Performance Requirements Summary (PRS) The contractor service requirements are summarized into performance objectives that relate directly to mission essential items. The performance thresholds provided below describe the minimum acceptable levels of service required for each requirement.

5.1.4 Food and Beverage

5.1.5 Additional Support Items

7.
5.1.7 Shuttle Service
a. Provide shuttle service for up to 700 attendees and staff, beginning Thursday through Sun.

5.2 Child and Youth Activities
Child and youth service activities must be provided to support up to 7 children from the age of new-born to 18 months; up to 7 children from the age of 19-36 months; up to 30 children between the ages 3-5; up to 30 children between the ages of 6-12, and up to 25 children between the ages of 13-17 (up to 100 total).

5.2.1.1 Child and Youth Activity Providers
The contractor shall provide proof, no later than 2 days prior to the start of the contract, that staff meets the minimum qualifications.

5.2.2 Health and Safety
All children in care shall have a registration form on file with the contractor that includes parents’/legal guardian emergency contact information and child’s allergies or other serious medical conditions.

5.2.3 Licensing and Insurance Compliance
The contractor shall be licensed, bonded or certified, Nationally Accredited and have the appropriate number of certified personnel available for the event.

Contractor primary staff must ensure LOSS – Line of Sight of local hired staff . Staff must where identifiable shirts.

5.2.4 Supplies
The contractor shall provide all personnel, equipment, supplies, and materials necessary to furnish full-time quality childcare.

13.
5.2.5 Security

a. Local, State, and Federal Background Investigation Requirements Childcare contractor must provide all necessary certifications for each employee.

b. Contractor shall ensure staff will be available Friday and Saturday for pre-registration, (photos, parental release for pictures, activities etc., forms to include documenting medical conditions, dietary restrictions, special needs, parental contact information, etc).

c. Contractor shall implement a sign-in/sign-out procedure. Each child shall be signed in/signed-out of childcare rooms by the child’s parent or legal guardian.

14.
5.2.6 Reports and Documentation
a. Customer surveys on the childcare services rendered shall be distributed by the contractor and collected by COR. The surveys will be completed by the parent/guardian of the child enrolled in childcare under this agreement. b. The last question on the survey will ask the parent or guardian to address any MAJOR issues/concerns with the childcare that the COR should be aware of, if any.

15.
PWS Para 5.3 Report ALL Contractor manpower required for performance of this contract
Reported NLT the 31st of October each year. Reporting period shall be the Period of Performance, not to exceed 12 months, ending September 30 of each government fiscal year. for Yellow Ribbon via secure data collection site.

Note: Government surveillance of contractor performance is not limited to the Performance Objectives as outlined in this PRS. The Government reserves the right to conduct compliance surveillance of any contractual requirement of this acquisition.

PRS Description of contents:

a. PRS #: indicates the numbering sequence within the PRS table .

b. Performance Objective: PWS paragraph number(s) specifying each required task(s) and brief summary of required task(s) to be monitored.

c. Performance Standard: appropriate performance standard, usually described in terms of quality and timeliness, so that a distinction can be made between satisfactory and unsatisfactory performance.

d. Acceptable Quality Level (AQL): This column lists the maximum allowable degree of deviation from perfect performance of each requirement before the Government considers contractor performance unsatisfactory. An AQL does not say the contractor may knowingly offer defective service to the Government, but recognizes defective performance sometimes happens unintentionally.

e. Workload Indicator(s)/Lot size: This column indicates what will be directly utilized to obtain the lot size and validate the performance objective is achieved.

f. Method of Surveillance: method(s) utilized to objectively perform surveillance of requirement.

g. Consequence of failure to achieve the AQL
