Past Performance Questionnaire
Include 3 completed past performance questionnaires with your quote.
The Florida Air National Guard will be evaluating the past performance of the offeror(s) as it relates to the probability of successful accomplishment of the work required by the solicitation.
The following standards shall be used in arriving at the rating.
Very Low Risk (Exceptional)- Offer’s past performance record provides essentially no doubt that the offeror will successfully perform the required effort.
Low Risk (Very Good)- Offer’s past performance record provides little doubt that the offeror will successfully perform the required effort.
Moderate Risk (Satisfactory)- Offer’s past performance record, although satisfactory, provides some doubt that the offeror will successfully perform the required effort.
High Risk (Unsatisfactory)- Offer’s past performance record provides substantial doubt that the offeror will successfully perform the required effort.
Neutral/Unknown Risk- The offer has no relevant past performance record. A through search was unable to identify any past performance information

For example: Did the contractor meet the Governments needs, performed successfully, and accommodate changing requirements?
7. Accuracy
For example: Were the services comprehensive, complete, and feasible?
8. Overall Quality
For example: How was the vendor in regards to compliance with contract, requirements, quality achieved and overall technical expertise demonstrated?

For example: Was the contractor successful in planning and proposing realistic schedules, and implementing changes in a timely manner?
