About this file
This is a Performance Requirements Summary (PRS) document for a grounds maintenance contract at multiple facilities in Kansas, detailing performance standards and incentives/disincentives. The PRS outlines key contractor requirements including employee identification badges, herbicide usage restrictions, and service ticket documentation procedures. Each performance objective has a 100% compliance threshold with incentives of positive CPARS ratings and full payment, while non-compliance results in Corrective Action Requests (CARs), potential negative CPARS ratings, and possible payment reductions.
The document specifies requirements for different types of service tickets that must be submitted for various grounds maintenance activities, including general lawn maintenance (Types 1-4), spring cleanup, fall cleanup, and initial cleanup. Service tickets must be entered into the appropriate invoicing system within 30 days of work completion and must include details of services performed, dates, and signatures from both contractor POC and COR/DR. Special documentation is required for fertilizer applications using the 88th RD Universal Pest Management Application Reporting Form.

The KO may also reduce payment to reflect the value of the services performed if re-performance is not possible, or may have the identified non-conforming service performed and charge the contractor for any related cost incurred.
Use of herbicides (5.1.1) The contractor shall not use any chemicals in the performance of this contract with the exception of Paragraphs 5.3.5, 5.6.1, and 5.6.2, with the prior approval of the COR through the 88th RD Pest Manager.

Service Tickets (5.7) The contractor shall provide a service ticket for all grounds maintenance services provided each time services are performed on location and shall be submitted with the monthly invoice. The service ticket shall indicate which services were performed on which date and shall have a signature from the Contractor POC and COR or DR. COR or DR signature does NOT indicate acceptance of services, but simply indicates that the Contractor was on site working that day. The government still holds the right to perform surveillance as indicated in PWS 1.6. A sample service ticket for use is included in Part 7.4 of the PWS. The Contractor may use their own service ticket provided it contains which services were completed, the date they were completed and signature lines for the Contractor and COR or DR.
