About this file

This document contains a past performance questionnaire template and details of a federal contract opportunity for emergency temporary roofing repairs. The past performance questionnaire is a standard template used by the Navy Facilities Engineering Command and U.S. Army Corps of Engineers to collect feedback from clients on a contractor's past work quality, schedule, management and other performance factors.

The federal contract opportunity is a solicitation by the U.S. Army Corps of Engineers to award four single-award task order contracts for emergency temporary roofing repairs in 20 states divided into two regions. The contracts have a total maximum value of $45 million each and a seven-year period of performance including a five-year base and two-year option. The solicitation will result in awards to one small business, one service-disabled veteran-owned small business, one HUBZone small business, and one woman-owned small business. The primary NAICS code is 238160 for roofing contractors and responses are due by contacting the named contracting specialist by email.
