About this file
The document is a Quality Assurance Surveillance Plan (QASP) for a Trout Production Contract. The QASP outlines the criteria for evaluating the contractor's performance in meeting the requirements of the Performance Work Statement (PWS), including the production and release of specified hatchery trout, adipose fin clipping, and fish health monitoring. The COR or Quality Assurance Evaluator will conduct periodic inspections and review the contractor's quality control records to ensure compliance with the performance standards. Deficiencies exceeding the allowed number will result in deductions. The QASP is referenced in the associated federal contract opportunity for the Willamette River Basin Trout Production IDIQ, which is a full and open competition under NAICS code 112511 for up to 277,000 pounds of hatchery rainbow trout to be released in the Willamette Valley River Basin.

Quality Assurance Surveillance Plan (QASP) for Trout Production Contract
This Quality Assurance Surveillance Plan (QASP) has been developed to evaluate Contractor actions while implementing the performance requirements in the Performance Work Statement (PWS). It is designed to provide an effective surveillance method for monitoring contractor performance for each listed objective on the PWS in the contract.
The QASP provides a systematic method to evaluate contractor requirements. This QASP is cited on the premise the Government desires to maintain a quality standard in achieving contract objectives.
The Contractor, and not the Government, is responsible for management and quality control actions to meet the terms of the contract. The role of the Government is quality assurance to ensure contract standards are achieved.
In this contract the quality control program is the driver for product quality. The Contractor is required to develop a quality control plan. The first major step to ensuring a “self-correcting” contract is to ensure that the quality control plan developed at the beginning of the contract provides measures needed to lead the Contractor to success.
Once the quality control program is approved, careful application of the process and standards presented in the remainder of this document will ensure a robust quality assurance program.
The Quality Control Plan must minimally include:
1. A description of the inspection system for aspects listed on the PWS; must specify a monitoring strategy and include frequency and location of inspections, and the titles and organizational placement of the inspectors. The organizational list should also include an email address and phone numbers at which the individuals may be reached.
2. A description of the methods to be used for identifying and preventing defects in the quality of service performed.
3. A description of the records to be maintained to document inspections and corrective or preventative actions taken.
4. Records of Quality Control inspections must be kept and made available to the COR, when required by schedule or request, throughout the contract performance period and for the period after contract completion until final settlement of any claims under this contract.
5. Failure of the Contractor to pass the surveillance standards or documented complaints in excess of the maximum allowed shall result in a deduction equal to the allowed number of deficiencies for each validated complaint which exceeds the total acceptable number of customer complaints for the affected area or areas.
Performance Requirements Summary (PRS)
These are the criteria against which the performance will be evaluated. Work shall be considered not to have been performed when any one of the following conditions exists in accordance to the Quality Assurance Surveillance Plan.
