The classifications are listed in alphabetical order of ""identifiers"" that indicate whether the particular rate is a union rate (current union negotiated rate for local), a survey rate (weighted average rate) or a union average rate (weighted union average rate).

Union Rate Identifiers

A four letter classification abbreviation identifier enclosed in dotted lines beginning with characters other than ""SU"" or ""UAVG"" denotes that the union classification and rate were prevailing for that classification in the survey. Example:

PLUM0198-005 07/01/2014. PLUM is an abbreviation identifier of the union which prevailed in the survey for this classification, which in this example would be Plumbers. 0198 indicates the local union number or district council number where applicable, i.e., Plumbers Local 0198. The next number, 005 in the example, is an internal number used in processing the wage determination. 07/01/2014 is the effective date of the most current negotiated rate, which in this example is July 1, 2014.

Union prevailing wage rates are updated to reflect all rate changes in the collective bargaining agreement (CBA) governing this classification and rate.

Survey Rate Identifiers

Classifications listed under the ""SU"" identifier indicate that no one rate prevailed for this classification in the survey and the published rate is derived by computing a weighted average rate based on all the rates reported in the survey for that classification. As this weighted average rate includes all rates reported in the survey, it may include both union and non-union rates. Example: SULA2012-007 5/13/2014. SU indicates the rates are survey rates based on a weighted average calculation of rates and are not majority rates. LA indicates the State of Louisiana. 2012 is the year of survey on which these classifications and rates are based. The next number, 007 in the example, is an internal number used in producing the wage determination. 5/13/2014 indicates the survey completion date for the classifications and rates under that identifier.

Survey wage rates are not updated and remain in effect until a new survey is conducted.

1.2 DEFINITIONS

a. "Cementitious material" as used herein must include all portland cement, pozzolan, fly ash, and slag cement.

b. "Exposed to public view" means situated so that it can be seen from eye level from a public location after completion of the building. A public location is accessible to persons not responsible for operation or maintenance of the building.

c. "Chemical admixtures" are materials in the form of powder or fluids that are added to the concrete to give it certain characteristics not obtainable with plain concrete mixes.

d. "Supplementary cementing materials" (SCM) include coal fly ash, slag cement, natural or calcined pozzolans, and ultra-fine coal ash when used in such proportions to replace the portland cement that result in improvement to sustainability and durability and reduced cost.

e. "Design strength" (f'c) is the specified compressive strength of concrete at time(s) specified in this section to meet structural design criteria.

f. "Mass Concrete" is any concrete system that approaches a maximum temperature of 158 degrees F within the first 72 hours of placement.

In addition, it includes all concrete elements with a section thickness of 3 feet or more regardless of temperature.

g. "Mixture proportioning" is the process of designing concrete mixture proportions to enable it to meet the strength, service life and constructability requirements of the project while minimizing the initial and life-cycle cost.

h. "Mixture proportions" are the masses or volumes of individual ingredients used to make a unit measure (cubic meter or cubic yard) of concrete.

i. "Pozzolan" is a siliceous or siliceous and aluminous material, which in itself possesses little or no cementitious value but will, in finely divided form and in the presence of moisture, chemically react with calcium hydroxide at ordinary temperatures to form compounds possessing cementitious properties.

j. "Workability (or consistence)" is the ability of a fresh (plastic) concrete mix to fill the form/mould properly with the desired work (vibration) and without reducing the concrete's quality. Workability depends on water content, chemical admixtures, aggregate (shape and size distribution), cementitious content and age (level of hydration).

1.3 SUBMITTALS

Government approval is required for submittals with a "G" or "S" classification. Submittals not having a "G" or "S" classification are for Contractor Quality Control approval. 1.4 MODIFICATION OF REFERENCES

Accomplish work in accordance with ACI publications except as modified herein. Consider the advisory or recommended provisions to be mandatory.

Interpret reference to the "Building Official," the "Structural Engineer," and the "Architect/Engineer" to mean the Contracting Officer.

1.5 DELIVERY, STORAGE, AND HANDLING

Follow ACI 301 , ACI 304R and ASTM A934/A934M requirements and recommendations. Do not deliver concrete until vapor retarder, vapor barrier, forms, reinforcement, embedded items, and chamfer strips are in place and ready for concrete placement. Do not store concrete curing compounds or sealers with materials that have a high capacity to adsorb volatile organic compound (VOC) emissions. Do not store concrete curing compounds or sealers in occupied spaces.

1.5.1 Reinforcement

Store reinforcement of different sizes and shapes in separate piles or

SECTION 03 30 00 Page 9 racks raised above the ground to avoid excessive rusting. Protect from contaminants such as grease, oil, and dirt. Ensure bar sizes can be accurately identified after bundles are broken and tags removed.

1.6 PRE-INSTALLATION MEETINGS

Conduct a pre-installation meeting to verify project requirements, manufacturer's installation instructions and manufacturer's warranty requirements. Review the following:

a. Environmental Requirements

b. Scheduling and phasing of work.

c. Coordinating with other work and personnel. Remind all trades that they are working on a surface that is to become a finished surface.

d. Protection of adjacent surfaces.

e. Surface preparation.

f. Repair of defects and defective work prior to installation

g. Cleaning.

h. Installation of polished floor finishes.

i. Application of liquid hardener, densifier.

j. Protection of finished surfaces after installation.

k. Placing of materials on the concrete surface that may cause staining, etching or scratching

1.7 QUALITY ASSURANCE

1.7.1 Design Data

1.7.1.1 Concrete Mix Design

Sixty days minimum prior to concrete placement, submit a mix design for each strength and type of concrete. Submit a complete list of materials including type; brand; source and amount of cement, supplementary cementitious materials, and admixtures; and applicable reference specifications. Submit mill test and all other test for cement, supplementary cementitious materials, aggregates, and admixtures. Provide documentation of maximum nominal aggregate size, gradation analysis, percentage retained and passing sieve, and a graph of percentage retained verses sieve size. Provide mix proportion data using at least three different water-cementitious material ratios for each type of mixture, which produce a range of strength encompassing those required for each type of concrete required. If source material changes, resubmit mix proportion data using revised source material. Provide only materials that have been proven by trial mix studies to meet the requirements of this specification, unless otherwise approved in writing by the Contracting Officer. Indicate clearly in the submittal where each mix design is used when more than one mix design is submitted. Resubmit data on concrete components if the qualities or source of components changes.

For previously approved concrete mix designs used within the past twelve

SECTION 03 30 00 Page 10 months, the previous mix design may be re-submitted without further trial batch testing if accompanied by material test data conducted within the last six months. Obtain mix design approval from the contracting officer prior to concrete placement.

1.7.2 Installer Qualifications

Submit data for company, personnel, experience, and training to conform to

Unless otherwise specified. submit verification that a competent supervisor who 1s at the Project during the specified Work 1s certified as a Concrete Polishing Council (CPC) Craftsman, or equivalent.

Specify if more or fewer certified concrete polishers are required or permitted. Concrete polishers should be certified on the basis of work experience or successful completion of a certification program that includes written and performance examinations as described in ASCC CPC Certification Policies, February 19.

1.7.3 Finishing Components

Concrete finishing components and materials shall be from single manufacturer.

1.7.4 Manufacturer Qualifications

a. Manufacturer capable of providing field service representation during construction and approving application method.

b. Manufacturer shall have a minimum 5 years of experience in manufacturing components similar to or exceeding requirements of project.

1.7.5 Shop Drawings

1.7.5.1 Reinforcing Steel

Indicate bending diagrams, assembly diagrams, splicing and laps of bars, shapes, dimensions, and details of bar reinforcing, accessories, and concrete cover. Do not scale dimensions from structural drawings to determine lengths of reinforcing bars. Reproductions of contract drawings are unacceptable.

1.7.6 Control Submittals

1.7.6.1 Concrete Curing Plan

Submit proposed materials, methods and duration for curing concrete elements in accordance with ACI 308.1 and ACI 310.1 .

1.7.6.2 Pumping Concrete

Submit proposed materials and methods for pumping concrete. Submittal must include mix designs, pumping equipment including type of pump and size and material for pipe, and maximum length and height concrete is to be pumped.

1.7.6.3 Finishing Plan

Submit proposed material and procedures to be used in obtaining the polished concrete finish for the first, second, and third floors. Include qualification of person to be used for obtaining floor tolerance measurement, description of measuring equipment to be used, and a sketch showing lines and locations the measuring equipment will follow.

1.7.6.3.1 Polishing Schedule

Submit plan showing polished concrete slab finishes and schedule of polishing operations for each area of polished concrete before start of polishing operations. Include locations of all joints, including construction joints. Schedule to include details of sequencing Work with respect to coordination of other trades interior construction. final weather protection, and when protection is necessary, if other construction activities could damage the in-place polished Work.

1.7.6.3.2 Repair Data

Submit manufacturer's technical literature for each repair product specified or proposed. Include manufacturer's technical data, application instructions, and recommendations.

1.7.6.3.4 Crack Repairs

Unless otherwise specified, submit plan locating and identifying concrete cracks. Reference crack locations to building column lines.

Consider eliminating this provision if there are no cracks, if the cracks are not detrimental to polishing, or if the cracks will not be repaired.

1.7.6.4 VOC Content for form release agents, curing compounds, and concrete penetrating sealers

Submit certification for the form release agent, curing compounds, and concrete penetrating sealers that indicate the VOC content of each product.

1.7.6.5 Safety Data Sheets

Submit Safety Data Sheets (SDS) for all materials that are regulated for hazardous health effects. SDS must be readily accessible during each work shift to employees when they are at the construction site.

1.7.7 Test Reports

1.7.7.1 Fly Ash and Pozzolan

Submit test results in accordance with ASTM C618 for fly ash and pozzolan. Submit test results performed within 6 months of submittal date.

1.7.7.2 Slag Cement

Submit test results in accordance with ASTM C989/C989M for slag cement.

Submit test results performed within 6 months of submittal date.

1.7.7.3 Aggregates

Submit test results in accordance with ASTM C33/C33M, and ASTM C1293 or ASTM C1567 as required in the paragraph titled ALKALI-AGGREGATE REACTION.

1.7.8 Field Samples

1.7.8.1 Slab Finish Sample (Mock-Up)

Install minimum of 10 foot by 10 foot slab. Slab finish sample must not be part of the final project. Finish as required by specification.

Field mock-up-Before performing Work of this Section, provide field mock-up to verify selections made under submittals and to demonstrate aesthetic effects of polishing in accordance with 1.7.8.1.1 through

1.7.8.1.7. Approval does not constitute approval of deviations from Contract Documents unless Architect/Engineer specifically approves deviations in writing.

1.7.8.1.1 Procedures and Material

Unless otherwise specified, construct field mock-ups using same procedures, equipment, and materials that will be used for production of polished concrete. Accepted field mock-up will serve as a reference to which polished concrete will be compared for periodic and final acceptance. Construct field mock-up at an acceptable location on site.

Provide a simulated repair to demonstrate acceptable procedures. Repair procedures will provide and acceptable color and texture match. Protect from physical damage and retain mock-ups until final acceptance of polished concrete.

1.7.8.1.2 Minimum Cure Time of mock-up

Concrete in mock-up area to have a minimum cure of 28 days

1.7.8.1.3 Mock-up Hardness Testing

Prior to starting mock-up installation, verify surface hardness with a Mohs Hardness testing kit.

1.7.8.1.4 Slab Edge and Joints

Mock-up shall include slab edge polishing work. Mock-up area shall also include complete joint filling and shall demonstrate the polishing processing over one or more floor joints.

1.7.8.1.5 Abrasives

Provide preconditioned (previously used) bonded abrasives, having a complete range of tooling available for a variety of concrete surface hardnesses.

1.7.8.1.6 Mock-up Approval

The mock-up shall be reviewed and approved by the Contracting Officer, and shall become the minimum standard for acceptance of the polish work.

1.7.8.1.7 Start Concrete Polishing Work

Concrete polishing work shall not begin until mock-up has been accepted by Contracting Officer; in writing.

1.7.8.1.8 Mock-up Acceptance as a Standard

When accepted, mock-up will demonstrate minimum standard of quality required for this work.
