ATTACHMENT 2

PERFORMANCE REQUIREMENTS SUMMARY

The contractor service requirements are summarized into performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success.

Performance Objective (The Service required—usually a shall statement)

Standard Performance Threshold (This is the maximum error rate. It could possibly be “Zero deviation from standard”)

Method of Surveillance

PRS # 1. The contractor shall perform pumping and wash down operations at vault toilets. (Service Calls)

Work will be performed according to section 5.1 of the PWS.

Zero deviation from standard. Payment will not be made for the specific service call if there is any deviation from the standard.

Periodic Surveillance

PRS # 2 The contractor shall perform pumping and cleaning operations at all other tanks. (Service Calls)

Work will be performed according to section 5.2 of the PWS.

Zero deviation from standard. Payment will not be made for the specific service call if there is any deviation from the standard.

PRS # 3 The contractor shall perform disposal of sewage and the required record keeping and documentation.

Disposal and documentation must be performed according to section 5.3 of the

PWS.

Zero deviation from standard. Payment for pump outs will not be made unless the disposal is done correctly and all documentation is provided with the invoices.

PRS # 4 The contractor shall perform all work according to the response time set forth in the contract.

All response times will meet specifications outlined in section 5.3.6 of the PWS.

Zero deviation from standard. Payment will not be made for pump outs or service calls if the response time is not met.

PRS # 5 The contractor shall take, route, and administer all samples as required by the contract.

All samples will be handled according to local, state, and federal regulations.

Zero deviation from standard. Payment will not be made for mishandled samples.

All inspections will be performed and administered to ensure the safe and efficient operation of the systems.
