feet high cable barrier with two (2) 20 feet cable barrier manual vehicle swing gate entrances and warning signs at every 200 feet at lagoon landfill 003 (LF003) located on MHAFB.
5.2. Optional Services. The Contractor shall provide services for the following tasks and in accordance with the timelines established in Section 1.5, Period of
Performance:
a. Furnish and install 4 feet high cable barrier with cable barrier manual man gate entrance and warning signs at every 200 feet at lagoon landfill 023 (LF023) located on MHAFB.
5.2.1. Contractor’s performance shall be measured in accordance with Technical Exhibit
1 Performance Requirements Summary. The Contractor shall be responsible for the execution, completion, and corrective actions pertaining to this PWS. Defective services requiring clarification, corrective action, or rework shall be promptly resolved to the
Government’s satisfaction by the Contractor.
5.2.1.1. Performance Objectives:
The general level of effort required for this task are as follows:
Work to be performed in accordance with laws, codes, regulations, and local requirements.
Monthly Activity Report.
Monthly Activity Report shall provide an overview of activities conducted since last submitted invoice, current activities, and projected activities to be conducted over the next 30 days

Report will be a key source document to justify invoicing, show productivity, ensure planed actions are in line with required priories, and provide a basis for official communication regarding short and long term program recommendations
Level of effort shall be at a minimum to achieve the above stated objects.
Contractor shall work with the USACE Contracting Officer for format and level of detail.
Weekly updated status report when conducting field activities showing schedule of facilities to be audited and percent complete shall be provided to the Contracting Officer every Wednesday. Report shall reflect actual past efforts and updated forecast.
5.2.1.2. Standards:
a. Management and Supervision:
Properly prioritize and execute activities and workflow to meet established program goals and objectives
Plan and schedule Contractor’s personnel activities to meet mission requirements
Ensure employees meet security requirements (Paragraph 1.6.8.2 Physical
Security)
Ensure employees operate equipment and perform services in a safe manner
Meet minimum quality standards as outlined in Technical Exhibit 1.
b. Coordination:
Timely program coordination with USACE Contracting Officer’s
Representative
Timely activity coordination with Mountain Home AFB’s EPM, PW Staff, Mountain Home AFB customers, and other contract employees as necessary to meet mission requirements with no degradation of service or support of this
PWS
5.3. Submittals:
5.3.1. Quality Control and Work Plan (Paragraph 1.6.1)
5.3.2. Résumés (Paragraph 1.6.11)
5.3.3. Monthly Invoices (Paragraph 1.6.15).
5.3.4. Monthly Activity Report (Paragraph 5.2.1.1). Report shall provide an overview of activities conducted since last submitted invoice, current activities, and projected activities to be conducted over the next 45 days. Include an updated tracking sheet for light installations that show: Start date, building number, fixture type, quantities installed, and date of installation.

5.3.5. Weekly Schedule Report (Paragraph 5.2.1.1). Report required when field work is planned or being conducted. Report shall be updated to reflect actual past efforts, future schedule, and percent complete.
5.3.6. Meeting—Contractor shall participate in scheduled meetings that pertain specifically to this PWS. Meetings are estimated to be one physical meeting and one teleconference meeting per month. Meetings will be streamlined with specific agendas, prepared by the Contractor and will generally be kept to within one hour. The Contractor is required to take all meeting minutes and provide meeting minutes to the Government for approval. Minutes will be published in MS Word. Once approved, minutes may be published in PDF format.
5.3.7 Submittals –
5.3.7.1. Submit fixture and lamp product data to COR for review and approval 15 days prior to installation. Product data to include, cut sheets, Manufacturer’s instructions and warranty information if applicable.
5.3.7.2. Provide project schedule to COR for review and approval within 30 days of NTP. Include updated project schedule with Monthly Invoice.
5.4 Contract Administration:
The Government will use the Resident Management System for Windows (RMS) to assist in its monitoring and administration of this contract. The Contractor must use the
Government-furnished Construction Contractor Module of RMS, referred to as QCS, to record, maintain, and submit various information throughout the contract period. The
Contractor module, user manuals, updates, and training information can be downloaded from the RMS web site (http://rms.usace.army.mil). This joint Government-Contractor use of RMS and QCS will facilitate electronic exchange of information and overall management of the contract. QCS provides the means for the Contractor to input, track, and electronically share information with the Government in the following areas:

7.2 Special Contract Clauses
Required insurance in accordance with FAR 28.306(b)
a. In accordance with FAR clause entitled "Insurance Under Fixed-Price Contracts" the Contractor shall, at its own expense, procure and thereafter maintain the following kinds of insurance with respect to performance under the contract. The
Contractor will have the option to complete a form entitled "Notification of
Compliance with Contract Insurance Requirements" that will be provided by the
Government at time of award, or to submit an insurance certificate before any work commences on the installation.
b. Workman's Compensation and Employers Liability Insurance as required by law except that if this contract is to be performed in a state which does not require or permit private insurance, then compliance with the statutory or administrative requirements in any such state will be satisfactory. The required Workmen's compensation insurance shall extend to cover employer's liability for accidental bodily injury or death and for occupational diseases with a minimum liability limit of $100,000.00.
c. General Liability Insurance. Bodily injury liability insurance, in the minimum limits of $500,000.00 per occurrence shall be required on the comprehensive form of policy.
d. Automobile Liability Insurance. This insurance shall be required on the comprehensive form of policy and shall provide bodily injury liability and property damage liability covering the operation of all automobile used in connection with the performance of the contract. At least the minimum limits of $200,000.00 per person and $500,000.00 per occurrence for bodily injury and $20,000 per occurrence for property damage shall be required.

ENVIRONMENTAL
8.1 General Requirements
Minimize environmental pollution and damage that may occur at Mountain Home
AFB (MHAFB) as the result of construction activities. The environmental resources within the project boundaries and those effected outside the work limits must be protected during the entire duration of this Contract. Comply with all applicable local, state, and federal laws and regulations regarding protection of the environment including stipulations from agencies and offices having authority including the US Environmental Protection Agency (EPA), Idaho Department of
Environmental Quality, and Idaho State Historic Preservation Office (SHPO).
Any delays resulting from failure to comply with environmental laws and regulations will be the Contractor’s responsibility. In the event of conflicting requirements, coordinate with the Contracting Officer for resolution.
8.2 Asbestos Containing Material
An Asbestos Hazard Emergency Response Act (AHERA) survey is not required as part of this project.
8.3 Lead-Containing Paint
A lead paint survey is not required as part of this project.
8.4 Sustainable Acquisition
Comply with all applicable Federal, DoD, Air Force, and Mountain Home Air
Force Base sustainable acquisition laws and policies.
In order to comply with federally mandated sustainable acquisition requirements and Department of Defense (DoD) and Air Force Green Procurement Program
(GPP) policies, Mountain Home Air Force Base requires the purchase, supply, and use of environmentally preferable products and services. Program elements include recovered material products, energy and water efficient products, alternative fuels and fuel efficiency, bio-based products, non-ozone depleting substances, and environmentally preferable products. These elements are described on the Office of Federal Procurement Policy website at:

If compliant products are unavailable -- or determined to be impracticable based on cost or performance -- contact the Contracting Officer or their representative for further guidance and exception requirements.
8.6. Environmental Management System
MHAFB Civil Engineering office is responsible for day to day management of the

Base EMS conforms to the criteria defined in the international standard, ISO
14001:2004 Environmental Management Systems – Requirements with guidance for use.
In accordance with EMS procedures, the Contractor shall be familiar with the
Mountain Home Air Force Base environmental policy and shall ensure that this information is considered and incorporated into this project. Information regarding the Mountain Home Air Force Base EMS is available from the installation EMS

8.7 Hazardous Materials
For all hazardous materials, submit a list of hazardous materials to be used including unit of measure and Safety Data Sheet for each product. A copy of these documents must be submitted to the Mountain Home AFB Environmental Management office for approval prior to bringing the materials on-site. Do not use any hazardous materials not authorized by MHAFB Environmental Management.
For any in-use hazardous materials, submit a hazardous material inventory on HJB
Form 953 noting the hazardous materials used, the units of measure, quantities used, storage locations, and other information indicated on the form. This inventory shall be submitted at the end of the project. If the project goes into a new calendar year, submit a copy of this inventory accounting for all hazardous materials by 15 January.
For any hazardous materials stored on Mountain Home Air Force Base, this inventory shall be submitted by the 15th day following the calendar year quarter in which the hazardous materials were used or stored. All HJB 953 inventories shall be submitted electronically and addressed to the Contracting Officer or their representative with usAir Force.Mountain Home Air Force Base.imcom.list.dpw-hm-inventory@mail.mil included in the carbon copy (“cc”) line of the email. 8.8 Construction and Demolition (C&D) Waste Planning and Reporting

8.9 Non-hazardous solid waste disposal and recycling procedures.
The Contractor may coordinate with the MHAFB Recycle Center or Solid Waste
Manager through the Contracting Officer when preparing the plan for advice or suggestions to meet recycling requirements.

2. Identify subcontractors responsible for the transportation and disposal of solid waste. Submit licenses or permits for solid waste disposal sites that are not a commercial operating facility.
3. Evidence of the disposal facility’s acceptance of the solid waste must be attached to this plan.
8.10 Lead-based paint abatement procedures
(containment/work area, clean rooms, load-out, and clearance samples), and handling and transportation of LBP material. Ensure procedures have been reviewed by a certified industrial hygienist and meet applicable OSHA and EPA regulations for lead-based paint abatement or removal projects.
8.11 Air Pollution Control Plan
Provide procedures to assure that dust, debris, materials, trash, etc., do not become air borne and travel off the project site.
8.12.Recycling and Solid Waste
Non-hazardous solid waste stream diversion: Make every practical effort to maximize non-hazardous solid waste and C&D diversion from landfills or incinerators through reuse, donation, recycling, composting and mulching, or other waste diversion activities to optimize reduction in both the volume of solid waste disposed and overall cost of non-hazardous solid waste management. (AFI 32-7042)
Support MHAFB’s recycling policy in accordance with Executive Order 13101, whenever feasible. Consistent with the demands of efficiency and cost effectiveness, incorporate waste prevention and recycling. It is the national policy to prefer pollution prevention. Disposal should be employed only as a last resort.
Do not deposit any refuse in existing containers or dumpsters. Refuse is defined as debris other than organic materials like trees, brush, leaves, grass, stumps, etc.
Dispose/manage limited waste streams to the MHAFB recycling program with the
Government's approval only. The acceptance of these waste streams is dependent on the Government's ability to process the waste stream and the Contractor's ability to adequately prepare the waste stream to meet the Government's condition requirements. The Government currently recycles the following items on MHAFB:
all scrap metal, corrugated cardboard, paper and PET #1 plastic. Segregate all recyclable material prior to delivery to the MHAFB Recycling Center.
Contact the MHAFB recycling center through the Contracting Officer for the latest recycling information concerning what materials are currently accepted and to make arrangements for on-site bins.

Dispose of refuse off site in accordance with all local, state, and federal rules and regulations, following coordination with the 366 CES/CEIE through the Contracting
Officer. Dispose of refuse in containers that are emptied on a regular schedule.
Conduct all handling and disposal of refuse to prevent contamination of the environment.
Submit weight tickets to 366 CES/CEIE through Contracting Officer monthly, or at the end of the project if lasting less than one month, for all non-hazardous solid waste diversion and disposal of construction and demolition debris. Scales available for use are located at LRC, Building 1132.
8.13 Spills
8.13.1 Uncontrolled Spills
In the event of an uncontrolled spill, notify the MHAFB Fire Department by calling
911 if using a phone on the MHAFB system or 208-828-0911. (NOTE: Calling 911 from a cell phone or an off-base phone system will reach emergency services at the
City of Mountain Home and then will be rerouted to MHAFB, delaying response).
MHAFB Fire Department will initiate the notification system.
8.13.2 All Spills
Notify the Contracting Officer of all spills regardless of substance or quantity using the Environmental Hazardous Material Release Report in Attachment 3. The
Contracting Officer will coordinate with 366 CES/CEIE to ensure that all spills have been properly contained, cleaned-up, and reported.

SPECIAL ACCESS REQUIREMENTS
All Contractors who work on base must submit a SFS Form 30 to the 366
Contracting Squadron with the name, driver’s license number, social security number, date of birth, and employer of each employee to obtain a base pass. The
SFS Form 30 is then submitted to the 366 Security Forces Squadron. If approved, a
DBIDS identification card will be issued for each employee that has a Contract longer than 30 days. (NOTE: In accordance with AFI 31-101/Integrated Defense, a complete background check will be conducted on all of the employees submitted.
The 366 FW Commander has granted the 366 Security Forces Squadron the authority to deny entry of any person requesting access to Mountain Home AFB, based upon the results of the background check). Contractor passes will be issued for the Contract period not to exceed a period of six months. If access is required beyond six months, individuals will be subject to another background check and a subsequent pass issued.
When Contract dates are less than 30 days, a localized visitor pass will be issued.
Verification of Contractor employees will be accomplished by verifying the applicant's credentials (federal or state issued identification) against the approved
SFS Form 30.
Contractors will provide the Contracting Officer with a SFS Form 30 for each employee/subcontractor who will need unescorted base access in support of the
Contract for any length of time, at least three weeks in advance. Contractors who have not received confirmation that their SFS Form 30 has been approved, shall not attempt to access the installation unless under escort by the appropriate sponsor
(NOTE: If the SFS Form 30 has not been approved, the sponsor will have to escort the individual at all times while he/she is working on the installation; includes traveling in the same vehicle). Once the SFS Form 30 has been approved, Contractors may proceed to Building 6400, Visitor Control Center, with a copy of their approved SFS Form 30, valid federal or state issued identification card, proof of vehicle insurance and valid vehicle registration (if driving a vehicle onto the installation) to receive their DBIDS Identification Card.
It is the Contractor's/Subcontractor's responsibility to ensure all deliveries have the required information, to include the delivery destination and a point of contact with vouching authority for validation of delivery. It is the Contractor's/Subcontractor's responsibility to ensure Contracting Officer’s Representative is notified of all deliveries prior to arrival. Projects requiring multiple deliveries from the same supplier will be treated as a subcontractor and will meet the Form 30 submission requirements.

Secure areas require Contractor to be escorted. Contact Contracting Officer 14 days in advance of performing work in secure area.

PART 10
OTHER CONDITIONS AFFECTING WORK
10.1Work Coordination
10.1.1 Interfacing With Others
Plan work activities in such a manner that there will be minimal interference and inconvenience to ongoing traffic and daily operations in the facility and local areas.
Schedule all work and coordinate with the Government Project Management Team or their designee at all times.
Do not block traffic, roads, access routes, or exits without prior coordination with the
Contracting Officer or other authorized Government representative.
10.1.2 Pre-Construction Conference
Attend a pre-work meeting presided over by the Contracting Officer prior to commencing construction activities. Contact the Contracting Officer’s
Representative within five business days of issuance of the Notice to Proceed (NTP) to establish a date and time for the preconstruction and base orientation meeting. The orientation will cover all emergency, safety, environmental, and other procedures for working on Mountain Home AFB.
Submit a complete work schedule within seven days after NTP and update the schedule weekly throughout the performance period. The schedule shall be in sufficient detail to identify all aspects of the work including all significant project start, finish, and duration times.
10.3. Compliance with Rules, Regulations, and Statutes
All Contractor employees shall observe and comply with all applicable local, State, and Federal rules, regulations, and statutes including those concerning fire, safety, sanitation, security, vehicle safety, and hazardous material handling while performing this work. All work shall meet or exceed all applicable industry standards.
10.4 Firearms
Contractor personnel shall carry no weapons or firearms while performing work under this Contract.
10.5 Work Hours
The construction activity period shall be restricted to 7:30 a.m. to 4:30 p.m. daily, Monday through Friday, excluding Federal holidays. Work hours other than as specified above shall be coordinated with and approved by the Contracting Officer's
Representative.
10.5.1 Potable Water
The Government shall make available to the Contractor, from existing outlets and supplies, reasonable amounts of potable water without charge. Conserve potable water furnished. The Contractor, at their own expense, shall install and maintain necessary temporary connections and distribution lines and shall remove the
PWS – Landfill Fencing Installation Mt Home AFB connections and lines prior to final acceptance of construction. All connections must be coordinated through the MHAFB Civil Engineering office.
10.5.2 Electricity
Subject to available supply, the Government, without charge, shall make reasonable amounts of electric current available to the Contractor for performing work at the work area. Conserve electric power furnished. The Contractor at their own expense shall install and maintain necessary temporary connections and distribution lines and shall remove the connections and lines prior to final acceptance of construction.
10.6 Permits
Obtain and submit copies of ALL permits, unless otherwise specified. Each permit shall be obtained before starting the respective items of work. This includes, but is not limited to, permits necessary for decontaminating water and sewer systems, and digging permits.
Obtain a "Dig Permit" from the Civil Engineering (CE) Office prior to any ground disturbing activities. Once utilities have been marked as part of the dig permit process, document utility locations by means of photographs and annotated plan drawings, and maintain the markings for the duration of the project.
A minimum of two weeks prior to excavating, contact the local utility locating service to mark utilities. Verify existing utility locations indicated on Contract drawings within the area of work.
10.7 Safety
EM 385-1-1 USACE Safety and Health Requirements Manual (available online at http://publications.usace.army.mil/publications/eng-manuals/) shall be considered to be a part of this Contract and will be enforced as such. Prepare an accident prevention plan in accordance with EM 385-1-1.
10.8 Operation and Maintenance (O&M) Requirements
Operation and maintenance manuals as required by UFGS technical specification sections (Divisions 02 through 48), shall be provided by the Contractor, and edited as required. The intent of the O&M Manuals is to promote and maximize the efficiency, economy, safety, and effectiveness of the life cycle operation, maintenance, and repair of the facility.
10.10. Construction Facilities and Temporary Controls
1. A staging area shall be provided for the Contractor at the pre-construction conference.
2. The Contractor is responsible for security of his own property and security of government property when construction activities effect existing security measures.
3. Provide and maintain portable toilet facilities at the site for the use of Contractor and subcontractor personnel, as required by EM 385-1-1. The units shall be emptied and cleaned at least once a week, or more often if required by the Contracting Officer.
The doors to the unit shall be self-closing. Additionally, the toilet units shall be located out of public view as the site allows.
10.11.Access and General Protection/Security Policy and Procedures
10.11.1. Subcontractors and Personnel Contacts
Furnish a list of contact personnel of the Contractor and subcontractors including addresses and telephone numbers for use in the event of an emergency. As changes occur and additional information becomes available, correct and change the information contained in previous lists.
10.11.2. Work Areas
Confine operations to areas within Contract limits indicated. Portions of the site beyond areas in which construction operations are indicated are not to be disturbed.
10.11.3 Vehicle use
Strictly enforce the following:
1. Keep all vehicles, mechanized or motorized equipment locked at all times when parked and unattended on Base's premises.
2. Do not, under any circumstances, leave any vehicle unattended with motor or engine running, or with ignition key in place.
3. Parking: Permitted only in areas designated by Contracting Officer's
Representative.
4. All traffic control subject to Contracting Officer's Representative approval.
10.11.5. Government Operations
Perform the Work so as not to interfere with the Government's operations.
10.11.6 Employee Parking
Contractor employees shall park privately owned vehicles in an area designated by the Contracting Officer. The area will be within reasonable walking distance of the construction site. Contractor employee parking shall not interfere with existing and established parking requirements of the military installation.
