B. TANK INSPECTION COMMENTS

1. TANK CONSTRUCTION

Tank 1 is a welded, steel, vertical AST. The tank was constructed in 1995 to API 650 9th Edition and repaired/altered in 2012 to API 653 4th Edition by CAPE. The tank is 27’-0” in diameter and 28’-3 ½” in height. According AmSpec strapping charts from February 2012, Tank 1 has a capacity of 2,558 BBL

(107,424 gallons). Current strapping charts provide fuel level in increments of 1/16-inch and in units of gallons.

2. TANK FOUNDATION

Tank 1 rests on a concrete ringwall foundation with four (4) perimeter leak detection ports. A settlement evaluation was performed during this inspection. No signs of out-of-plane settlement were noted.

See Appendix A for settlement results. The tank is provided eight (8) shell attached seismic anchors around the perimeter of the tank. One (1) seismic anchor bolt exhibits signs of deformation and should be monitored during future tank inspections.

3. CATHODIC PROTECTION

The tank is equipped with an ICCP system for protecting the tank bottom from corrosion. According to the previous cathodic protection (CP) report, dated September 2019. Tank 1 is receiving adequate CP as all potential measurements were greater than the -850 millivolts required by NACE International (NACE) standards.

4. TANK SHELL

The tank shell is equipped with four (4) butt-welded carbon steel courses. Shell plates were examined using ultrasonic thickness testing (UTT) and visual testing (VT) methods. Nine (9) UTT measurements were taken on each plate of the 1st shell course. Nine (9) UTT measurements were taken on the remaining shell course plates using the spiral staircase for access. UTT measurements indicated no significant signs of metal loss on shell plates.

Refer to Drawing A-1, “Shell Layout” in Appendix A for shell layout.

Tank 1 is equipped with twenty (20) nozzle penetrations: Three (3) 36-inch manways, one (1) 8-inch receipt, one (1) ¾-inch water draw-off, one (1) 4-inch low suction, one (1) 8-inch main suction, five (5)

1-inch threaded plugs, two (2) 1-inch penetrations for the low level alarm (LLA), two (2) 1-inch penetrations for the high level alarms (HLA), two (2) 1-inch penetrations for the high-high level alarm

(HHLA), and two (2) 1-inch penetrations for the high level control valve (HLCV) float pilot. Tell-tale holes at shell nozzle reinforcement pads are open to the atmosphere and not provided a heavy lithium or white grease. The grease provided should be dissolvable when in contact with jet fuel to allow for adequate leak detection at interstitial nozzle welds. Hinges installed at three (3) shell manway covers are not provided davit arm attachments with grease fittings. Davit arm attachments allows for efficient operation of manway covers while opening and closing.

Conduit supports provided for the HLCV float pilot, level alarms, and ATG are not fully seal welded to the tank shell, allowing a crevice for water ingress at the support-to-shell interface. Interfaces exhibit signs of crevice corrosion with corrosion staining. Spacing between vertical shell plate butt-welds and two (2) conduit supports attached adjacent to manway 1 along with two (2) LLA penetrations do not meet the 6” minimum spacing requirement per API. Adequate weld spacing in the heat affected zone is a critical factor in limiting stress risers in the tank shell. A bird’s nest was observed inside one (1) shell overflow vent hood adjacent to the spiral staircase upper platform. The current position and orientation of vent screens allows for animal infestation underneath the vent hoods. To prevent further infestation, provide

1/2-inch stainless steel wire bird screens at four (4) vent openings that are oriented perpendicular to the tank shell.

5. FIXED DOME ROOF

Tank 1 is equipped with a fixed dome roof with welded steel plates. The fixed dome roof was examined using VT methods. The fixed dome roof is provided a non-slip coating with a rough surface profile, that prevented the acquisition of UTT measurements. Two (2) conduit supports attached at the roof are no longer in use and exhibit signs with of crevice corrosion at the support-to-roof interface.

The fixed dome roof is provided with fourteen (14) nozzle penetrations: One (1) 10-inch Enraf automatic tank gauge (ATG), one (1) 36-inch manway with 8-inch manual gauging hatch, two (2) 3-inch anti-rotation cables, one (1) 72-inch x 24-inch internal floating roof (IFR) installation hatch, one (1) 6-inch

Vito temperature/water combination probe, one (1) 10-inch sampling hatch, one (1) 8-inch blind flange, two (2) 3-inch grounding cable connection, two (2) 1 ½-inch tape level gauge (TLG) guide wires, one (1)

2-inch Varec TLG, and one (1) 24-inch manway with 8-inch center vent.

Refer to Drawing A-2, “Fixed Roof Layout” in Appendix A for fixed roof layout.

6. TANK APPURTENANCES

a. INTERNAL FLOATING ROOF

Tank 1 is equipped with an aluminum honeycomb IFR with double wiper peripheral seals.

Accessible portions of the IFR were inspected using VT methods. No signs of damage or compromise were noted during this inspection.

b. GAUGING

The tank is provided with an 8-inch gauging hatch for manual gauging, a 10-inch Enraf ATG system for automatic tank gauging, and 2-inch Varec TLG for monitoring product level from the ground. Both the manual gauging hatch and ATG are provided with fully slotted stilling wells. Hardware provided for the ATG and Vito temperature/water combination probe exhibits signs of corrosion.

c. LEVEL ALARMS

The tank is provided with independent, shell-mounted alarms in the low, high, and high-high positions. Tank 1 is not provided an independent, shell-mounted level alarm in the low-low position. Existing level alarms were functioning properly at the time of this inspection.

d. STAIRS AND PLATFORMS

The tank is provided with a spiral staircase with one (1) work platform for access to the IFR manway, and one (1) upper platform for fixed roof access. The HHLA/HLA/HLCV piping and the IFR access manway cover extend into the pathway of the spiral staircase. Spacing between these shell appurtenances and the spiral staircase handrail is approximately 20-inches. The landing distance from the ground to the first step is 22-inches. The Occupational Health and

Safety Administration (OSHA) mandates that staircases have a minimum clearance of 26-inches between appurtenances and handrails and a maximum landing distance of 9 ½-inches.

Wasp nests were observed on the underside of the spiral staircase work platform. This situation may create hazards for facility personnel.

e. GROUNDING AND BONDING

The tank shell is equipped with four (4) grounding lugs and cables that are in good overall condition with no signs of fraying or disbondment.

7. TANK AND PIPING COATING

The tank is assumed to be provided with an epoxy/polyurethane coating system. External tank shell and fixed roof coatings exhibit chalking, indicating the polyurethane topcoat is reaching the end of its life cycle. It is recommended to conduct a coating condition survey to determine if the entire coating system should be replaced, or if only a topcoat application is required. External coatings at one (1) pipe support and one (1) thermal relief valve (TRV) assembly for each of the main and low suction, along with two (2) pipe supports for the product recovery tank (PRT) drain line exhibit cracking and base metal exposure with surface corrosion.

The PRT coating exhibits flaking, peeling and disbondment; however, the PRT is not of carbon steel construction, therefore, an external coating system is not required. Various areas of bio-growth were identified on the tank shell and fixed roof plates, containment piping, spiral staircase, and shell and fixed roof nozzles and appurtenances. The spiral staircase and fixed roof perimeter handrail coatings exhibit cracking, flaking, and disbondment with base metal exposure. Fixed roof nozzle coatings at the IFR installation hatch, center vent, Vito probe flange, and blind flange exhibit cracking, chalking, and base metal exposure with surface corrosion. The tank is not provided NFPA emergency responder labeling.

Emergency responder labeling will aid emergency personnel to quickly identify hazard ratings for health, flammability, and instability.

8. TANK PIPING SYSTEM

Two (2) pipe supports provided for the PRT drain line are not properly anchored to the containment dike floor, allowing for excessive pipe movement during normal operations. The PRT drain line connecting flange is missing one fastener. Improper tightening of hardware may create a leak path at pipe flange connections. The PRT recirculation line is not properly supported at the containment dike wall. The pipe currently rests on the containment subdivision wall, which may result in contact point corrosion at the concrete-to-pipe interface.

The receipt and main suction TRV crossover piping are provided with sight flow indicators. Sight flow indicators are no longer used in this application as they are potential leak paths. The main suction piping is provided two (2) roller supports. Roller supports are no longer used in this application as they create excessive stress at the contact point. No contact point corrosion was observed during this inspection.

9. FIRE PROTECTION SYSTEM

For internal fire protection, Tank 1 is provided with an aluminum honeycomb IFR with double wiper peripheral seals.

10. SECONDARY CONTAINMENT

Tank 1 and Tank 2 are situated in a shared concrete containment dike with vertical walls. Based on dimensions, the containment dike can hold up to 133% of the nominal capacity of Tank 1 in the event of a spill, which results in a calculated freeboard height of approximately 1’-1 ¼” above the entire containment area. A flexible membrane liner (FML) is installed beneath the concrete containment floor.

Vegetation growth was observed within the secondary containment at locations of damaged sealant and at the drain opening. The secondary containment is equipped with one (1) drain. The drain is provided a valve normally in the closed position. The drain is gravity fed to an oil-water separator when the valve is open to drain the secondary containment. The steel cover provided at the drain opening is 6-inches below grade. This configuration may create trip hazards for facility personnel, lead to excessive debris build-up, and vegetation growth. According to facility personnel, the secondary containment drainage system is fully functional.

The dike access stairs are not in compliance with OSHA requirements, as each step has a tread depth of

4-inches, riser height of 11-inches, and the angle between the stair and containment floor is greater than

60°. OSHA requires a minimum tread depth of 9 ½-inches, maximum riser height of 9 ½-inches, and an angle of elevation between 30 to 50 degrees from the horizontal. This configuration is a potential fall hazard due to the steep elevation and improperly sized treads.

The link seal provided at the containment wall penetration for the receipt piping exhibits signs of damage with tearing and general deterioration. The containment wall penetration for the PRT drain line is not provided isolation between the piping and concrete. Dike crossover stairs are not provided for egress/ingress between the containment areas for Tank 1 and Tank 2. The containment dike concrete expansion joints exhibit signs of sealant damage with voids and disbondment.

RECOMMENDATION: Replace twenty-eight (28) corroded fasteners at flanges for the Enraf ATG and

Vito probe, and provide insulating flange kits to isolate hardware from flanges to prevent galvanic corrosion.

x. Shell Attachments

OBSERVATION: Tank 1 is not provided labeling in accordance with NFPA 704.

REFERENCE: UFC 3-460-01, Section 2-17

RECOMMENDATION: Provide NFPA responder labeling to aid in identifying hazard ratings for stored product.

3. RECOMMENDED REPAIRS AS PART OF REGULAR MAINTENANCE

Regular maintenance repairs that relate to repairs/upgrades of an asset to bring it up to the current applicable standard from the standard at time of construction. These repairs represent items that should be addressed in a timely manner to prevent future potential risks to system operators, equipment integrity, or the adjacent environment.

y. Overall Tank Coating

OBSERVATION: Various areas of bio-growth were identified on the shell and fixed roof plates, piping, spiral staircase, and shell and fixed nozzles and appurtenances.

REFERENCE: BMP

RECOMMENDATION: Pressure wash tank to remove bio-growth. Bio-growth can potentially accelerate coating deterioration and corrosion. After bio-growth removal perform a coating condition survey to determine if the entire coating system should be replaced or if only a topcoat application is required.

z. Shell Reinforcement Pad

OBSERVATION: Tell-tale holes at shell nozzle reinforcement pads are open to the atmosphere.

REFERENCE: BMP

RECOMMENDATION: Provide a heavy lithium or white grease at tell-tale holes for seven (7) nozzle reinforcement pads to prevent water ingress into the interstitial space. Grease should dissolve in the presence of jet fuel to allow for adequate leak detection at nozzle welds.

aa. Containment Drain

OBSERVATION: Steel cover provided for the containment drain is currently 6-inches below grade.

This configuration may create trip hazards for facility operators and lead to excessive debris build-up.

REFERENCE: BMP

RECOMMENDATION: Rework containment drain cover to be flush with the surrounding containment grade.

bb. Containment Drain

OBSERVATION: Vegetation growth was observed at the dike drain and at various control joints.

REFERENCE: UFC 3-460-03, Section 8-10.2.1. A

RECOMMENDATION: Remove vegetation from dike drain to prevent blockage and debris build-up and at various control joints and provide sealant repair as necessary.

cc. Spiral Staircase

OBSERVATION: Wasp nests were observed at stair platform adjacent to the IFR access manway.

RECOMMENDATION: Remove wasp nests to prevent injury to facility personnel.

dd. Tank Overflow Vent

OBSERVATION: A bird’s nest was observed inside one (1) overflow vent hood adjacent to the spiral staircase. Bird screens provided at four (4) overflow vent openings are oriented parallel to the tank shell allowing an opening for animal infestation underneath the vent hood.

REFERENCE: AW 78-24-37, Sheet D.07 – Detail 6

RECOMMENDATION: Remove birds’ nest from inside one (1) overflow vent hood. Additionally, rework the existing vent screens or install new ½-inch stainless steel wire bird screens at four (4) overflow vent hood openings. Wire screens should be oriented perpendicular to the tank shell and vent hood to limit further animal infestation.

ee. Fixed Roof Conduit Supports

OBSERVATION: Tank roof is provided two (2) conduit supports that are no longer in use. Crevice corrosion staining was present at the roof-to-support interface.

RECOMMENDATION: Remove two (2) unused conduit supports on the fixed roof.

The interval between tank inspections (both internal and external) should be determined by the tank service history.

Inspection intervals can be adjusted based on known corrosion rates. Corrosion rates can only be determined when a comparison is made between UTT measurements taken over an interval of time. In this case, corrosion rates were determined using assumed nominal thicknesses and UTT measurements taken during this inspection. Listed below are recommended inspection intervals:

1. Routine In-service Inspection

Facility personnel shall perform a visual inspection of the tank exterior surfaces and associated piping. Gross defects, leaks, shell distortions, signs of settlement, and corrosion should all be monitored along with the foundation condition (including tank chime-to-foundation joint), coating, and appurtenances.

a. Interval: Monthly

2. External Visual Inspection

Formal visual external inspection shall be performed by an API certified inspector every five (5) years or RCA/4N years (where RCA is the difference between the measured shell thickness and the minimum required thickness and

N is the shell corrosion rate), whichever is less. Based on the review of shell thickness measurements, the 5-year interval will govern the next external visual inspection. This inspection includes inspection requirements listed in a routine in-service inspection as well as a visual inspection of the tank grounding system.

b. Interval: Five (5) years; December 2024

3. External Visual Inspection with Ultrasonic Thickness Testing

An API certified inspector with the assistance of an NDT examiner shall perform this inspection. UTT measurements shall be taken to verify thickness of the tank shell and associated components. Obtained UTT measurements are used to determine corrosion rates and remaining life of the tank shell. UTT measurements of shell plates can be performed with the tank in service and scheduled at a maximum interval of fifteen (15) years or RCA/2N, whichever is less.

Based on shell thickness measurements and corrosion rates, the recommended interval for this inspection is fifteen

(15) years.

c. Interval: Fifteen (15) years; December 2034

4. Internal Inspection

An API certified inspector with the assistance of an NDT examiner shall perform this inspection. This inspection consists of verifying that the tank bottom is not severely corroded and/or leaking, identifying and evaluation tank bottom settlement, and inspection requirements listed in both external visual inspections. Based on the internal

(out-of-service) API 653 preliminary inspection report dated February 2015, the next internal inspection should be performed within ten (10) years.

d. Interval: Ten (10) years; April 2025 (UFC)

Ten (20) years; February 2035 (API)

APPENDIX B: API 653 CHECKLISTS FOR TANK INSPECTIONS

Tank In-Service Inspection Checklist

Item Completed

X

C.1.1 Foundation

Measure foundation levelness and bottom elevations (see Appendix A for extent of measurements). X

C.1.1.1 Concrete Ring a Inspect for broken concrete, spalling, and cracks, particularly under backup bars used in welding butt-welded annular rings under the shell.

X b Inspect drain openings in ring, back of water draw basins and top surface of ring for indications of bottom leakage.

X c Inspect for cavities under foundation and vegetation against bottom of tank. X d Check that runoff rainwater from the shell drains away from tank. X e Check for settlement around perimeter of tank. X

C.1.1.2 Asphalt X a Check for settling of tank into asphalt base which would direct runoff rain water under the tank instead of away from it.

N/A b Look for areas where leaching of oil has left rock filler exposed, which indicates hydrocarbon leakage. N/A

C.1.1.3 Oiled Dirt or Sand

Check for settlement into the base which would direct runoff rain water under the tank rather than away from it.

N/A

C.1.1.4 Rock a Presence of crushed rock under the steel bottom usually results in severe underside corrosion.
