1. Will there be a site visit?
Yes, on Wednesday 5 June 2019 @ 0930
2. The attics. Do the attics need to be totally remediated of mold, or are they referring to the ductwork in the attic?
We would like to hear proposal on how we can treat the attic space; with the insulation. Yes it includes cleaning the all ductwork.
3. What is required besides a price for proposal submission?
Require a proposal of how the contractor plans to clean space
4. The Industrial Hygienist who conducted the test should have submitted a remediation protocol. If possible, I'd like to have a copy of the protocol prior to the site visit.
We did not get a confirmed recommendation for how to treat. But we had a site visit by Servpro how listed ways to treat.
MSgt Scott, COR, completed a course of actions (COA) for phases on how to start to mediate issue and prevent future problems. Please see attached.
-The COA were approved by 125FW Industrial Hygienist -The Industrial Hygienist will also be present at the site visit, Wednesday, 5 June to answer questions.
