About this file
This file is a template for presenting previous contract experience as part of a federal proposal response. The template requires offerors to document contract information and schedule details including the contract/task order number, contract role (prime or subcontractor), percentage of work performed, Unique Entity Identifier (UEI), contract type (FFP, CPFF, CPAF, CPIF, etc.), award date, initial contract amount, original completion date, final contract amount, actual completion date, and amount added through modifications. Offerors must also explain any cost growth and late finishes or time extensions, indicate whether early termination or cure/show-cause letters were issued, describe insurance arrangements to cap costs or indemnify, and identify whether multiple interim schedule milestones existed.
The template further requires documentation of program accomplishments presented as bullet points that denote project delivery team personnel and their current association with the offeror. Contact information must be provided for both the client contact and client contracting officer, as well as the offeror's point of contact. Additionally, offerors must identify whether a CPARS (Contractor Performance Assessment Reporting System) rating is available for the project and provide the date of the last CPARS rating. This is a standard format template used in federal proposal submissions to evaluate an offeror's relevant past performance and contract execution history.
