About this file
This document is a Standing Safety Procedure (SAF) for Hazardous Energy Control (HEC) at The Dalles Lock and Dam, which supplements the Portland District's SAF 385-1 HEC program. The SAF lists the personnel authorized to perform HEC duties, equipment that can be removed from service without a clearance, procedures for contractors and non-project personnel, requirements for personal protective grounds, and records and measurements.
The related federal contract opportunity is a solicitation for Elevator Maintenance Services at The Dalles Lock and Dam, issued as a Request for Quote (RFQ) under NAICS code 811310. The government is requesting quotes for the services outlined in the attached Performance Work Statement. The solicitation is advertised as unrestricted, and all prospective offerors must be registered in SAM to be eligible for award.

Foreword
This Standing Safety Procedure (SAF), Hazardous Energy Control (HEC), is supplemental to, and is authorized by ER 385-1-31, The Control of Hazardous Energy. ER 385-1-31 is the overall governing document for USACE HEC programs and it mandates that Districts will supplement its contents with specific procedures. This procedure is a supplement to Portland Districts SAF 385-1. This SAF is not intended to be the all-encompassing single source document for the HEC Program. This document strives to capture those processes which have been identified to establish safe hazardous energy control at The Dalles Lock and Dam.
Instances not specifically identified in this document may very well require additional research into those documents.

5.0 Authorities
5.1 The Chief Power Plant Operator on duty is the Issuing Individual (II) and has authority over all project equipment in operation.
5.2 All Safe Clearances, or any maintenance work not requiring a Safe Clearance, which would affect powerhouse operations, shall be authorized by the Issuing Individual. The II may delegate the clerical work and labor of the Safe Clearance process. Delegation, however, shall not relieve the Issuing Individual of any responsibility to be fully informed at all times of the current status of the Project, and for the safety implied by the conditions established by the Safe Clearances or personal lockout/tagout.
5.3 The PAI has full authority over the facility or equipment which has been cleared, from the time he or she accepts the Safe Clearance, until he or she releases the Safe Clearance.
5.4 The PAI is fully responsible for the equipment covered by the Safe Clearance until Safe Clearance is released to the Issuing Individual.
5.5 The PAI’s supervisor is responsible for assigning work within the technical capability of the PAI. Authorized Individuals are required to have a knowledge of the type, magnitude, and hazards of the energy to be controlled, and the limits of the controlled area.
6.0 Clearance Submission and Review
6.1 Clearances requests with more then 4 (four) cards, shall be submitted at least twenty four hours in advance.
6.2 Generator Outage requests require notice 24 days in advance of the week the outage will occur in.
6.3 Line Outage requests require notice 75 days in advance of the month the outage will occur in.

8.0 Additional Items Concerning Contactors and or Non-Project Personnel
8.1 Only qualified personnel are authorized as Issuing Individuals (II), Designated Representatives to Issuing Individuals (DRII), Principal Authorized Individuals (PAI), and Authorized Individuals (AI).
8.2 Portland Districts SAF 385-1 shall be used for the isolation of hazardous energy
8.3 Contractor's Hazardous Energy Control Procedures may be applied to equipment which has not been connected to Corps property. This may include temporary, newly installed, or existing permanent energy sources of any kind (including, but not limited to: electrical, mechanical, pneumatic, hydraulic, or chemical).
8.4 The contractor's Hazardous Energy Control Procedures shall not inhibit or interfere with the project's Hazardous Energy Control Procedures.
8.5 The Contractor shall be responsible for their own safe clearance procedure while performing work that requires the clearance points specified within the Contractor Controlled Work Zones defined in the section below. The clearance points specified are available for the Contractor to interface with or use as clearance points when needed for tests, inspections, or preventative maintenance (refer to SAF 385-1 Portland District HECP policy section 20.0 Non-Standard Maintenance). The Contractor is required to inform The Dalles Lock & Dam Control Room prior to any interfacing with the clearance points. Clearance points desired or required outside of those specified will require informing The Dalles Lock & Dam Control Room and a formal clearance be established for the Contractor. Contractor controlled work zones are defined as follows: elevator mechanical rooms, elevator shafts/pits, and entry/exit ways at each stop. The elevator disconnect points are listed below.
Clearance points/Disconnects components for Elevator HECP work are:
• Bay 15 elevator mechanical room electrical panel (CQ31) and downstream components.

Personnel will be authorized at their appropriate level for switching base on the concurrence of the RO. This authorization is the level of practice for the position. Based on the authorization level, the appropriate PPE will need to be used to perform said function. As an example, a journeyman electrician is issued a 40 cal/cm2 arc flash PPE and is authorized to interface with equipment at 40 cal/cm2 and lower. All personnel are authorized to interface with arc flash equipment that requires PPE at 1.2cal/cm2.

9.5.1 Arc Flash PPE for Personal Lockouts
1. Arc flash training is intended to provide all personnel the training to understand arc flash and its associated dangers. Personnel that have received arc flash training, do not have the ability to use personal lockouts on all equipment.
Personnel are required to be authorized/designated to perform work on equipment using personal lockouts. Based on the authorization level, the appropriate PPE will need to be used to perform said function. All personnel are authorized to interface with arc flash equipment that requires PPE at 1.2cal/cm2.
9.6 Equipment for which a Personal Lockout may NOT be used:
9.6.1 Any equipment which adversely affects the operation of the main units, fish units, station service units, navlocks, and fishways requires a clearance.
9.6.2 DC Emergency Lighting
10.0 Personal Protective Grounds
10.1 PAIs shall be responsible for notification of grounding locations to the Issuing Individual for tagging purposes. The II or DRII will accompany the PAI during placement and removal of grounds to facilitate tagging.
10.2 OSHA regulations with regards to development of Hazardous Energy Control, De- energizing and Temporary Protective Grounding procedures vary depending on whether equipment is regarded as "Generation" or "Transmission.
For the powerhouse, the dividing point between "Generation" and "Transmission & Distribution" is the load side of the ZMT/ZWT disconnect. Therefore generation equipment in the powerhouse falls under 1910.269(d). Equipment on the Transmission side falls under 1910.269 (m). On the generation side, alternatives to grounding are employed where equipment is isolated and there are no induced voltages or other hazards. On the transmission side, full fault rated Temporary Protective Grounds are required.
