Monthly Site ID Report: June 2021 Page 1 of 5
Month: June 2021
This document lists the Environmental Events reported this month to Defense Logistics Agency (DLA) Installation Management Environmental Restoration Division (DM-ER) via the DLAEnvironmentalEvent@dla.mil inbox or the alternative methods outlined in the Site Identification (ID) of Environmental Liabilities (ELs) Standard Operating Procedures (SOP). The events identified in Sections 1 – 3 were logged into the Environmental Event Repositorya and assigned an identifier (aka Site ID). Because the following Environmental Events were recently identified, there is no EL determination associated with them, and the information included for each event represents only that provided up until June 30, 2021. Each event will be reviewed with the respective DLA point of contact (POC) during the next data gathering, updated to include any additional information provided after June 30, 2021, and assigned an EL category.

"Statement of Incident: An exterior, above-ground water reservoir, contaminated with AFFF, intercepted rain water during a recent rain event (09 JUN2021) resulting in overflow of the reservoir into an adjacent drainage ditch leading off-base.
Corrective Action Required/Taken: An emergency work order was submitted on 10 JUN 2021 to clean off the top of the tank of leaf, limb and tree debris (i.e.
potential cause of standing water), repair/seal the manhole and cap exterior overflow piping for AFFF reservoirs at ALL fuel tank farms on Okinawa.

"We have had a constant loss in pressure during our weekly static pressure tests and went to investigate the reason why. We were in contact with our LFM and he was aware of the issue. Upon investigation, we discovered that there was fuel in multiple isolation pits on the flightline. LFM blocked off a section of the hydrant system to investigate why there was fuel. We were able to suck them out (mostly consisting of a lot of water) but also able to recover most of the fuel that was in the pits. LFM is now in the process of determining where the leak is coming from now that the pits are cleaned out so he can see the issue and fix it. This was a slow leak that happened over time, it was not just a overnight leak/spill." (Incident 2014 USAF Petroleum Office Incident Report)

"On 28 May 2021, I was notified of a fuel release at approximately 1000. I was able to respond within 15 minutes and discovered that the gasket on the slug valve cover of issue line 2 filter separator was blown and actively leaking. I was able to defuel 18 gallons with an R-11 and 12 gallons via bucket from the F/S. I estimate 5-10 gallons made its way to our collection pond.
On 02 June 2021, we discovered active leaks coming from all three of our fillstand meter register’s and Cla-Vals (approximately 5 gal). The drain line on the slug valve cover of our alternate filter separator released approximately 1 gallon. Most of the pressure relief return lines are leaking at the gasket connection points. The filter separator cover opposite of the slug valve cover on issue line 3 was bulging and cracked. While shutting down the system for the day, we noticed that the pressure gauges were maxed out at over 300PSI. While attempting to relieve pressure in the line, the valve upstream of the basket strainer was operated which caused the gasket on the downstream pump to burst and soaked one airman from the waist down with JAA.
An emergency work order was submitted through the Huntsville contract and a repair team is scheduled to arrive at the end of our duty day 03 June

"Tanker Pump House Bldg 1211//Fuel was not recovered/clean up took places and all fuel and clean up materials were disposed of IAW local procedures. "At approximately 1400 hours on 8 June 2021, a 2F0X1 operator that was escorting a contractor into the PSI 9 Pump Room and observed fuel on the interior floor. The 2F0X1 operator immediately evacuated the area, then radioed the Fuel Service Center. The Fuel Service Center ran the applicable Emergency Action/Spill Response checklist. The fuel release originated at the issue line gasket. The system was not in use.
Approximately 200 gallons of JP8 was released and contained within the pump room eliminating environmental concerns. 195 gallons of JP8 was recovered using a pneumatic pump and bowser. The remaining 5 gallons of JP8 was soaked up using spill pads. The incident is still under investigation for root cause." (PSI 9 Incident Reporting Form)

"BLUF: CE contractors found a fuel leak during a dig yesterday near bldg.
1086 RAF Lakenheath UK. Environmental and WFSM were contacted, a leak was located from a pressure gauge connector and has since been isolated. ZERO mission impact.
Discussion: Last Thursday, 10 June, CE had contactors conducting a dig near bldg. 1086 and smelled fuel. On 15 June, they noticed liquid fuel coming from the ground as they continued their dig. They then terminated the dig and contacted environmental. They then contacted WFSM whom

Event Background Installation Facility contacted POL around 1700. The pipeline was isolated at Pit X (receipt pit) and 1913. As the investigation continued, they dug into the ground further, resulting in more pooling water/fuel. After this, they proceeded to Pit T. Once they opened Pit T, the pit was full of fuel. WFSM removed approximately 600 liters of fuel/water mixture from the pit. After the mixture was removed a noticeable fuel leak was discovered coming from a pressure gauge in the pit. (see pressure gauge in attachment). WFSM closed the valve on Pit T and are currently looking into repairing it.
Environment will conduct their investigation and proper authorities on base have been notified. We are unsure how much fuel was lost but will be reviewing past receipts to see what our losses look like." (Email from MSgt Cornwell, FOUO\\Pit T - Fuel Leak FYSA, 6/16/2021)

"Clean up in the containment pond complete. Clean up under and around the tank is ongoing, ETA completion 23 June 2021.
On Monday 21 June 2021 at approximately 0730 hrs., my FDSO arrived at his work location Building P-7529 Bulk Storage at Simmons Army Airfield. Upon walking into the area, he immediatly noticed the strong smell of fuel in the air. He walked around the area and found a large quantity of fuel around the tanks. He notice that Tank #3 had wet sides below the manhole cover and below the Hydrant pump area.
He walked back to the building and saw a large quantity of fuel in the containment pond. HE called me at 0748 hrs. I immediately drove down to the Bulk Storage area. By the time I had arrived at Bulk Storage he had looked at the ATG ad found that Tank #3 was overfilled. He immediately set the system up to transfer fuel out of Tank #3 to get it to a safe level...
...we noticed that other than approximately 20 gallons of fuel under the tanks, most of the fuel had flowed into the secondary containment pond. Because NO fuel had gone into the earth or storm drains, none of the organizations wrote up a report. They all stated that hte spill was contained and not reportable and they all departed.

My dayshift crew started the clean up process around the tanks. I contacted EHC Environmental from Red Springs, NC. They arrived within 2 hours an dcompletely cleaned out the containment pond of all fuel and fuel residue." (Louis Berger Services, spill docs, 6/21/2021)

"At approximately 1313 on 29 June 2021, Mr. Perry Osburn (Maytag Aircraft Corporation) was conducting a defuel of an aircraft at the 224MI aircraft ramp. Operator error. Operator changed the setting on the fuel truck from “Defuel mode” to “Refuel mode” when he completed defueling the first wing of the C-12 type aircraft, transitioning to the second wing of the C-12 type aircraft. This switching of modes put the fuel truck into to a pumping fuel action vice an evacuation of fuel action.
No fuel got into any of the storm water drains.
REMEDIAL ACTIONS: Fuel contained and was cleaned up with fuel absorbent pads at the 224 MI aircraft ramp. Absorbent pads were stored in the proper containment drum. The 224 MI aircraft ramp is dry and clear of any fuel residue." (DLA E P 40 Spill Report June 30 2021)
