The following have been modified:
ADDENDUM TO 52.212-1
Addenda to 52.212-1(b), Submission of Proposals. The paragraph is tailored as follows:
Paragraph (b), the second sentence is deleted and replaced with the following: “Proposals must be submitted on the SF 1449 and accompanied by all requested items elsewhere specified in the solicitation.
Paragraph (b) (10) is deleted and replaced by information required to be submitted in Volume III of the proposal.
Addenda to 52.212-1(c), Period for Acceptance of Proposals. The paragraph is tailored as follows: “The Offeror agrees to hold the prices in its offer firm for 180 calendar days from the date specified for receipt of proposals.”
Addenda to 52.212-1(e), Multiple Proposals. This paragraph is deleted in its entirety.
Addenda to 52.212-1(g), the following paragraph is added to the end of the current paragraph as follows: "After evaluating proposals, if discussions are deemed necessary, the contracting officer may determine that the number of most highly rated proposals that might otherwise be included in the competitive range exceeds the number at which an efficient competition can be conducted. In this event, the contracting officer may limit the number of proposals in the competitive range to the greatest number that will permit an efficient competition among the most highly rated proposals."
Addenda to 52.212-1 (m) Organizational Conflict of Interest. For those Offerors planning to engage in a teaming arrangement or to use subcontractors, a conflict of interest may be created if a company is identified as a prime Offeror, teaming partner/subcontractor, or as a member of a Joint Venture on more than one proposal. Each participating company shall only submit one proposal. If the contracting officer concludes during proposal evaluation that a potential conflict of interest exists because one or more companies have participated in more than one proposal as a prime Offeror, teaming partner/subcontractor, or as a member of a Joint Venture, the contracting officer reserves the right to require a conflict of interest mitigation plan from the prime Offerors of those proposals.
If an Offeror fails to submit a plan, or submits an inadequate plan, the Offeror may not be considered.
INSTALLATION ACCESS. Effective 1 May 2013, access to Fort Sam Houston (FSH) Main is closed except for pre-vetted and approved credential holders IAW Air Force Federal Acquisition Regulation Supplement 5352.242-9000 (Contractor Access to Air Force Installations); AFI 31-101 (Integrated Defense); AFI 31-113 (Installation Perimeter Access Control); and AFI 31-502 (Defense Biometric Identification Systems (DBIDS) Operations. Therefore, the US Government (USG) is not responsible for delays at the gate when proposed Offerors are in the process of delivering proposals to the Health Readiness Contracting Office (HRCO) office at Bldg 4197 by the due date and time annotated on the Request for Proposal. As Visitor Control Centers become operational, the proposed Offeror must stop by one of the Visitor Control Centers prior to entry to be vetted and issued a pass. The Visitor Control Centers are located at the FSH Main Entrance through Walters Street and Harry Wurzbach-East off Scott Road. The proposed Offeror must present a valid photo ID at the Visitor Control Center to be vetted and issued a pass. Any proposed Offeror found to have derogatory information during the vetting process will be denied access.
(A) PROPOSAL SUBMISSION
1. INTRODUCTION.
a) The Offeror shall submit its proposal to the office indicated in block 9 of the Standard Form (SF) 1449, Solicitation/Contract Award. To be considered for evaluation, proposals must be received no later than the date and time (Central Standard Time) specified in Block 8, SF 1449. The Offeror shall mark the exterior of their package and each CD-R with the solicitation number specified in Block 5, SF 1449. Proposals shall be submitted in the format stated in the “Proposal Table of Contents” listed below in Section 2 “Proposal Files”. Each volume of the Offeror’s proposal shall be submitted in electronic form on a separate CD-R as specified in the table below entitled, “Proposal Table of Contents.” Telegraphic, electronic mail, and facsimile proposals will not be accepted. Files shall not contain classified data. Each paragraph shall be separated by at least one blank line. A standard, 12-point minimum font size applies. Margins: Top, Bottom, Left, Right – 1” Page Size, Width – 8.5” Page Size, Height – 11”. Tables and illustrations may use a reduced font size not less than 8-point and may be landscape.
b) Electronic Files (CD-Rs). Offerors shall submit their proposal on CD-Rs that are clearly labeled with the Offeror’s name, cage code, volume number and volume title. Electronic files shall not contain classified data.
Each CD-R shall be made read only and “burned” at a minimum 8x speed. In addition, each CD-R must be made “final.” “Final” is a recording option that renders the CD totally used so that no other data tracks can be added. Do not use compressed file formats. Submissions shall be virus free. Electronic files shall be submitted in a format that is compatible with Microsoft Office 2007 applications. Each CD-R shall be submitted in a separate CD package (i.e. die-cut jacket, plastic case, CD sleeve, CD bag) clearly marked with the volume number and volume title and shall be submitted as a set.
(1) Submit price proposals on Attachment 9 entitled “Medical Referrals Price Matrix”. The Price Matrix must remain in Excel format and all pre-loaded formulas shall remain intact. The USG will not correct errors created due to changes by the Offeror of pre-loaded formulas. Such errors may render the Offeror’s proposal ineligible for award. The Offeror may submit with their price proposal pricing rationale in Microsoft Word format.
(2) Each volume of the Offeror’s proposal (Business proposal, Technical Quality proposal, Performance Risk proposal (Past & Present Performance), and Price proposal) shall be submitted on a separate CD-R.
c) Additional Restrictions.
(1) Technical Quality Proposal (Volume II) is limited to 50 pages. Page limitations shall be treated as maximums. If exceeded, the excess pages will not be read or considered in the evaluation of the proposal. If there are more than 45 lines of text on any page, the additional lines will not be evaluated. Headers, footers and page numbers are not included in the line count.
(2) Each page shall be counted except the following:

(3) Questions and Answers. Offerors may submit questions regarding this solicitation to the attention of Ms. Quinella M. Purks, at e-mail: quinella.m.purks.civ@mail.mil, no later than 5 Mar 18 at 12:00 pm Central Standard Time (CST) in the format at Attachment 1 entitled “Question and Answer Template.”
(4) Glossary of Abbreviations and Acronyms. Each volume shall contain a glossary of all abbreviations and acronyms used, and with an explanation for each. Glossaries do not count against the page limitations for their respective volumes.
(5) E-mail correspondence. Offeror’s shall ensure that all e-mails identify in the subject the solicitation number referenced and specific title of reference i.e. “W81K04-18-R-0006- Questions”
d) Complete Proposal. To participate in the source selection evaluation process, an offeror's proposal must be complete. Complete is defined as including all five proposal volumes submitted in accordance with proposal submission instructions and by the date and time stated herein. Proposals determined by the contracting officer as incomplete will not be evaluated. Offerors will receive a notification from the contracting officer that based on its submission of an incomplete proposal, the offeror is not eligible to participate in the source selection process.
2. PROPOSAL FILES.
Offerors shall use the “Proposal Table of Contents” below to organize their electronic proposal files. Each volume of the Offeror’s proposal shall be submitted in electronic form on a separate CD-R as specified in the table. Each document shall be saved to the appropriate CD-R as a separate electronic file using the document titles provided in Solicitation Attachment 2 entitled, “Proposal File Naming Convention.”

3. CONTENT REQUIREMENTS.
All information shall be confined to the appropriate volume/section. For example, only information included in the section labeled “Subfactor 1B - Management and Technical Approach” will be evaluated for "Subfactor 1B - Management and Technical Approach". If "Subfactor 1B - Management and Technical Approach" information is included elsewhere in the proposal, it will not be evaluated as part of the "Subfactor 1B - Management and Technical Approach". The Offeror shall confine its submission to essential matters, sufficient to define the proposal and provide adequate basis for evaluation. Offerors are responsible for including sufficient details, in a concise manner, to permit a complete and accurate evaluation of each proposal. Proprietary information shall be clearly marked.

VOLUME I - BUSINESS PROPOSAL
1. COVER LETTER.
The cover letter shall be a no more than 2-page introduction identifying the Offeror (name, address, telephone and fax numbers, email address, cage code, DUNS number, the solicitation, number, and the contents of the proposal).
2. TABLE OF CONTENTS
Volume I – Business Proposal shall contain a Master Table of Contents referencing the contents in all volumes. The Master Table of Contents will not count against the page limitations for their respective volumes.
3. CONTRACT ADMINISTRATION DATA.
The Offeror shall provide with its proposal, Points of Contact (POCs) for administration of any resulting contract should the Offeror be selected. These POCs shall be available during normal business hours and other than normal business hours. The POC for other than normal business hours will be contacted only in the event of emergencies.
Accordingly, the named individual(s) should possess sufficient corporate authority to effectively deal with emergency situations. The telephone number(s) provided should be functional at all times. If there are different POCs for different conditions, such as weekdays, weekends, holidays, etc., please furnish all and explain as follows:
(a) Offeror’s normal business hours/days.
(b) POCs during normal business hours.
(c) POCs for other than normal business hours.
4. SOLICITATION/CONTRACT/ORDER FOR COMMERCIAL ITEMS (SF 1449) AND AMENDMENTS (SF
30).
Page 1 SF 1449 with signature of a corporate officer authorized to negotiate for the company in Block 30a, acknowledgement of all amendments, (SF 30) with signature in Block 15B, and agreement of 180 days acceptance period.
5. REPRESENTATIONS AND CERTIFICATIONS.
Offerors shall submit one completed copy of all representations and certifications. If the Offeror has completed the Online Representations and Certifications at the System for Award Management (SAM) https://www.sam.gov/portal/public/SAM, submit a printout of the Representations and Certifications.
6. RESPONSIBILITY DETERMINATION.
To assist in determining Offeror/Contractor responsibility IAW FAR Part 9, the Offeror shall explain how they plan to have or obtain adequate financial resources to support any awarded contract and resultant TOs. If the Offeror intends to rely on internally available working/operation capital, evidence of availability must be submitted. If the Offeror plans to rely on financial support from other sources, evidence of availability and the maximum lines of credit that may be available must be identified and must be based upon the inclusion of this contract effort.
Evidence of external funding must be provided in the form of a dated letter from the external source.
Documentation to support the amounts must also be included.
Offerors who fail to provide sufficient information for the USG to make an affirmative responsibility determination may be referred to the Small Business Administration (SBA) for Certificate of Competency consideration USG FAR Part 19.6. To be eligible for award of a contract, the Offeror must receive an affirmative responsibility determination or a Certificate of Competency issued by the SBA.
7. GLOSSARY OF ABREVIATIONS AND ACRONYMS.

VOLUME II – TECHNICAL QUALITY PROPOSAL (Factor 1)
Volume II shall include the Technical Quality Proposal and shall be specific and complete. The technical proposal shall not contain cost/pricing information, e.g. Sub-factor 1C, Scheduling Capabilities, shall not include salary information. The Offeror shall ensure that the Technical Quality Proposal is legible, clear, and cohesive. The USG will evaluate Offeror‘s responses against the Technical Proposal sub-factors defined in the Addendum to 52.212-2.
Using the instructions provided below, provide as specifically as possible the actual methodology you would use for accomplishing/ satisfying these sub-factors. All the requirements specified in the solicitation are mandatory. By your proposal submission, you are representing that your firm will perform all the requirements specified in the solicitation. The Offeror should ensure that their proposal does not merely reiterate the objectives or reformulate the requirements specified in the solicitation.
1. TABLE OF CONTENTS.
The Table of Contents shall contain details which delineate the subparagraphs and sections within that volume.
Technical Quality Proposal must specifically address the following sub-factors:
2. SUB-FACTOR 1A: TRANSITION PLAN. The offeror shall provide, in narrative format, a description of their technical approach on how they intend to execute a timely and effective transition period within 60 days in order to meet PWS requirements by the contract start date by addressing the following: At a minimum, include the following:
(a) Describe your company’s detailed plan to provide a network of sufficient qualified medical specialists to support each of the 66 locations. Substantiate the offeror’s capability to provide a medical network of the magnitude required by the PWS. Describe the process for the prime contractor to obtain signed services contracts within the Medical Specialist Network (MSN).
(b) Describe your company’s plan to provide Medical Specialists with orientation/procedural training on all MEPS policies and procedures for completing a consultation and the training for the proper completion and disposition of MEPS specific forms.
(c) Describe your company’s understanding of industry standards for the credential processing and include a plan for validating all Medical Specialist credentials and medical licenses by the contract start date and how currency will be maintained throughout the life of the contract.
(d) Describe your company’s implementation plan that adequately addresses key milestones in the order that each transition task will be executed and provide the timeline (start and end dates) allocated to each task (e.g., acquiring network providers, human resource actions for hiring key personnel and scheduling staff, validating credentialing and licenses, procedural training, etc.). Include a narrative for each transition task to support the successful accomplishment of the overall transition phase.
3. SUB-FACTOR 1B: MANAGEMENT AND TECHNICAL APPROACH. The offeror shall provide a narrative describing their management and technical approach that addresses their method for accomplishing PWS requirements under the basic contract and resultant task orders. At a minimum, include the following:
(a) Describe your company’s management approach for retaining sufficient numbers of Medical Specialists with minimal turnover at each of the 66 locations, geographically dispersed across the United States including Alaska, Hawaii and Puerto Rico, in order to maintain viable pools of Medical Specialists during the course of contract performance.
(b) Describe your company’s organizational key personnel structure and provide the qualifications required to perform their duties.
(c) Describe how your company will ensure that Medical Specialists will be available to promptly communicate with MEPS Chief Medical Officers on an “as needed” basis for all MEPS locations during their hours of operation.
(d) Describe your company’s plan addressing the prevention of problems with Medical Specialists that pose a threat to the welfare of applicants, significant problems in utilization of services, potential fraudulent/abusive behavior, etc.
(e) Describe how your company will ensure all facilities maintain state and local health, sanitation, and fire prevention standards; licensure, certifications and registries are being maintained.
4. SUB-FACTOR 1C: SCHEDULING CAPABILITIES. The offeror shall provide a narrative describing their scheduling capabilities addressing their method for accomplishing PWS requirements under the basic contract and resultant task orders. Include, at a minimum:
(a) Describe your company’s electronic customer support system to be utilized for scheduling/tracking real-time armed forces applicant appointments.
(b) Describe your company’s plan to ensure your staff and Medical Specialists are available during MEPS work hours to accomplish the scheduled consultations as requested by the MEPS on an “as needed” basis within the timeframes established in the PWS.
(c) Describe your company’s approach for notifying the MEPS of unforeseen appointment cancellations within the MSN due to their non-availability and alternative solution for rescheduling the appointment so that the consultation may occur within the initial PWS required timeframes.
(d) Your company shall identify those consultations, tests or services in Attachment 11 that can receive appointments and results on the same duty day and identify that as Group A.
(e) Your company shall identify all consultations, tests or services in Attachment 11 which can receive appointment and results within one business day of the request and identify that as Group B.
(f) Your company shall identify a list of all consultations, tests or services in Attachment 11 where appointments are likely to be scheduled beyond one business day, with results submission within 15 business days of appointment and identify that as Group C.
5. SUB-FACTOR 1D: WEB BASED SOLUTION. The offeror shall provide a narrative describing their web based solution capabilities addressing their method for accomplishing PWS requirements under the basic contract and resultant task orders. Include, at a minimum
(a) Describe your company’s web based solution to manage the medical referral process.
(b) Describe your company’s understanding of security requirements for your web based solution.
(c) Describe how your company’s web based solution will be used by USG personnel to schedule referrals, view referral status, access consult and ancillary/laboratory results, review applicant appointment scheduling, monthly reports, statistical data (what was ordered by MEPS, type of consult and ancillary or laboratory service), developed training plan, historical data.
(d) Describe how your company’s web based solution will be used to send appointment notifications (initial, reminders, reschedule, and cancellations) to applicants via text, voicemail, and email. The contractor shall ensure that an email notification, by order number, is sent to the appropriate government personnel who submitted the referral with updated appointment information.

VOLUME III – PERFORMANCE RISK PROPOSAL (Past and Present Performance) (Factor 2)
Volume III includes documentation that illustrates the Offeror’s performance on current or past USG or commercial contracts. Volume III may include, no more than, three Performance Assessment Submissions (PASs). Each PAS shall describe work performed under one contract (see contract definition below). Each PAS shall meet the relevancy definition in Addendum to 52.212-2 and performance must have occurred within the last three years prior but not less than six months of the issuance date on the solicitation. The three PASs may include work performed by the prime Offeror and major subcontractor(s) stated in the Offeror’s technical proposal to perform on any resultant contract. A complete Volume III shall include all files described below:
1. TABLE OF CONTENTS. The Table of Contents shall contain details which delineate the subparagraphs and sections within that volume.
2. SUBCONTRACTOR/TEAMING PARTNER CONSENT LETTERS. Performance Risk information concerning subcontractors and teaming partners cannot be disclosed to a private party without the subcontractor’s or teaming partner’s consent. Because a prime Offeror/Contractor is a private party, the USG will need that consent before disclosing subcontractor/teaming partner past and present performance information to the prime Offeror during discussions or negotiations. In an effort to assist the USG in assessing your performance risk, Offerors must have each of their subcontractors and teaming partners complete Attachment 4 entitled “Subcontractor/Teaming Partner Consent letter”. A completed consent form should be submitted as part of your Performance Risk (Past and Present Performance) Volume for each Subcontractor/Teaming Partner identified in your Volume III.
(a) Teaming Arrangements. If a teaming arrangement is contemplated, provide complete information as to the arrangement, including any relevant and recent past performance information on previous teaming arrangements with same partner. Past experience of one team member will be considered past experience of all team members for purposes of evaluation.
(b) A major subcontractor is defined as a subcontractor performing more than 25% of the contract effort.
3. SUBCONTRACTOR/TEAMING PARTNER TABLE. The Offeror shall complete Attachment 4 entitled “Subcontractor/Teaming Partner Table” which describes business arrangements in which the Offeror intends to participate during the performance of any resulting contract/TO. In the table, the Offeror shall use the same designation (i.e. STP-1) for each business partner as they use in their sanitized copy of their technical proposal.
4. PERFORMANCE ASSESSMENT INFORMATION. The offeror shall list all performance assessment submissions in the format referenced on the attached excel spreadsheet, Attachment 5, entitled, “Performance Assessment Information”.
5. PERFORMANCE ASSESSMENT SUBMISSION. The Offeror shall submit performance assessment information only for those persons or entities (Offeror, key personnel and prime subcontractors/teaming partners), who will perform on contracts resulting from this solicitation. The Offeror shall submit no more than three performance assessments on their current or past USG or commercial contracts. The word “contract” is defined as a single contract, purchase order, or a TO on an Indefinite Delivery/Indefinite Quantity (IDIQ) contract. The word “contract”, as defined herein, does not mean basic IDIQ contract, for the purpose of past performance submissions under this solicitation. A single performance assessment submission shall only include one (1) contract, TO or purchase order and MAY not include a combination of contracts, purchase orders, or TOs. If an Offeror submits more than three contracts, the USG will evaluate only the first three listed contracts for recency and relevancy. If an Offeror submits multiple contracts, purchase orders or TOs on one (1) performance assessment submission, then that submission will not be evaluated. Performance assessments shall meet the relevancy definition in Addendum to 52.212-2 and performance must have occurred within the last three years prior but not less than six months of the issuance date on the solicitation. The total number of performance assessment submissions shall include prime and any major subcontractor(s) projected to perform on any resultant contract. Only those subcontractors that meet the definition of a major subcontractor, as defined as performing more than 25% of any resultant contract arising from this solicitation will be used in the Offeror’s performance assessment. The Offeror shall submit a summary of each performance assessment submission, which shall be limited to two pages for each submission. Each submission shall include the following:
(a) Statement of Relevancy: Provide a complete description of the work involved to include dollar value.
Include rationale as to why and how the acquisition meets one of the relevancy definitions for this acquisition.
(b) Reference Customer Information Sheet. The Offeror shall provide a list of all points of contact (POC) to whom Performance Risk Assessment Questionnaires were provided to by the Contractor for evaluation by the USG. Reference information shall be submitted in the format referenced in Attachment 6 entitled “Reference Customer Information Sheet”.
(c) Performance Risk Assessment Questionnaires (PRAQs). The Offeror shall request completion of Attachment 8 entitled “Performance Risk Assessment Questionnaire (PRAQ)” by those persons who have knowledge of the performance quality provided on the contracts listed above. PRAQs should be completed by a member of the program office, administrative contracting office, end users of the service provided, or any other technical or business advisor who has knowledge of the Contractor’s performance. The USG will use the results of submitted questionnaires as a method to evaluate an Offeror’s past performance. The Offeror shall provide a copy of the attached PRAQs to each POC with a letter the same as or similar to that at Attachment 7 entitled “Sample Questionnaire Cover Letter”. Instruct each source to complete and e-mail the entire questionnaire to Ms. Quinella M. Purks at e-mail: quinella.m.purks.civ@mail.mil. Completed Performance Risk Assessment Questionnaires (PRAQs) must be received on or before the date and time established for receipt of proposals.
Questionnaires received after the date established for receipt of proposals will not be considered.
6. GLOSSARY OF ABREVIATIONS AND ACRONYMS. A glossary of all abbreviations and acronyms used, and with an explanation for each. Glossaries do not count against the page limitations for their respective volumes.
VOLUME IV – PRICE PROPOSAL (Factor 3)
1. Volume IV shall include pricing information that will be used to evaluate an Offeror’s total price. Volume IV shall include the Offeror’s proposed prices incorporated into Attachment 9, entitled “Medical Referrals Price Matrix”. The Offeror must include pricing for every specialty on each tab of the spreadsheet, which encompasses five 12-month periods. The Offeror shall only provide that information essential to document or support the Offeror’s price proposal.
2. The Contracting Officer has determined there is a high probability of adequate competition on this acquisition.
Upon examination of the initial proposals, the contracting officer will review this determination and if in the Contracting Officer’s opinion, adequate competition exists no additional cost information will be requested.
However, if at any time during this competition the Contracting Officer determines that adequate competition no longer exists; Offerors may be required to submit information to the extent necessary for the contracting officer to determine the reasonableness of Offerors’ price proposals. Additionally, all information related to pricing shall be included in Volume IV of the proposal only. UNDER NO CIRCUMSTANCES should pricing information be included elsewhere in the proposal. A complete Volume IV shall include the information below:
3. Medical Referrals Price Matrix
Price proposals shall include individual specialty pricing, as well as the total amount of each specialty listed on the spreadsheet. Offerors shall not re-format or remove cells from any of the spreadsheets. Manipulation or alteration of format or cells contained in Attachment 9 may potentially cause your proposal to be rejected.
Instructions for completing Attachment 9 are located at Tab 1 of Medical Referrals Price Matrix spreadsheet.
(a) MEDICAL SPECIALTY CONSULTATION PRICING. (Medical, Laboratory, Ancillary & Dental).
Offerors shall propose prices for each medical specialty identified in the spreadsheet to include each year of the five year ordering period. Offerors shall ensure proposed rates are complete and accurate. The unit and total price for each consultation shall be rounded to the nearest cent. Failure to submit prices for all medical specialties identified in Attachment 9 shall render a proposal ineligible for award. The following CLIN structure will be utilized for specialty consultations:
Ancillary Services - 0001 Laboratory Services - 0002 Medical Consultations - 0003 Dental Consultations - 0004
(b) NO SHOW/LATE CANCELLATION FEE. Offeror will provide their fee in the event a scheduled consultation is cancelled. CLINs 0005 will be utilized for the No Show/Late Cancellation Fee.
(c) CONTRACTOR MANPOWER REPORTING (CMR). Offerors shall not provide separate pricing for annual CMR requirements. CMR appears as Not Separately Priced “NSP” on Attachment 9. CLINs 0006 will be utilized for the CMR.
VOLUME V - SMALL BUSINESS PARTICIPATION PROPOSAL.
Small Business Participation Proposal (Format Example)
All offerors (both large and small businesses) are required to complete a Small Business Participation Proposal.
Offerors should propose the level of participation of small businesses (as a small business prime, joint venture, teaming arrangement, and/or small business subcontractors) in the performance of the acquisition relative to the objectives/goals set forth in the evaluation of this area.

(1) Pursuant to Sections 8(d) of the Small Business Act, a business is considered small for Government procurements if it does not exceed the size standard for the NAICS code that the prime contractor believes best describes the product or services being acquired. In other words, the size of the prime’s suppliers is determined by the applicable NAICS code of their joint venture, teaming partner, or subcontract, which may or may not be the same NAICS code as the one for your prime contract with the Government.
(e) Describe the extent of commitment to use small businesses (e.g., what types of commitments, if any, are in place for this specific acquisition either – small business prime, written contract, verbal, enforceable, non-enforceable, joint venturing, mentor-protégé, etc.) Provide documentation regarding commitments to small business for this effort. Copies of such agreements should be provided as part of your small business participation plan and will not count against the page limitation for this volume.
VOLUME VI – SMALL BUSINESS SUBCONTRACTING PLAN. It is the intent of the Government to ensure that small business concerns have the maximum practicable opportunity to participate in the performance of this requirement.
(a) If the offeror is other than a small business, the offeror shall submit an Individual, Master, or Commercial Subcontracting Plan. The offerors plan must detail a logical and realistic approach in their establishment, oversight, and quality control of subcontracts which comply with FAR 19.702(a)(1) and (2) (Small Business Programs).
(b) Current DOD goals are:
