TECHNICAL EXHIBIT 9

Laundry/Dry Cleaning Forms

The Contractor furnished laundry/dry cleaning forms shall conform to standard commercial practices. All forms shall be serially numbered.

Cadet/Cadet Candidate Laundry and Dry Cleaning forms shall include space for name, (last, first, middle initial), date, class, company, social security number (last four) and special instructions. It shall contain a column for quantity turned-in. Listings and groupings of items shall be furnished by the Government.

Sample forms currently used will be provided upon request. The Contractor shall coordinate with the COR in order to develop an acceptable plan to distribute laundry and dry cleaning forms and claims forms to the cadet companies/cadet candidates at no additional cost. All tickets/tags/forms provided by the Contractor shall be done at no extra cost to the Government and be approved by the Contracting Officer prior to their use.

Academic year forms shall be overprinted with the regimental number. There are 4 regiments.

USMAPS Cadet Candidate forms shall be overprinted with USMAPS.

Cadet Summer Training forms shall be overprinted with the appropriate program, currently CBT, CFT, CLDT, and SGR / STAP.

The Contractor is responsible for furnishing the forms upon request and shall have adequate amount of forms on hand to ensure uninterrupted service. The forms are requires to be 3 part forms.

Est. 1100 cadets in each REGIMENT Est. 250 cadet candidates in USMAPS Est. 1600 cadets CBT Est. 1500 cadets CFT Est. 500 cadets SGR / STAP Est. 1000 cadets CLDT
