MEMORANDUM FOR OPTIMA LAKE MANAGER

SUBJECT: Asbestos Sampling Results, Optima Lake Office Building

1. Bulk asbestos samples were collected, to determine if asbestos was present at Optima lake office building on 24 May 2017 by Michael Mader of the Safety and Occupational Health Office.

Samples were sent via Federal Express to QuanTEM Laboratories for analysis using standard chain of custody and sample handling protocols. Samples were analyzed using EPA 600/R-

93/116 method.

2. Sample results are summarized in the table below, detailed analytical results are included in

Enclosure 1.
