PAGE AND PARAGRAPH OFFEROR’S QUESTIONS GOVERNMENT’S RESPONSE
1. p. 3-5 lists maximum quantities listed
Can you provide the numbers of services for 2017 or another recent year in the total of medical consult services, ancillary, and laboratory?
See attached Amendment 12 which lists historical Medical Consult data for FY 16 and FY 17. Data for Ancillary and Laboratory services is unavailable.
2. p. 41 par., 1, p. 69 mid-page as a header, other locations pages 41-69
Exhibit G is missing from the RFP. Can you please provide one?
There is no Exhibit G until the contract is awarded.
Attachment 11, which is attached to this amendment is to be filled out and will turn into Exhibit G once the contract is awarded.
3. p. 76 par. 3(b) Regarding MCN’s key personnel, are we required to submit resumes/CVs with the proposal as part of describing their “Qualifications,” (or in any other context) and if so does this count against the page total?
Resumes are not required
4. Attachment 11
Will the Government confirm that contractor's response to the categorization of Attachment 11 will become Exhibit G after contract award?
Yes, Attachment 11 will become Exhibit G after the contract is awarded. See attached replacement Attachment 11.
5. Page 3, Amendment 1, Paragraph A
Are Offerors instructed to change the naming convention of Attachment 9 from "Attachment9PriceMatrix13F eb18" to simply "Medical Referral Price Matrix" as referenced on Page 3?
Yes, the instructions in the solicitation and in the amendment state to name the CD, “VOL IV, (Company Name), Medical Referrals Price Matrix (date)
6. Pages 5 and 10, RFP Amendment 1, Proposal Files Table
Section 3, Content Requirements, Volume III
The proposal files and content instructions appear to differ for the Performance Risk Proposal volume. The seven proposal file table
Section 3, will be amended to add, Performance Assessment Information, Attachment 5. The requirements of Volume III, document files (page 5 and Attachment 2) do not align to the six sections outlined in the content requirement section (pages 9-10). Can the Government clarify the requirements for Volume III?
Can the Government confirm that each vendor's performance assessment write-up should be in an individual file? What file should the vendor include the information required in Attachment 5? Should Attachment 5 be submitted as a separate excel file?
include 1 CD-R with the following files: Table of Contents, Subcontractor Teaming Partner Consent Letter, Subcontractor Teaming Partner Table, Performance Assessment Information (Attachment 5- Excel Document); one file for each Performance Assessment Submission, up to 3, and Glossary of Abbreviations and Acronyms.
7. Page 12, Paragraph 1.13.2
Will MEPCOM be supplying biometric scanners to provider office locations or will the MEPS be sending these along with the applicants to the appointments? Will the government be providing the training to the providers on the biometric scanners?
When and if implemented the government would provide biometric scanners to each individual MEPS facility.
The government would provide training documents on the use of a biometric scanner.
8. Page 51, Paragraph 5.3
Can the Government confirm the exhibit references in Section 5.3 and its subsections?
The exhibit reference in the Performance Work Statement paragraph 5.3, Exhibit G is changed to Exhibit B. See attached amendment.
9. Page 68, Exhibit C
For Objective 1, please confirm that no more than 2% of medical provider network locations should exceed the requirement of 30 miles from each MEPS station without prior approval of the KO or COR.
Yes, as stated on page 68, Exhibit C, Performance Requirements Summary, “No less than 98% of medical provider network locations should exceed more than 30 miles from each MEPS station without prior approval of the KO or COR.”

Please confirm that Objective 2 is based on exams completed in network versus out of network.

11. Page 72, Paragraph 1.c(1)
"If there are more than 45 lines of text on any page, the additional lines will not be evaluated. Headers, footers and page numbers are not included in the line numbering" Are section headers, tables, and graphics included in the line count (Microsoft Word 2007 does not have a function to include tables and graphics in the line count)?

12. Page 74, Paragraph 3, Proposal Instructions
"All information shall be confined to the appropriate volume/section. For example, only information included in the section labeled "Subfactor 1B - Management and Technical Approach" will be evaluated for "Subfactor 1B - Management and Technical Approach". If "Subfactor 1B
- Management and Technical Approach" information is included elsewhere in the proposal, it will not be evaluated as part of the "Subfactor 1B - Management and Technical Approach". For example, information about systems security would be addressed in detail in Subfactor 1D, but might also have relevance to Subfactor 1A
13. Page 80, Small Business Participation Proposal
To fulfill the subcontracting goal of 65% there will be thousands of providers in the network. Providing copies of each provider's subcontracting agreement will result in approximately 10,000 pages. Will the Government accept the subcontracting template with an attestation that each identified subcontracted provider has signed the agreement?
See attached amendment.
Goals have been changed from Current MEDCOM goals to Current DOD goals.
Additionally, as the Small Business Participation Proposal is a sample format only, the government will accept the template with an attestation that each subcontractor has signed their agreement.
14. Page 80, Small Business Participation Proposal
Small businesses typically do not have CAGE codes. Will the Government waive the requirement of providing CAGE codes for small businesses?
Based on the estimated magnitude of potential subcontractors, providing cage codes for each small business is not required.
15. Page 81, Small Business Subcontracting Plan
Can the Government confirm that Offerors are to only propose small business subcontracting goals for the 4 categories listed (Small Business, Small Disadvantaged Business, Woman-Owned, & Service Disabled Veteran-Owned)?
See attached amendment.
Section has been updated to include HUBZone, which brings the number of categories to 5.
16. Page 81, Small Business Subcontracting Plan
Section (a) states that if the Offeror is other than a small business, it may complete Attachment 10 Subcontracting Plan.
However, Attachment 10 Subcontracting Plan states in the heading: "Individual Subcontracting Plan Checklist and Review For Informational Purposes Only (Offeror does not fill in)" (emphasis added).
Would the government provide an attachment that the Offerors may fill in to fulfil the requirement of Volume VI Section (a)?
Section (a) under Volume V- Small Business Subcontracting Plan, will be corrected to, “If the offeror is other than a small business, the offeror shall submit an Individual, Master, or Commercial Subcontracting Plan.”
Attachment 10 is for the government to use only to evaluate an offerors plan.
17. Page 83, Attachment

Please confirm offerors are allowed to provide their experience within Volume II.
No, experience should only be listed in the Performance Risk Volume, just as pricing should only be listed in the Pricing Volume.
20. Pages 16 and 39, Paragraphs 4 and b(10)
Can the Government confirm if the Service Contract Act applies?
Yes, the Service Contract Act applies
21. Pages 3,4 and 5, SF 1449 continuation sheets
Should offerors populate the SF1449 continuation sheets with pricing? If so, what proposal volume should they be included?
No, only the price matrix shall include pricing information. Include a signed page 1 of the SF 1449 by an authorized representative and the two signed amendments in your Volume I, Business Volume.
22. Pages 4 and 5, Amendment 1
Letter d) "Complete Proposal" defines complete as including all five proposal volumes. However page 5, Proposal Table of Contents references six volumes. Can the Government confirm that submission of six volumes would comprise a complete proposal?
It should be six volumes.
Corrected to state six. See amendment, (A) Proposal Submission, 1. Introduction,

23. Pages 5 and 15, Amendment 1 and Solicitation page 83, Attachment 2
Will the Government clarify the discrepancy between the instructions on pages 5 and 15 with regards to the labeling of Volume II 'Regular vs Sanitized' copies?
Corrected. There should be 1 CD-R, unsanitized and 3 CD- Rs sanitized. See corrected Proposal Table of Contents in the attached amendment
24. Pages 79 and 104, Price Proposal 3(a) and Price Evaluation Approach 3(d)
These pages contain references to Offerors supplying composite labor rates and proposed hourly rates for each medical specialty. Since Offerors are proposing firm-fixed prices via Attachment 9 requirements, will the Government please remove the language regarding submission of labor/hourly rates?
Language-labor rates and proposed hourly rates corrected to state, “prices”.
