PERFORMANCE WORK STATEMENT (PWS)
FOR
Industrial Machine Maintenance Services
1.0 General:
1.1 Scope: The contractor shall provide all personnel, equipment, tools, materials, supervision, quality control and other items and non-personal services necessary to perform emergency repairs, non-emergency repairs, Preventative Maintenance (PM) services and OEM (Original Equipment Manufacturer) upgrades and modifications for Annex industrial machines as defined in this PWS, except as specified in Paragraph 3.0 as Government Furnished. The contractor shall perform to the standards in this contract.
1.2 Background: The industrial machines (Attachment A, B, & C) support all of the programs at Letterkenny Army Depot (LEAD). When these machines breakdown, long downtimes impact the ability of LEAD to meet production and delivery schedules of these programs. LEAD is requesting a service contract to ensure the machines are repaired and operational in the shortest amount of time to reduce the impact on production and delivery schedule. LEAD is a data-driven, metrics-based organization that is ISO 45001 certified, AS 9100 and AS 9110 certified.
1.3 Period of Performance (PoP): The Period of Performance shall be one Base Year of 12 months and two 12 month option years. The Period of Performance reads as follows:
1.3.1 Base Year: 29 APRIL 2025 to 28 APRIL 2026
1.3.2 Option Year One: 29 APRIL 2026 to 28 APRIL 2027
1.3.3 Option Year Two: 29 APRIL 2027 to 28 APRIL 2028
1.4 General Information:
1.4.1 Quality Control (QC): The contractor shall develop and maintain an effective QC program to ensure services are performed in accordance with this PWS. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The contractor’s QC program is the means by which it assures itself that its work complies with the requirements of the contract. As a minimum, the contractor shall develop QC procedures that address the areas identified in Technical Exhibit 1, Performance Requirements Summary. After acceptance of the QC Plan, the contractor shall obtain the Contracting Officer’s (KO) acceptance in writing of any proposed changes to its QC system.
1.4.1.1 Quality Assurance (QA): The Government shall evaluate the contractor’s performance under this contract in accordance with the Quality Assurance Surveillance Plan (QASP). This plan is primarily focused on what the Government must do to ensure that the contractor has performed in accordance with the performance standards. It defines how the performance standards are applied, the frequency of surveillance, and acceptable quality level(s) and defect rate(s)).
“Each worksite will be assessed for hazards prior to the start of work. All training certifications and an Activity Hazard Analysis (AHA) will be on hand at the job site. All training must adhere to all Federal, State, Local, DOD, Army, and all other applicable standards. All training must be performed by authorized trainers certified to perform training in the required field or discipline.”
1.4.2 Recognized Holidays: The following are recognized US holidays and the date for each can be found at the following website: http://www.opm.gov/Operating_Status_Schedules/fedhol/Index.asp. The contractor shall not perform services on these days:

1.4.3 Place and Performance of Services: The contractor shall operate within operating hours of Letterkenny’s standard work schedule of 0600-1530 (Eastern Time) Monday through Friday. The exception to this is the alternating Friday Regular Day Off (RDO), or on recognized US holidays or when the Government facility/installation is closed due to local or national emergencies, administrative closings, or similar Government-directed facility/installation closings. Performance for work under this contract shall be at Letterkenny Army Depot, 1 Overcash Ave, Chambersburg, PA 17201. The contractor shall at all times maintain an adequate work force for the uninterrupted performance of all tasks defined within this PWS when the Government facility/installation is not closed for the above reasons. When hiring personnel, the contractor shall keep in mind that stability and continuity of the work force is essential.
1.4.4 Access and general protection/security policy and procedures: Contractor and all associated sub-contractor’s employees shall comply with applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by Government representative). The contractor shall also provide all information required for background checks to meet installation access requirements to be accomplished by installation Provost Marshal Office, Director of Emergency Services or Security Office. Contractor workforce shall comply with all personal identity verification requirements as directed by DOD, HQDA and/or local policy. Should the Force Protection Condition (FPCON) change, the Government may require changes in contractor security matters or processes.
1.4.4.1 Background Checks: For contractors that do not require CAC (Common Access Card), but require access to a DoD facility or installation. Contractor and all associated sub-contractors employees shall comply with adjudication standards and procedures using the National Crime Information Center Interstate Identification Index (NCIC-III) and Terrorist Screening Database (TSDB) (Army Directive 2014- 05/AR 190-13), applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by Government representative), or, at OCONUS locations, in accordance with status of forces agreements and other theater regulations.
1.4.4.2 Physical Security: The contractor shall safeguard all Government property provided for contractor use. At the close of each work period, Government facilities, equipment and materials shall be secured.
1.4.4.3 AT Level 1 Awareness Training: All contractor employees, including subcontractor employees, requiring access to Army installations, facilities, and controlled-access areas shall complete AT Level I awareness training within 30 calendar days after the contract start date or effective date of incorporation of this requirement into the contract, whichever is applicable. The contractor shall submit certificates of completion for each affected contractor employee and subcontractor employee, to the COR or to the contracting officer, if a COR is not assigned, within 30 calendar days after completion of training by all employees and subcontractor personnel. 1.4.4.4 iWATCH Training: The contractor and all associated sub-contractors shall brief all employees on the local iWATCH program (training standards provided by the requiring activity ATO). This local developed training will be used to inform employees of the types of behavior to watch for and instruct employees to report suspicious activity to the COR. This training shall be completed within 30 calendar days of contract award and within 30 calendar days of new employees commencing performance with the results reported to the COR NLT 30 calendar days after contract award. iWATCH training can be found at:https://www.letterkenny.army.mil/ctrresources/
1.4.4.5 OPSEC Training: Per AR 530-1 Operations Security, the contractor employees must complete Level I OPSEC Awareness training. New employees must be trained within 30 calendar days of their reporting for duty and annually thereafter. Level I OPSEC Awareness training can be found at:

1.4.5 SAFETY: The Contractor shall comply with all applicable federal (OSHA), UFC, DOT, state, local and Letterkenny rules, regulations, and standards that pertain to safety. Contractor personnel shall be equipped with the appropriate protective safety equipment (e.g. respirator, clothing, foot protection, hearing and eye protection, etc.) before conducting maintenance in the industrial work environment at (LEAD). Safety equipment and procedures shall adhere to LEAD Safety Specifications as well as OSHA, NIOSH and ANSI recommendations/standards (See Section 6.0: Applicable Documents). Additionally, the contractor shall make themselves familiar with and comply with LEAD's local safety regulation LEAD- Reg 385-1, Safety Bulletins, Army Regulation 385-10 (The Army Safety and Occupational Health Program) prior to the start of any work at LEAD.
1.4.5.1 Job Safety Analysis and Safety Plan (JSA): The contractor shall complete a Job Safety Analysis (JSA) as well as a Safety Plan and submit to LEAD Contracting Office. The LEAD Safety Office will review the contractor’s JSAs and Safety Plan. The contractor shall be subject to a safety evaluation by Government personnel without prior notice any time the contractor is on-site and performing work at Letterkenny Army Depot.
1.4.5.2 Permitted Confined Space Entry: Letterkenny Army Depot requires a permit for Permitted Confined Space Entry in accordance with LEAD-R 385-1, Chapter 20. The contractor shall be responsible for obtaining and maintaining these permits and shall comply with all federal, state and local Letterkenny rules and regulations for Confined Space Entry (Permitted and Non-Permitted) (see Applicable Documents, Section 6.0, in this PWS). Contractor employees shall have permitted and non-permitted confined space program training by an OSHA-Certified instructor. The contractor shall supply a copy of all closed permit confine spaced permits to the LEAD Safety Office. Machinery requiring a confined space entry Permit can be confirmed by the COR or technical point of contract (TPOC) to the
COR.
1.4.5.3 Unsafe Condition: If contractor personnel determine a procedure is unsafe, they shall discontinue work immediately and contact their Program Manager who shall notify safety personnel and the COR. As soon as LEAD Safety Office personnel determine that the unsafe conditions have been corrected, the COR will notify the contractor Program Manager that personnel may resume work.
1.4.5.4 Accident Reporting: The contractor shall comply with reporting requirements of 29 CFR Part 1904, AR 385-40, and shall report accidents resulting in injury or death and accidents resulting in damage to Government property, supplies, and equipment to the COR promptly of the occurrence. The contractor shall a complete safety Investigation of injury/accident report; submit it to the COR Office with a copy to the LEAD Safety Office.
1.4.5.5 Safety Data Sheet: All shipped material shall include a current product Safety Data Sheet (SDS) for any chemical products being shipped other than residue. This includes items like engine oil. All safety messages or recalls related to the safe use of products produced by the contractor will be provided to the COR for the Contract with 5 days of contractor’s awareness of the message or notice.
1.4.5.6 Note: Safety Bulletin 23-01 Purpose: LEAD 385-1, Chapter 27-2 a (3) will be amended to read “A list of hazardous materials will be maintained at all times on LEAD worksites. All chemicals used on worksites must be vetted and approved by the LEAD safety office prior to use. The AMLD Form 4783 Contractor Chemical Checklist must be submitted along with the appropriate safety data sheets (SDS) for approval prior to any chemical use on LEAD worksites. All SDS’s must be present on file at all worksites as well as complete copies forwarded to the LEAD safety office. The LEAD safety office, may at any time inspect all worksites, including storage areas, to audit chemical inventories and there corresponding SDS’s for compliance.
1.4.5.7 Note: Safety Bulletin 23-02 Purpose: updates Chapter 10 of LEAD-R 385-1 which will supersede LEAD Safety Bulletin 22-06 and the Environmental Management System Procedure EEJ-010, Hazardous Material Management.
The purpose of this program is to define the responsibilities, procedures, and training requirements to ensure compliance with OSHA 29 CFR 1910.1200 Hazard Communication, OSHA 29 CFR 1926 Subpart D Occupational Health and Environmental Controls, LEAD policies, and National Fire Protection Association (NFPA) standards with regard to the handling, storage, and management of hazardous materials and selected hazardous items not to include radioactive materials, storage tanks, or hazardous wastes. This program applies to all activities, products, and services performed within the boundary of Letterkenny Army Depot (LEAD).
1.4.5.8 Safety Policy and Programs: The contractor is reminded that LEAD maintains a Safety Management System (SMS) LEAD-R 385-1 and Safety Bulletins and is ISO 45001 certified. The (LEAD-R 385-1) can be found at https://www.letterkenny.army.mil/info/Visitors-Information/. The LEAD Safety and Occupational Health program (LEAD-R 385-1). It is the contractor’s responsibility to be familiar with these documents and to comply with their contents. LEAD has classified their paint and blast booths as hazardous locations IAW with NFPA 70 Article 500 regulations. Please contact your local COR for guidance and training prior to entering these regulated areas.
1.4.5.9 Electrical Work: The contractor shall ensure all electrical work is performed in accordance with the National Electrical Code, the National Electrical Manufacturers Association and National Fire Protection Agency (NFPA 70). The contractor will be responsible for the electrical circuitry from the point of the main disconnect and any electrical circuitry that is a part of or subcomponent of the machine.
1.4.5.10 Hazardous Waste: All hazardous waste generated by the contractor’s activities shall be disposed of in a manner which is environmentally safe and diminishes potential health hazards by Letterkenny Army Depot (filters, augers, dust, etc.). The contractor shall coordinate with the TPOC or COR for the disposal of those wastes.
1.4.5.11 As part of the applicability of engineering controls, all equipment with rotating and moving parts at point of operation shall meet or exceed the LEAD 385-1 Chapter 17- 3 machine guarding requirements.
1.4.5.12 The contractor shall meet or exceed LEAD lockout/tagout (LOTO) action and procedures for addressing and controlling hazardous energy during service and maintenance of machines and equipment in accordance with LEAD-R 385-1, Chapter 12. See LEAD Technical Point of Contact (TPOC) for the safe servicing and maintenance of machines and equipment, in which unexpected, energized equipment, damage to equipment or start-up of machine or equipment, or release of stored energy, could cause injury to employees.
1.4.6 Conservation of Utilities: The contractor shall instruct employees in utilities conservation practices.
The contractor shall operate under conditions that preclude the waste of utilities, which include turning off the water faucets or valves after using the required amount.
1.4.6.1 Compliance with Environmental Laws and Regulations: Contractor shall comply with all applicable federal, state, and local environmental laws, statutes, regulations, executive orders, permits, Army regulations (with supplements), as well as Major Subordinate Command (MSC) and installation regulations and policies. Contractor shall immediately report any conflicts between applicable federal, state, local environmental laws, statutes, executive orders, and provisions of Army Regulation 200-1, and any specifications within this contract to the Contracting Officer Representative (COR) as well as the Director of Engineering & Public Works.
1.4.7 Special Qualifications: The contractor shall ensure that all personnel and sub-contractor personnel performing under this contract are fully trained, licensed, and competent to perform maintenance on the equipment listed in this contract.
1.4.8 Post Award Conference/Periodic Progress Meetings: The contractor agrees to attend any post award conference convened by the KO in accordance with Federal Acquisition Regulations Subpart 42.5.
The KO, Contracting Officer’s Representative (COR), and other Government personnel, as appropriate, may meet periodically with the contractor to review the contactor’s performance. At these meetings, the Contacting Officer will apprise the contractor of how the Government views the contractor’s performance and the contractor shall apprise the Government of problems, if any, being experienced. The contractor shall resolve outstanding issues raised by the Government. Contractor attendance at these meetings shall be at no additional cost to the Government.
1.4.9 Contract Manager (CM): The contactor shall provide a CM, who shall ensure performance under this contract. The name of this person, and an alternate who shall act for the contractor when the CM is absent, shall be designated in writing to the KO. The CM or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract.
1.4.10 Organizational Conflicts of Interest (OCI): The contractor and subcontractor personnel performing work under this contract may receive or have access to contractual documentation or participate in the development of proprietary or source selection information (e.g. cost or pricing information, budget information or analyses, specifications or work statements, etc.) or perform evaluation services which may create a current or subsequent OCIs, as defined in FAR Subpart 9.5. The contractor shall notify the KO immediately whenever it becomes aware that such access or participation may result in any actual or potential OCI and shall promptly submit a plan to the KO to avoid or mitigate any such OCI. The contractor’s mitigation plan will be determined to be acceptable solely at the discretion of the KO and in the event the KO unilaterally determines that any such OCI cannot be satisfactorily avoided or mitigated, the KO may affect other remedies as he or she deems necessary, including prohibiting the contractor from participation in subsequent contracted requirements which may be affected by the OCI.
1.5.5 Identification of Contractor Employees: All contact personnel attending meetings, answering Government telephones and working in other situations where their contractor status is not obvious to third parties are required to identify themselves as such to avoid creating an impression in the minds of members of the public that they are Government officials. The contractor shall also ensure that all documents or reports produced by contractor personnel are suitably marked as contractor products or that contractor participation is appropriately disclosed.
1.5.6 Contractor Travel: The contractor shall be responsible for making all travel arrangements for their OEM Technicians traveling to Letterkenny Army Depot (LEAD) in order to perform requested OEM machine maintenance service(s). The contractor shall provide the Contracting Official Representative (COR) with quote for all travel expenses listing the individual costs for lodging and travel with the name(s) of their technicians who will be performing the service work order. Travel expenses may also include shipping of parts, tools & equipment on the billing invoice for OEM Technical Services under the correct CLINs. The contractor shall only quote and submit necessary travel expenses to the LEAD-COR for validation and approval.
1.5.7 Data Rights: The Government has unlimited rights to all documents/material produced under this contract. All documents and materials, to include the source codes of any software, produced under this contract shall be Government owned and are the property of the Government with all rights and privileges of ownership/copyright belonging exclusively to the Government. These documents and materials may not be used or sold by the contractor without written permission from the KO. All materials supplied to the Government shall be the sole property of the Government and may not be used for any other purpose.
This right does not abrogate other Government rights.
1.5.8 Contract Manager (CM): The contactor shall provide a CM, who shall ensure performance under this contract. The name of this person, and an alternate who shall act for the contractor when the CM is absent, shall be designated in writing to the KO. The CM or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract.
1.5.10 Phase In / Phase Out Periods To minimize any decreases in productivity and to prevent possible negative impacts on additional services, the contractor shall have personnel available and on call 24/7 to provide services during phase in/ phase out periods. The phase in/out service periods are primarily used during the transition between contractors. During the phase in period, the contractor shall become familiar with performance requirements in order to commence full performance of services on the contract start date (There are No other special phase in/out requirements for this contract except coordination between contractors).
2.0 Definitions and Acronyms:
2.1 CONTRACTOR. A supplier or vendor awarded a contract to provide specific supplies or service to the Government. The term used in this contract refers to the prime. Reference to Contractor includes sub- Contractors.
2.2 CONTRACTING OFFICER (KO). A person with authority to enter into, administer, and or terminate contracts, and make related determinations and findings on behalf of the Government. Note: The only individual who can legally bind the Government.
2.3 CONTRACTING OFFICER'S REPRESENTATIVE (COR). An employee of the U.S. Government designated by the KO to monitor contractor performance. Such appointment shall be in writing and shall state the scope of authority and limitations. This individual has authority to provide technical direction to the Contractor as long as that direction is within the scope of the contract, does not constitute a change, and has no funding implications. This individual does NOT have authority to change the terms and conditions of the contract.
2.4 DEFECTIVE SERVICE. A service output that does not meet the standard of performance associated with the Performance Work Statement.
2.5 DELIVERABLES. Anything that can be physically delivered and includes non-manufactured things such as meeting minutes or reports.
2.6 KEY PERSONNEL. Contractor personnel that are evaluated in a source selection process and that may be required to be used in the performance of a contract by the Key Personnel listed in the PWS.
When key personnel are used as an evaluation factor in best value procurement, an offer can be rejected if it does not have a firm commitment from the persons that are listed in the proposal.
2.7 PHYSICAL SECURITY. Actions that prevent the loss or damage of Government property.
2.8 QUALITY ASSURANCE (QA). The Government procedures to verify that services being performed by the Contractor are performed according to acceptable standards.
2.9 QUALITY ASSURANCE SURVEILLANCE PLAN (QASP). An organized written document specifying the surveillance methodology to be used for surveillance of contractor performance.
2.10 QUALITY CONTROL. All necessary measures taken by the Contractor to assure that the quality of an end product or service shall meet contract requirements.
2.11 SUBCONTRACTOR. One that enters into a contract with a prime contractor. The Government does not have privity of contract with the subcontractor.
2.12 WORKDAY. The number of hours per day the Contractor provides services in accordance with the contract.
2.13 WORK WEEK. Monday through Friday, unless specified otherwise.

3.0 GOVERNMENT FURNISHED PROPERTY, EQUIPMENT AND SERVICES (GFP/M/E/S): The Government will provide the facilities, equipment, materials, and/or services listed below:
3.1 Facilities: All utilities in the facility will be available for the contactor’s use in the performance of this contract and clear access to the equipment will be provided.
3.2 Equipment: The Contractor shall furnish all the necessary equipment to provide support outlined in this PWS to include mechanical equipment and tools.
3.3 Materials/Consumables: The Government shall supply oils necessary for servicing of machines. The Government shall also dispose of replaced oils for service of machines. Should the Government not have the necessary oils, parts and materials readily available, the contractor may provide them upon prior approval or request by the COR (See para 3.6). Government will make available technical service manuals for onsite use and reference.
3.4 Services: The contractor shall provide all required services or arrange such services by an approved & certified sub-contractor per the PWS. The contractor shall only provide services per the service agreements of this contract. All changes and modification to this services agreement must be approved by the LEAD Contracting Officer without exception.
3.5 Utilities: All utilities in the facility will be available for the contactor’s use in the performance of this contract.
3.6 This is a non-personal service(s) contract under which the personnel rendering the service(s) are not subject, either by the contract’s terms or by the manner of its administration, to the supervision and control usually prevailing in relationships between the Government and its employees. This contract is not for services that are inherently Governmental.
3.7 Contractor acquired materials/consumables: When the Government does not have the necessary, oils, materials or consumables available to service machinery, the contractor shall notify the COR for approval prior to obtaining, purchasing and shipping any parts, materials or consumables to Letterkenny Army Depot. The COR shall submit a request to the contractor for any additional maintenance parts, materials or consumables needed to service the machinery. The Government maintains the right to replace or install parts and materials shipped by the contractor. The contractor shall provide the COR with all quotes and shipping costs requiring approval prior to purchasing any parts, materials and consumables items. The contractor shall provide the COR with copies of all tracking numbers and shipping documents for all items being shipped to Letterkenny Army Depot to perform maintenance services. The COR will notify the contractor upon receipt of all parts, materials and consumables and provide copy of the packing slip(s) to the contractor for their records. Material and consumables acquired by the contractor shall be invoiced under the parts CLIN of the contract.
4.0 Contractor Furnished Property, Materials & Equipment (CFPM& E)
4.1 General: Except for those items specifically stated to be Government-Furnished in Paragraph 3.0, the contractor shall furnish everything required to perform these services as indicated in Paragraph 1.1.
5.0 REQUIREMENTS:
5.1 The contractor shall provide all labor, material, parts, and equipment necessary to perform preventative maintenance (PM) on the machines in paragraphs 5.1 through 5.4 and perform unscheduled emergency and nonemergency maintenance on the machines list in Technical Attachment A, B, and C (See para 3.3 and 3.6).
5.1.1 All maintenance shall be performed in accordance with and meet all original equipment manufacturer (OEM) specifications. All PMs shall be completed within 30 days of notification by the COR that a PM is due. To eliminate delays the contractor shall notify the Government of the types of oils, lubricants, or filters and quantities needed for the service to be performed as specified in the applicable OEM manuals.
5.1.2 The contractor shall coordinate the delivery schedule of these oils, lubricants or filters with the COR when shipping hazardous material to LEAD (See para 3.3 and 3.6). The contractor’s technician’s shall notify the COR, utilize the In/Out Sheet upon their arrival/departure at end of each work day.
5.1.3 PREVENTIVE MAINTENANCE AND CHECKLISTS: See Letterkenny Army Depot’s (LEAD) PM Checklist for Specific Machinery Service Requirements (Attachment A):
5.1.4 All PMs shall be completed within 30 days of notification from the COR that a PM is due.
5.1.5 A PM report shall be submitted upon completion of each PM for each machine.
5.2 PREVENTATIVE MAINTENANCE HAAS HORIZONTAL CNC (MODEL EC1600, S/N 2051935): The PM shall be performed one (1) time per year during LEAD duty hours and as described in the specific OEM Manual, Technical Exhibit 1 (Item No. 1) and LEAD PM Checklists (Attachment A).
5.3 PREVENTATIVE MAINTENANCE INGERSOLL CNC MACHINE (MODEL MASTERSPEED, S/N
2N00066-1)
5.3.1 The PM shall be performed one (1) time per year during LEAD duty hours and as described in the specific OEM Manual, Technical Exhibit 1 (Item No. 1) and LEAD PM Checklists (Attachment A).
5.4 PREVENTIVE MAINTENANCE OF MILL AND LATHE MACHINES:
5.4.1 The contractor shall provide all, labor, tools, materials, parts, and equipment necessary to perform annual preventative maintenance on the mill and lathe machines on specified scheduled dates provided by the COR. All maintenance shall be performed specific to the OEM Manual, Technical Exhibit 1-2 and LEAD PM Checklists (Attachment A), Annual PMs shall all include all evaluating or repair of electrical and mechanical components as well as ball bar and laser alignments and required changes of worn or broken parts, air filter, oils & lubricants, light bulbs, drive belts, fuses, and evaluating and resolving machine malfunctions.
Reporting Mill & Lathe Minimal Items that must be addressed and reported during Mechanical PMs:
- R1: Record voltage and amp readings
- R2: Mega-ohm servo-motors and spindle drive motors.
- R3: Verify electrical connections being tight.
- R4: Test safety interlocks, verify axis over travel limits switches.
- R5: Operational Test, spindle speed ranges, tool change functions.
- R6: Draw oil for oil analysis, results to be included in report, top off oil levels, drain and fill as necessary.
- R7: PM spindle tapers and drawbar functions.
- R8: Lube pressures and air pressure verifications.
- R9: Verify counterbalance pressures.
- R10: Provide documentation via individual report, above information with printed report if Ball bar and laser testing results with any comp that has been entered to correct and linear errors.
- R11: Results of OSHA Safety Requirements.
- R12: Ball bar and laser alignment reporting.
5.4.2 All PMs shall be completed within 30 days of notification from the COR that a PM is due. To eliminate delays the contractor shall notify the Government of the types of oils and quantities needed for the service to be performed as specified in the applicable OEM manuals. The contractor shall coordinate the delivery schedule of oils, lubricants and other hazardous materials with the COR (See para 3.3 and 3.6). The contractor shall notify the COR prior to disposal of any hazardous waste on Letterkenny Army Depot.
5.4.3 A PM report shall be submitted to the COR upon completion of each PM.
5.3.4 Minor adjustments to mill/lathe machines (defined as repairs which require less than 1.0 hour to perform and zero part(s) orders) shall be performed under CLIN 0001 during preventive maintenance. If, upon PM, the contractor determines that major adjustments or repairs (defined as repairs which require more than 1.0 hour to perform and/or a part(s) order) are needed, then these actions shall be classified as and performed under nonemergency repair.
5.4.4 In addition, a safety review corresponding to all applicable OSHA requirements for mill and lathe machines shall be performed.
5.5 PREVENTATIVE MAINTENANCE INGERSOLL CNC MACHINE (MODEL MASTERHEAD, S/N
28525):
5.5.1 The PM shall be performed one (1) time per year during LEAD duty hours and as described in the specific OEM Manual, Technical Exhibit 1 through 2 and LEAD PM Checklists (Attachment A).
5.6 PREVENTATIVE MAINTENANCE STRIPPIT CNC LASERS (MODEL IMPULS 4030, S/N 30401 &
Phoenix FL-6020, S/N 39945): The contractor shall perform (1) Annual Preventive Maintenance Service Call (PM) on 2 LVD Strippit Lasers Machines with (2) Annual PM to reset the machines timers and prevent stoppages. The contractor shall perform performance and preventative maintenance on the Strippit Laser Machine as stated in below paragraphs (See Attachment B: 1 = 1 PM ).
5.6.1 The contractor shall perform Annual Preventative Maintenance (PM) on the prescribed Laser Machines to include all items specific to the OEM Manual, Technical Exhibit 1-2 and LEAD PM Checklists (Attachment A).
5.6.2 The dust collection systems on the Laser machines shall be equipped with sensors and vibration analysis components to continuously monitor those systems for impending failures. COR will be notified of degrading of systems for approvals on repairs and replacements.
5.6.3 All maintenance shall be performed in accordance with original equipment manufacturer (OEM) specifications following the OEM’s manual. All PMs shall be completed within 30 days of notification from the COR that a PM is due. A PM report shall be submitted upon completion of each PM.
5.6.4 Turbo Blowers shall be replaced every 24,000 operating hours. During replacement, the turbo blower oil shall be replaced with new oil per the OEM manual.
5.6.5 The contractor shall coordinate the delivery schedule of oils, lubricants and other hazardous materials with the COR (See para 3.3 and 3.6). The contractor shall coordinate with the COR prior to disposal of used hazardous waste.
5.6.6 All PM parts required to perform shall be billed under the parts CLIN as actuals.
5.7 PREVENTIVE MAINTENANCE FOR CNC WATERJET MACHINES: The contractor shall perform Annual Preventive Maintenance (PM) on the Waterjet Machines within 30 days after task order is issued and every 4,000 hours thereafter (pending as task order for services).
5.7.1 The contractor shall provide all, labor, tools, materials, parts, and equipment necessary to perform preventative maintenance (PM) on the waterjet machines listed in the Original Equipment Manufacturer (OEM) Manual, Technical Exhibit 1 through 2 and LEAD PM Checklists (Attachment A).
5.7.2 The contractor shall coordinate the delivery schedule of repair parts, oils, lubricants and other hazardous materials with the COR. This also includes disposal of used hazardous waste.
5.7.3 All PMs shall be completed within 30 days of notification from the COR that a PM is due. PMs shall be performed during LEAD duty hours unless otherwise requested by the COR unless, under special or emergency circumstances that may preventive maintenance work being performed under normal working hours.
5.7.4 A PM Report shall be submitted upon completion of each PM and/or related service work order.
5.8 WATERJET & LASER MACHINE PM ADJUSTMENTS: Minor adjustments to waterjet machines (defined as repairs which require less than 1.0 hour to perform and zero parts(s) order shall be performed under the appropriate Cost Line Item Number (CLIN) of the delivery/task order during the annual PM. If upon the PM the contractor determines that major adjustments or repairs (defined as repairs which require more than 1.0 hour to perform and/or order are needed, then these actions shall be classified as and performed under non-emergency repair.
5.9 PREVENTIVE MAINTENANCE FOR WIRE EDM’s: The contractor shall provide all, labor, tools, materials, parts, and equipment necessary to perform annual preventative maintenance on the Wire Electrical Discharge Machines (EDM’s) as listed in Technical Exhibits 1 through 2. All maintenance shall be performed in accordance with original equipment manufacturer (OEM) specifications following the machines OEM’s manual.
5.9.1 Minor adjustments to the EDM’s (defined as repairs which require less than 1.0 hour to perform and zero part(s) orders) shall be performed under designated CLIN during annual preventive maintenance. If, upon PM, the contractor determines that major adjustments or repairs (defined as repairs which require more than 1.0 hour to perform and/or a part(s) order) are needed, then these actions shall be classified as and performed under nonemergency repair.
5.10 NON-EMERGENCY MAINTENANCE RESPONSE: The Contractor shall be obligated to perform diagnostic services, upon request when notified by the COR. The Contractor shall be required to schedule the non-emergency repair within 72 hours, or less after receiving a call requesting service. Non- Emergency or Routine services performed IAW this scope of work will normally be accomplished Monday through Friday, between 0600 and 1530 hours. The Government reserves the right to perform non-emergency repairs in-house as required. The contractor shall schedule all non-emergency repair visits with the COR.
5.10.1 Repairs identified by non-emergency diagnostic services shall be coordinated with the COR, who shall authorize repair by the method most advantageous to the Government. All work performed must be approved by the COR and verified for completion.
5.10.2 The contractor shall coordinate arrival dates with the COR. The contractor does NOT have exclusive rights to perform all non-emergency maintenance. The Government reserves the right to perform minor non-emergency repairs in-house as required. The contractor must inform the COR for approval prior to purchasing any repair parts. A quote shall be provided to the COR for fair market price evaluation.
5.10.3 The contractor shall coordinate the delivery schedule of repair parts, oils, lubricants and other hazardous materials with the COR (See para 3.3 and 3.6). This also includes disposal of used hazardous waste.
5.11 EMERGENCY MAINTENANCE RESPONSE: The Contractor shall be obligated to perform emergency repair service call within 4 hours, upon request when notified by the COR. The Contractor shall be required to arrive on-site within 48 hours, or less after receiving a call requesting emergency service. Repairs identified by the emergency diagnostic services will be addressed by the COR who will authorize repairs by the method most advantageous to the Government. Emergency on-call services performed IAW this scope of work will normally be accomplished Monday through Friday, between 0700 and 1530 hours. The Government reserves the right to perform emergency repairs in-house as required.
The contractor shall schedule all emergency repair visits with the COR or TPOC.
5.11.1 Repairs identified by emergency diagnostic services shall be coordinated with the COR, who shall authorize repair by the method most advantageous to the Government. 5.11.3 The contractor shall coordinate the delivery schedule of repair parts, oils, lubricants and other hazardous materials with the COR. This also includes disposal of used hazardous waste.
5.12 SERVICE PARTS & EQUIPMENT: The contractor shall provide & perform repair OEM resolution services to include labor, tools and all necessary “parts and equipment” for all work to be performed under this contract (QASP Para. 5.0).
5.12.1 The COR shall approve all parts costing more than fifty dollars ($50) each that are parts required by services performed under paragraphs 3.0 through 3.6 and 5.0 through 5.13. Repair parts shall be billed as actual per the parts under CLIN/instructions. Funding for repair parts will be allocated on individual delivery orders. This shall not be construed to mean that the contractor has exclusive rights to supply all parts needed to perform non-emergency maintenance and emergency maintenance.
5.13 REPAIR/OEM RESOLUTION: The contractor shall be contacted when the COR determines that a machine requires maintenance support (see paragraphs 3.0 through 3.6 and 5.0 through 5.1). The COR or LEAD’s Technical Advisor shall determine if the contractor has repaired a machine back to its normal operating condition. This will include system upgrades, modifications as well as repairs and preventative maintenance. The contractor will coordinate the upgrade or modification with the OEM. Should an ongoing problem continue to re-occur and the contractor is unable to identify and/or resolve the issue within 48 Hours, the COR may submit a formal request that the contractor schedule a certified OEM technician to correct the failure the scheduling shall be at NO additional cost to the government.
5.13.1 The sub-contractor shall coordinate the delivery schedule of repair parts, oils, lubricants and other hazardous materials with the COR (See para 3.3 and 3.6). The contractor shall also coordinate with the COR prior to the disposal of used hazardous waste on Letterkenny Army Depot.
5.13.2 Payment for services are dependent upon successful repair of the subject units.
5.14 TECHNICAL TELEPHONE SUPPORT: The contractor shall supply all technical telephone support to aid in troubleshooting and repair of machine faults and problems at no additional cost to the Government.
If a resolution cannot be provided over the phone the contractor shall perform service under non-emergency or emergency service depending to the severity of the issue.
5.15 REPORT: At the end of each maintenance visit, the contractor shall provide the COR with a written report within 10 days of completion of approved work by the COR or LEAD’s Technical Advisor.
5.15.1 PM REPORTS: All PM reports shall contain the following items. The HAAS EC-1600 PM report shall also include a copy of the ball bar test results from the ball bar calibration.
5.15.2 General Condition: Status of the machine by major component.
5.15.3 Repair Parts: List of required parts, by nomenclature. Cost breakdown of parts and labor.
5.15.4 Repairs Completed: Cost breakdown of parts and labor.
5.15.5 Fire and Safety Violations: Location of violations. Cost breakdown by parts and labor, to repair violations.
5.15.6 Electrical Code Violations: Location of violations. Cost breakdown of parts and labor, to repair violations.
5.15.7 Corrective Action Statement: Status of system and description of malfunction. Location of system.
5.15.8 Estimated repair time: Estimated time for parts to be received, this includes cost of required repair parts and labor to complete the job.
5.16 REPAIR REPORT: The contractor shall submit a repair report for all services, emergency and non-emergency that includes a description of the repairs completed and cost breakdowns of parts and labor.
5.17 WARRANTY OF PARTS/MATERIALS: The contract shall provide a standard commercial warranty furnished with the equipment equal to at least the same warranty the OEM would provide. The warranty shall cover all workmanship and parts prescribed.

7.0. Contractor Compliance: The contractor shall not comply with any order, direction, or request of Government personnel unless it is issued in writing and signed by the Contracting Officer, or is pursuant to specific authority otherwise included as a part of this contract.
